In 1985-86 an ophthalmologist screened 221 patients, ranging in age from six to 81, at Naerlandheimen central institution for mentally handicapped. The screening showed an alarming incidence of eye disease in this group, regardless of the etiology of the mental handicap or the patient's age. Only 44 patients had completely normal eye status without errors of refraction. 40 patients had errors of refraction only and eight had presbyopia only, but otherwise normal findings. 129 had one or more eye anomalies or diseases. Modern lacrimal tract imaging has most recently been enhanced by computerized digital subtraction techniques. Current standard dacryocystography technique includes the injection of a water-soluble agent and the use of topical anesthesia. In this article the dacryocystogram of the normal lacrimal system and those with various types of obstruction are presented.

Forty patients more than 65 years old were anaesthetized with propofol for cataract surgery. Patients were divided into two groups: those intubated after topical oropharyngeal anaesthesia and those intubated after muscle relaxation obtained with suxamethonium. Contact allergy to various components of topical ophthalmological preparations was tested in 27 patients suffering from prolonged or chronic conjunctivitis, keratoconjunctivitis or blepharoconjunctivitis. Our results reveal relevant allergy towards ophthalmological preparations in 44.4% of patients tested. Allergic reactions to topical antibiotics, especially to neomycin (18.5%) and bacitracin (7.4%) was encountered as well as allergy to preservatives such as benzalconium chloride (3.7%). A total of 369 ophthalmological practices were asked to report the number of patients seen within one test week who drove their cars but did not satisfy minimum legal requirements for diurnal visual acuity applicable for applicants for a West German Class 3 driver's license. There were 471 such patients. From this number it can be calculated that approximately 570 000 individuals (+/- 9%) drive cars in the Federal Republic of Germany with inadequate visual acuity. Through investigations which we performed ophthalmological complications in chronic inflammatory intestinal disease are revealed. It could be demonstrated that in most cases ocular symptoms are associated with joint troubles which in their turn depend on the severity of the general disease. Especially in elderly patients with longstanding history it is important to co-operate with representatives of other medical branches in the control of ocular complaints, since there may quickly occur functional deterioration when the eye is also involved.
