Bilateral coexistent calcaneonavicular and talocalcaneal tarsal coalition: a case report.

Tarsal coalition refers to a union of two or more tarsal bones. The union may be fibrous, cartilaginous, or bony. The most common sites of tarsal coalition reported in the literature are the calcaneonavicular, the talocalcaneal, and, less commonly, the talonavicular areas. Bilateral coexistent multiple tarsal coalitions are a rare occurrence. The authors present a case report of a 17-year-old boy with bilateral coexistent calcaneonavicular and talonavicular bars. The diagnosis was established by radiographs and CT scanning. The patient was treated conservatively with immobilization of the foot

Effect of foot orthoses on 3-dimensional kinematics of flatfoot: a cadaveric study.

OBJECTIVE: To test in cadaveric feet the hypothesis that prefabricated foot orthoses will improve arch alignment in flatfoot deformity. DESIGN: Experimental, paired comparisons. SETTING: Biomechanics laboratory. CADAVERS: Nine cadaveric lower-extremity specimens with no abnormalities. INTERVENTIONS: To evaluate the performance of 2 orthoses specimens were tested in 4 combinations: intact, flatfoot, flatfoot with shoe and orthosis 1, and flatfoot with shoe and orthosis 2. To simulate the midstance phase of gait, loads were applied to 5 tendons and an axial load equivalent to two thirds of t

The Dynamic Stabilizing Innersole System (DSIS): the management of hyperpronation in children.

In a study of 50 children, the Dynamic Stabilizing Innersole System (DSIS) was found to decrease the mean resting calcaneal stance position (RCSP) by an average of 6 degrees. A comparison between the neutral calcaneal stance position and the RCSP with the DSIS showed no statistically significant difference between the means for the right or left foot, indicating that the DSIS is capable of returning severe flat foot deformities to their neutral calcaneal stance position. The RCSP with and without the DSIS differed significantly, indicating that the DSIS provides a considerable and statisticall

Monitoring recovery following syndesmosis sprain: a case report.

A sprain to the tibial-fibular syndesmosis often results in prolonged rehabilitation or surgical intervention before recovery occurs. This paper documents gait recovery both before and after surgery for a syndesmosis sprain. Ground reaction force (GRF) data were available before injury and before surgery. Data were also collected every 3 days from 4 days to 4 months after syndesmosis screw removal (8 weeks after surgery). Weightbearing during the stance phase of gait did not approach normal values until approximately 4 months after syndesmosis screw removal. The push-off phase of gait also was

Inferior (obturator) dislocation of the hip in neonates. A complication of treatment by the Pavlik harness.

Two neonates, treated by the Pavlik harness for congenital dislocation of the hip, developed inferior dislocation due to excessive hip flexion. Early recognition of the complication and diminution of the angle of flexion gave a stable relocation of the hip in both patients.

Late acetabular dysplasia following early successful Pavlik harness treatment of congenital dislocation of the hip.

We present the long-term results of Pavlik harness treatment in 61 patients with 74 affected hips. At the beginning of treatment, the hips were either dislocated and reducible or dislocated and irreducible. The average age of patients at follow-up was 12 years (range, 10-16 years). At 3- and 5-year follow-up, all of the hips appeared radiographically normal. However, at latest follow-up, 17% of the hips demonstrated changes in the acetabulum (an upward tilt of the outer portion of the acetabular roof or sclerosis in this area). This study emphasizes the importance of continued follow-up until

Orthotic effect on metatarsophalangeal joint extension. A preliminary study.

The authors evaluated the effect of modified Root orthoses on first metatarsophalangeal joint extension. Motion within the joint was measured dynamically following the insertion of bone markers into the first metatarsal and the hallux. Extension at the joint was greatest when the subjects were barefoot, the mean angle being 62 degrees. Plimsolls restricted extension of the joint to a mean angle of 57 degrees, while orthoses further reduced the angle of metatarsophalangeal joint extension to a mean angle of 55 degrees. The marginal reduction in first metatarsophalangeal joint extension caused b

Enigma of pediatric vertical talus deformity.

Vertical talus is an unusual pediatric deformity that often is unrecognized at birth. Its treatment is necessarily surgical and not typically addressed by the podiatric medical profession. In this case report, the enigmatic deformity is reviewed, along with a detailed photographic assay of useful techniques for its surgical reduction.

Energy cost of walking with flat feet.

A comparative study has been conducted to assess the effects of arch support on oxygen consumption in 20 subjects with flat feet who were generally complaining about fatigue, and also to explore whether their feeling of weariness was objective or not. The resting, walking and final recovery heart rates, blood pressures, and walking oxygen consumption values of the patients with flat feet were measured and calculated and compared to a control group using treadmill and oxygen consumption devices. In stage one the patients did not wear any arch support. Then suitable arch supports were prepared f

Use of the Pavlik harness in treating congenital dislocation of the hip.

The Pavlik harness has made an important contribution to the treatment of congenital dislocation of the hip before walking age. Our experience of 74 cases confirms the good results already reported by several authors; however, four instances of moderate avascular necrosis of the femoral head occurred. The precise indications for the harness and the rules governing its use must be clearly understood. It may be used from the neonatal period up to 6 to 8 months of age for dislocation or subluxation with limitation of hip abduction. Selection of suitable candidates is made more accurate by taking

Therapeutic efficacy of individual head orthoses in infants with positional plagiocephaly.

PURPOSE: Regarding the therapy for positional plagiocephaly, a distinction is made between physiotherapeutic-osteopathic treatment and treatment using individual head orthoses. This retrospective study aimed to compare the outcome of these treatment modalities for correcting positional plagiocephaly in infants. METHODS: From an initial pool of 148 patients, two groups were matched based on age, sex, and Argenta classification. Therapy was either helmet therapy in combination with physiotherapeutic-osteopathic therapy (experimental group/95 patients) or physical therapy alone (control group/28

Utilization of postcranioplasty skull molding caps in the treatment of Apert syndrome.

Patients with Apert syndrome have bilateral coronal craniosynostosis as one of their many distinguishing features. Surgical correction of this deformity is the mainstay of treatment. Abnormal widening of the skull base progresses despite cranioplasty. We describe a patient for whom a skull molding cap (SMC) was used after barrel-stave osteotomy and orbital rim advancement. This successfully restricted widening of her skull base while allowing growth in other dimensions. Utilization of skull molding caps after cranial surgery shows promise in this setting.

Comprehending Optimality of Finger Flexor Tendon Pulley System Using Computational Analysis.

Existing prosthetic/orthotic designs are rarely based on kinetostatics of a biological finger, especially its tendon-pulley system (TPS). Whether a biological TPS is optimal for use as a reference, say for design purposes, and if so, in what sense, is also relatively unknown. We expect an optimal TPS to yield a high range of flexion while operating with lower tendon tension, bowstringing, and pulley stresses. To gain insight into the TPS designs, we present a parametric study which is then used to determine optimal TPS configurations for the flexor mechanism. A compliant, flexure-based computa

BACKGROUND: Rheumatoid arthritis is found in approximately 2 % of the total population in Europe and the peak incidence of the disease is du … ction arthroplasty. The time window in which these interventions are possible should not be missed. Orthotic devices, bandages or even orthopedic shoes provide external support and splinting but do not represent a causal therapy.

The use of amniotic membrane/umbilical cord in first metatarsophalangeal joint cheilectomy: a comparative bilateral case study.

Hallux rigidus is marked by degenerative osteoarthritis of the first metatarsophalangeal (MTP) joint with pain and stiffness in the joint, with first ray plantarflexion and dorsiflexion, swelling and inflammation surrounding the first MTP joint, and reduced sagittal plane motion. Treatment of mild cases of hallux rigidus focuses on conservative, nonsurgical treatments such as rigid orthotic devices, physical therapy, pain relievers, and corticosteroid injections. Hallux rigidus recalcitrant to conservative management can be treated with dorsal cheilectomy to palliate pain at extremes of motion…

Non-surgical treatment of essential tremor: What's in store?

Non-surgical therapy is the mainstay of treatment for Essential Tremor (ET). Traditional pharmacological treatments were found to provide variable tremor reduction, with limited tolerability and high discontinuation rates. This narrative review analyzed studies on adults with ET, focusing on current and emerging non-surgical therapies, with priority given to efficacy and safety. While ACP-711 is a newly investigated positive allosteric modulator (PAM) of GABA-A receptors, other PAMs (SAGE-324 and PRAX-114) have been withdrawn after failure to demonstrate statistically significant effects in cl
