Long-term neurologic outcome in children with opsoclonus-myoclonus associated with neuroblastoma: a report from the Pediatric Oncology Group.

A retrospective data collection was performed on 29 children diagnosed with neuroblastoma and opsoclonus-myoclonus between 1983-1993 from Pediatric Oncology Group institutions. The aim was to describe neurologic outcome in children with neuroblastoma and opsoclonus-myoclonus. Age at diagnosis ranged from one month to 4 years (median age, 18 months). The duration of opsoclonus-myoclonus symptoms prior to the diagnosis of neuroblastoma ranged from 6 days to 17 months (median duration, 6 weeks). There was a prevalence of low stage disease according to the POG staging system: stage A (n = 18), sta

Parinaud's syndrome in a patient with multifocal glioma.

We describe the clinical and neuroradiological findings in a 63-year-old man with Parinaud's syndrome. Magnetic resonance (MR) imaging showed a mass lesion within the quadrigeminal plate. Additional MR findings included a right frontoparietal subcortical lesion as well as periventricular white matter edema due to acute deterioration of hydrocephalus. On MR, the diagnosis of multifocal glioma was proposed. Neuropathological examination after resection of the supratentorial lesion revealed an oligodendroglioma, grade II.

Parinaud syndrome caused by Bartonella henselae: case report.

The authors report a case of Parinaud syndrome (conjunctivitis with pre-auricular satellite adenitis) caused by Bartonella henselae, the etiologic agent of Cat Scratch Disease. The etiologic assessment of this case was performed by serum indirect immunofluorescence reaction and allowed for a better therapeutics and follow up, avoiding ineffective antibiotics and surgical interventions.

Mechanical properties in dysthyroid myopathy.

Isometric active force and passive load were measured quantitatively in five patients with dysthyroid myopathy during the alert state, to analyze the mechanism of ocular motor disturbance. The results were as follows: (1) the active forces in both agonist and antagonist muscles working in the direction of ocular limitation showed normal values and functioning according to Hering's laws; (2) increasing the passive load of the antagonist muscles consistently demonstrated a close correlation to ocular motor disturbance, but some other muscles also showed abnormal stiffness without clinical signs,

[Retrocollis and oculogyric crisis in association with bilateral putaminal hemorrhages].

A 50-year-old man with a history of hypertension developed right hemiparesis in February, 1985. Four years later, he noted tremor and involuntary extension of the neck (retrocollis) which was aggravated by walking or emotional stimuli. In addition to retrocollis, which was most pronounced upon turning the head to the left or backward, there also was upward deviation of the eyes (oculogyric crisis) and contraction of the left orbicularis oculi muscle. Magnetic resonance imaging study revealed one small old hematoma in the left posterior putamen and two in the right lenticular nucleus (one in th

Brown's syndrome.

Brown's syndrome is a well-recognized clinical disorder of ocular motility manifesting most notably a restriction of active and passive elevation in adduction. The original name, "superior oblique tendon sheath syndrome," is no longer appropriate, since it has been shown that the tissue surrounding the anterior superior oblique tendon is blameless as a restrictive force. "True" and "simulated" as descriptive modifiers should also be discarded, as they relate to the disproven sheath concept. Brown's syndrome occurs as a congenital or acquired, constant or intermittent condition; the common link

Idiopathic hypertrophic pachymeningitis in a patient with a history of diffuse large B cell lymphoma.

A man in his early 70s with a 4-year history of diffuse large B cell lymphoma (DLBCL) was admitted to our hospital with diplopia and achromatopsia. Neurological examination revealed visual impairment, ocular motility disorder and diplopia on looking to the left. Blood and cerebrospinal fluid investigations showed no significant findings. MRI revealed diffusely thickened dura mater and contrast-enhanced structures in the left apical orbit, consistent with hypertrophic pachymeningitis (HP). We performed an open dural biopsy to distinguish the diagnosis from lymphoma. The pathological diagnosis w

Opsoclonus-myoclonus syndrome as a paraneoplastic manifestation of renal cell carcinoma. a case report and review of the literature.

Opsoclonus is a rare but distinctive disorder of ocular motility, characterized by irregular, continual and conjugated chaotic saccades of the eyes. It is increased with eye closure and fixation, and it persists during sleep. Opsoclonus appears more commonly in children and in half of such cases in association with neuroblastoma. In adults the most frequent causes are idiopathic (50%) and paraneoplastic (20%). Among the second group, different types of tumors involving a wide variety of organs have been reported. Opsoclonus when accompanied by other symptoms of central nervous system involveme

[Dissociated ophthalmoplegia of abduction: so-called posterior internuclear ophthalmoplegia in Fisher's syndrome].

The anterior internuclear ophthalmoplegia, a dissociated ophthalmoplegia of adduction secondary to an ipsilateral supranuclear paresis of the internal rectus, represents a well-know clinical picture. The dissociated ophthalmoplegia of abduction, however, sometimes referred to as posterior internuclear ophthalmoplegia, is debated. Such an oculomotor disturbance emerging from an ophthalmoplegic polyneuropathy (Fisher syndrome) is presented and illustrated by oculographic tracings. A secondary finding was a limited range of accommodation. Oculographically, the dissociated ophthalmoplegia of abduc

Depression in Parkinson's disease: prevalence and impact on oculomotor features.

OBJECTIVE: To investigate whether oculomotor features can distinguish Parkinson's disease (PD) patients with depression (PD-D) from those without depression (PDND), and to assess their association with depression severity in PD. METHODS: Seventy-six PD patients were classified into PD-D and PDND groups based on clinical evaluation using the Hamilton Depression Rating Scale (HAMD). Eye movement data were collected using smooth pursuit, overlapping saccade, and anti-saccade tasks. Group differences in oculomotor parameters were analyzed. Receiver operating characteristic (ROC) curves were used

Superior oblique myokymia: diagnostic criteria and long-term outcome.

BACKGROUND: Superior oblique myokymia (SOM) is a rare disorder characterized by brief episodes of high-frequency, low-amplitude monocular nystagmus frequently caused by vascular contact with the trochlear nerve. Comprehensive analyses of SOM symptomatology and long-term progression are limited. No diagnostic criteria are available. METHODS: Adult patients who were diagnosed with SOM between 2008 and 2024 were asked to complete a detailed questionnaire on disease symptomatology, time course, and medication. The impact of the disease on patient-reported functioning was assessed using a Likert s

Patient-Reported Visual Function in Ataxias and Association with Clinical Oculomotor Findings.

Oculomotor dysfunction is common in ataxias, but its impact on patient-reported visual function remains insufficiently understood. To characterize patient-reported visual function across ataxias and assess the functional significance of specific oculomotor abnormalities in spinocerebellar ataxias. Participants were recruited at Massachusetts General Hospital (n = 117): 56 with genetically-defined ataxias (SCA2, SCA3, SCA6, SCA27B, CANVAS), and 61 controls. Patient-reported visual function was assessed using a 13-item subset of the Visual Activities Questionnaire targeting depth perception, vis

Multiple inflammatory cytokines correlate with the vestibular and oculomotor dysfunction in Fabry disease: a prospective, longitudinal study.

OBJECTIVES: Vestibular and oculomotor abnormalities have been widely identified in Fabry disease (FD), with inflammation potentially playing an important role. We aim to investigate the expression of inflammatory cytokines (ICs) in FD patients and their relationship with the vestibular/oculomotor dysfunctions. METHODS: This prospective observational study enrolled 40 FD patients. All 40 patients underwent the visuo-oculomotor examination, and 22 of them received the vestibulo-oculomotor examination. Plasma concentrations of 14 ICs were detected, including interferon-γ, interleukin (IL)-1β, IL

Randomized Controlled Trial of Brock String Vision Therapy for Receded Near Point of Convergence Following Concussion.

The objective of this study was to compare the effectiveness of Brock string vision therapy (BSVT) with usual care (UC) for receded near point of convergence (NPC) following concussion. This study employed a single-blind, randomized controlled trial design involving participants aged 11-30 years within 10 days of a concussion. Participants with receded NPC (i.e., mean NPC ≥5 cm) were randomized to either BSVT or UC. The BSVT group was prescribed BSVT exercises (5 reps, 2× daily, using ∼3 meters string with three colored beads). The UC group was prescribed symptom-specific management strategies

Histopathological Investigation of Progressive Encephalomyelitis with Rigidity and Myoclonus: An Autopsy Case Characterized by Oculomotor Dysfunction and Autonomic Failure.

Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a rare disease associated with the presence of anti-glycine receptor (GlyR) antibodies. We herein report an autopsy case of an 80-year-old man diagnosed with anti-GlyR antibody-positive PERM who presented with symptoms of oculomotor dysfunction and autonomic failure. Despite intensive immunotherapy, the neurological symptoms showed almost no improvement, and the patient succumbed to aspiration pneumonia and bacterial translocation. Postmortem pathology revealed mild inflammatory changes and neuronal loss that were disproportio

A Case of Superior Oblique Palsy without Brown Syndrome Induced by a Dog Bite.

BACKGROUND Pseudo-Brown syndrome is characterized by dysfunction of the superior oblique tendon-trochlear complex. Canine tooth syndrome, which involves superior oblique palsy with pseudo-Brown syndrome, results from damage to the trochlear and superior oblique tendon from dog bites around the eye. This report describes a variant of canine tooth syndrome without pseudo-Brown syndrome following a dog bite around the left upper eyelid. In this case, magnetic resonance imaging (MRI) facilitated early diagnosis and therapeutic intervention. CASE REPORT A 19-year-old man presented with torsional di

Pearls & Oy-sters: CACNA1A-Related Paroxysmal Tonic Upgaze With Ataxia Responsive to Acetazolamide.

A 9-month-old male infant was evaluated for sudden onset of paroxysmal episodes of forced, conjugate upward eye deviation. Extensive in-hospital evaluation including electrophysiology and neuroimaging studies were reassuring against seizures or a structural abnormality. Given the clinical presentation of sudden onset intermittent upward eye deviations, downbeating saccades, associated ataxia, and typical development, a clinical diagnosis of paroxysmal tonic upgaze (PTU) with ataxia was made. Targeted genetic testing of CACNA1A was performed, which revealed a variant of undetermined significanc
