Novel adenosine deaminase 2 mutations in a child with a fatal vasculopathy.

UNLABELLED: Adenosine deaminase 2 (ADA2) deficiency due to CECR1 mutations is a recently defined disorder that involves systemic inflammation and vasculopathy often associated with polyarteritis nodosa. We report on a 5-year-old girl with a severe vasculopathy who carried two novel mutations in CECR1. CONCLUSION: Identification of CECR1 mutations in patients with vasculopathy may lead to earlier diagnosis of ADA2 deficiency.

Group-C meningococcal conjunctivitis in a neonate.

Meningococcal conjunctivitis, although rare, may be complicated by ocular damage and systemic spread. Identification of the infecting organism is important for appropriate management. Meningococcal infection needs treatment of both the patient and the contacts. We report the use of meningococcal group-C vaccine in the prophylaxis of adult contacts of a neonate with meningococcal conjunctivitis. This measure, we believe, has not been reported before in the management of meningococcal conjunctivitis.

[Post-dislocation lesions of the midbrain and pons in neurosurgical patients].

The study of clinico-neurological, electroencephalographical and pathological data in 37 patients with post-dislocation lesions of midbrain and pons Varolii yielded identification of the syndroms of isolated functioning of brainstem-cerebral and brainstem-spinal systems: mesencephalo-pontine, supramesencephalic, unilateral. Clinico-physiological features characteristic of these syndromes are discussed as different from those of acute brain dislocations. Their value in diagnosis and prediction of the death of brain as an organ.

Analysis of corticosteroids in human adrenal tissue by high pressure liquid chromatography.

High pressure liquid chromatography (HPLC) was demonstrated to be a good tool for the separation, identification and quantitation of corticosteroids (CS) extracted from homogenized tissue of adrenal glands and adrenal tumors in patients with hypercorticism. A chromatographic system consisting of Sorbax-SIL and Sorbax-CN columns, organic solvent extraction and a UV detector was used to analyze both more polar and less polar corticosteroids in the adrenal tissue.

How the study of children with rheumatic diseases identified interferon-alpha and interleukin-1 as novel therapeutic targets.

Our studies in children with rheumatic diseases have led to the identification of two of the oldest cytokines, type I interferon (IFN) and interleukin 1 (IL-1), as important pathogenic players in systemic lupus erythematosus (SLE) and systemic onset juvenile arthritis (SoJIA), respectively. These findings were obtained by studying the transcriptional profiles of patient blood cells and by assessing the biological and transcriptional effect(s) of active patient sera on healthy blood cells. We also identified a signature that can be used to promptly diagnose SoJIA from other febrile conditions.

[Identification of GM2-gangliosidosis using an in vitro loading test].

A test system was developed for identification of conventional and atypical forms of GM2-gangliosidoses. The test system was based on a loading test using a culture of the patients skin fibroblasts. Gangliosides, isolated from small samples (several mg) of various tissues, were identified by means of thin-layer chromatography on silica gel plates. In the loading test both unlabelled and 3H-total gangliosides from bovine and rat brain were used. Skin fibroblasts from patients with Tay-Sachs disease, with Sandhoff disease and with unidentified form of leukodystrophy were cultivated. The fibrobla

Preschool-aged wheezing.

Wheezing is a common physical finding in the pediatric age group 0-4 years and can have multiple diverse causes. Infectious causes are the most common culprit and lead to bronchiolitis or preschool asthma. The identification and understanding of these causes is fundamental in the appropriate treatment of our patients. The differentiation and underlying pathologies of these 2 conditions can be confusing and complicated. A systemic review of these conditions attempts to alleviate some of this confusion and tries to provide some clinical guidance for the treatment of these patients.

Identifying organisms directly from BacT/Alert FAN plus blood culture bottles using Vitek MS in a state-wide laboratory network.

Identifying organisms directly from positive blood culture bottles using matrix-assisted laser desorption-ionisation time-of-flight mass spectrometry (MALDI-TOF MS) has many advantages to patients, clinical services, and laboratories. However, few published methods have demonstrated good performance using the current BioMérieux culture bottles and MALDI-TOF system: BacT/Alert FAN plus and Vitek MS. The effect of transporting bottles on test performance has not been assessed for any direct-from-bottle MS method. In this study, 802 positive blood culture bottles were analysed including 234 requi

Family medicine physician identification of obstetric lacerations: a US national survey.

INTRODUCTION AND HYPOTHESIS: We evaluated family medicine obstetric providers' identification and categorization of vaginal delivery lacerations in the USA. We hypothesized that there would be inaccuracy in family medicine physicians' identification of vaginal delivery injuries, similar to our previous studies of midwives and obstetricians (OBs). METHODS: We included clinically active physicians who attended deliveries within 2 years and evaluated their identification and categorization of delivery lacerations using descriptive text and visual images. We asked about their education on this to

Can nailfold videocapillaroscopy images be interpreted reliably by different observers? Results of an inter-reader and intra-reader exercise among rheumatologists with different experience in this field.

Nailfold videocapillaroscopy (VCP) allows non-invasive assessment of the microcirculation. Adequate training in this field is relevant for rheumatologists. There is increasing evidence of the reliability of VCP findings among different readers. Objective: To evaluate inter- and intra-reader agreement of rheumatologists to identify normal images and systemic sclerosis (SSc) patterns on VCP ("early," "active," and "late" proposed by Cutolo et al.). Thirteen rheumatologists with different experience in nailfold VCP received training to standardize reading criteria. They rated 60 VCP images from h

Identification and antibiotic susceptibility of Nocardia farcinica and N. nova in the UK.

Nocardia asteroides has long been recognised as a heterogeneous group of organisms. The description and identification of two new subgroups, N. farcinica and N. nova, in other countries encouraged us to re-examine a collection of N. asteroides isolates from the UK. Of 73 clinical isolates identified as N. asteroides from different parts of England and Wales during 1991-1993, and now subjected to further differentiation tests by the Mycobacterial/Nocardial Reference Laboratory, 15 (20.5%) were identified as N. farcinica based on three out of four characteristics: growth property, acetamide prod

[How the continuous glucose monitoring system can contribute to HbA1c interpretation in type 1 diabetes mellitus?].

The aim of the present study is a literature revision of the contribution of continuous glucose monitoring system (CGMS) in the interpretation of HbA1c levels in type 1 diabetes (T1DM). We performed a revision of the efficacy, benefits, interpretation of methods and experience with this system. The CGMS could be useful in T1DM patients that present great glycemic variability or severe hypoglycemia (mainly nocturnal) and postprandial hyperglycemia. The identification of altered patterns or the glycemic variability allows therapeutic adjustment and improvement of the metabolic control. CGMS is a

Arrhythmias in Patients with Cardiac Amyloidosis: A Comprehensive Review on Clinical Management and Devices.

Cardiac amyloidosis (CA) is a rare but potentially life-threatening disease in which misfolded proteins accumulate in the cardiac wall tissue. Heart rhythm disorders in CA, including supraventricular arrhythmias, conduction system disturbances, or ventricular arrhythmias, play a major role in CA morbidity and mortality, and thus require supplementary management. Among them, AF is the most frequent arrhythmia during CA hospitalizations and is associated with significantly higher mortality, while ventricular arrhythmias are also common and are usually associated with poor prognosis. Early diagno

Pervasive olfactory impairment after bilateral limbic system destruction.

What pattern of brain damage could completely obliterate the sense of olfaction in humans? We had an opportunity to address this intriguing question in Patient B., who has extensive bilateral damage to most of the limbic system, including the medial and lateral temporal lobes, orbital frontal cortex, insular cortex, anterior cingulate cortex, and basal forebrain, caused by herpes simplex encephalitis. The patient demonstrated profound impairments in odor identification and recognition. Moreover, he could not discriminate between olfactory stimuli, and he had severe impairments in odor detectio

Establishment of multiple predictive models for early identification of the dengue cases in outpatient healthcare systems.

Dengue affected countries have responded with many surveillance strategies to detect outbreaks early. However, the early identification for the individual case before a confirmed dengue diagnosis is still lacking, limiting the effectiveness of early intervention strategies. This case-control study involved 206 dengue cases and 824 controls, which selected from patients who visited outpatient clinics at any medical institutions affiliated to the Futian District of Shenzhen city, China from January 1, 2018 to December 15, 2024. All medical visit information was obtained from the "Futian District

Addressing under-registration in Chagas disease mortality: insights from the SaMi-Trop and REDS cohorts.

Chagas disease (ChD) remains a significant public health concern in the Americas, with challenges to accurately assessing its mortality burden due to under-reporting and misclassification. This study aimed to analyze mortality patterns of two cohorts of individuals with ChD-one comprising asymptomatic individuals with positive serology (REDS) and another with patients showing Chagas cardiomyopathy (SaMi-Trop)-to propose a method for estimating the potential under-registration of Chagas-related deaths and to find the factors influencing the identification of ChD as the underlying cause of death

Point-of-care multiplex PCR promises short turnaround times for microbial testing in hospital-acquired pneumonia--an observational pilot study in critical ill patients.

BACKGROUND: The early beginning of an adequate antibiotic therapy is crucial in hospital-acquired pneumonia (HAP), but depends on the results of conventional microbiological diagnostics (cMD). It was the aim of this study to evaluate the performance and turnaround times of a new point-of-care multiplex polymerase chain reaction (mPCR) system for rapid identification of pathogens and antibiotic resistance markers. We assessed the applicability of the system under real-life conditions in critical ill patients with HAP. METHODS: We enrolled forty critical ill patients with clinical signs for HAP
