The Feasibility and Usability of a Ranking Tool to Elicit Patient Preferences for the Treatment of Trigger Finger.

PURPOSE: Shared decision making is an approach where physicians and patients collaborate to make decisions based on patient values. This requires eliciting patients' preferences for each treatment attribute before making decisions; a structured process for preference elicitation does not exist in hand surgery. We tested the feasibility and usability of a ranking tool to elicit patient preferences for the treatment of trigger finger. We hypothesized that the tool would be usable and feasible at the point of care. METHODS: Thirty patients with a trigger finger without prior treatment were recru

Reducing stigma through education to enhance Medication-Assisted Recovery.

The National Alliance for Medication Assisted Recovery has started projects to address the stigma that impacts medication-assisted treatment. The Certified Medication Assisted Treatment Advocate Program trains patients and professionals for advocacy in seminars and conferences. The MARS Project educates (Einstein, Bronx, New York) buprenorphine and methadone patients to dispel stigma and achieve better treatment outcomes. Beyond MARS trains patients nationwide to replicate the MARS Project. Stop Stigma Now will create a national public relations campaign to overcome ignorance and stigma. These

The preferred priorities for care document in motor neurone disease: views of bereaved relatives and carers.

Increasing emphasis is being placed on the need for advanced care planning (ACP) at the end of life. The Preferred Priorities for Care (PPC) document is a patient-held record promoted by the End of Life Care Strategy as an ACP tool to promote discussion and communication amongst patients, family and health care providers. However, little research exists into evaluating its effectiveness or exploring patient and carer views, particularly in non-malignant disease. Because the majority of patients with Motor Neurone Disease (MND) lose verbal communication, early discussion of patients' wishes and

Consumer-oriented interventions for evidence-based prescribing and medicines use: an overview of systematic reviews.

BACKGROUND: Numerous systematic reviews exist on interventions to improve consumers' medicines use, but this research is distributed across diseases, populations and settings. The scope and focus of reviews on consumers' medicines use also varies widely. Such differences create challenges for decision makers seeking review-level evidence to inform decisions about medicines use. OBJECTIVES: To synthesise the evidence from systematic reviews on the effects of interventions which target healthcare consumers to promote evidence-based prescribing for, and medicines use, by consumers. We sought evi

Patients' preferred and retrospectively perceived levels of involvement during decision-making regarding carpal tunnel release.

BACKGROUND: Patient-centered care requires physicians to respond to patients' preferences, including their preferences regarding treatment decision-making. The authors surveyed patients to determine their preoperative preferences and their retrospectively perceived levels of involvement in decision-making for carpal tunnel release, and they attempted to identify factors that affect patient preferences and experiences. METHODS: Seventy-eight patients who underwent carpal tunnel release for carpal tunnel syndrome were requested to indicate their preferred level of involvement preoperatively and

Practical matters, rather than lack of trust, motivate non-participation in a long-term cohort trial.

OBJECTIVE: The objective of this study was to investigate the importance of trust in researchers and other reasons that participating parents, former participants, and non-participants had for participating, or not participating, in a longitudinal cohort study on prediction and development of diabetes in children. STUDY DESIGN: A questionnaire addressing each of these groups, where respondents graded the importance of a set of listed reasons for participating/not participating, was randomly distributed to 2500 families in the All Babies in Southeast Sweden (ABIS) study region with children bo

Information and involvement preferences of women in their 40s before their first screening mammogram.

BACKGROUND: Informed decision making regarding screening mammography is recommended for women in their 40s; however, what information women want and how much involvement in decision making they prefer are not known. METHODS: Surveys were mailed to women aged 40 to 44 scheduled for their first screening mammogram. Women were members of a large New England health maintenance organization and received medical care at a multispecialty practice in the greater Boston area. Outcome measures included information needs and decisional control preferences. RESULTS: Ninety-six women responded. Of 93 ide

Palliative care and orthopedics: what is on the horizon?

Patient-centered palliative care involves effective and empathetic communication between the physician and patient, with the ultimate goal of providing improved quality of life for individuals who have terminal illnesses. Although several nonoperative and operative treatment options are available to improve the patient's quality of life, providing palliative care requires more than technical expertise and knowledge. Orthopedic surgeons need to be supportive facilitators who inform patients of the available treatments, risks, and benefits and incorporate these treatments into the wishes of pati

Navigating Long-Term Co-Creative Research With Young Adults Diagnosed With Cancer: A Qualitative Study.

INTRODUCTION: Patient and public involvement in cancer research is increasingly recognised as essential for developing interventions that are relevant and meaningful to those affected. However, few studies provide detailed accounts of such collaborations or evaluate its impact. This study aimed to evaluate a long-term, co-creative process to refine and improve an internet-delivered intervention focusing on fertility and sexuality following cancer (Fex-Can 2.0). METHODS: This qualitative study evaluates the collaboration between patient research partners and researchers involved in a shared wo

Building and Sustaining Patient Engagement Infrastructure Focused on Overdose Prevention Within a US Veterans Affairs National Research Network.

INTRODUCTION: The U.S. Department of Veterans Affairs (VA) has funded intramural research for over 100 years. A growing number of research funders, including VA, have placed an increasing emphasis on engaging patients in the research process and have invested in engagement infrastructure; however, few accounts are available that detail that infrastructure. This article describes the process and outcomes to plan, build, operate, and evaluate an Overdose Prevention Veteran Engagement (VE) Panel. METHODS: We employed multiple methods to document panel creation and operation, including authors' e

Establishing a Consumer Panel to Inform Health Technology Assessment Processes and Policy: impact and insights from Singapore's experience.

Patient involvement in health technology assessment (HTA) at the organizational level remains relatively underreported in Asia. In 2022, Singapore's Agency for Care Effectiveness (ACE) established a Consumer Panel to provide strategic advice on strengthening patient participation in HTA processes and policy development. The Panel comprises representatives from local patient organizations, collectively covering a range of health conditions and bringing extensive lived experience within Singapore's healthcare system. This Perspective outlines ACE's approach to forming the Panel and highlights it…

What are the characteristics and impacts of a patient-led conference? A qualitative study.

BACKGROUND: Patient engagement is the practice of "meaningful and active collaboration [of patient partners] in governance, priority setting, conducting research and knowledge translation." Patient engagement has been implemented in various settings including clinical, research, and quality improvement, with varying levels of patient contributions and decision-making responsibility. However, little is known about the experiences of patient partners who are in leadership roles in patient-led events. For Patients, By Patients (PxP) is an annual, virtual, patient-led conference that focuses on to

Perceptions, Barriers and Pathways Forward: A Scoping Review of CALD Participation in Australian Clinical Trials.

BACKGROUND: Equitable participation in clinical trials is essential for ensuring that research evidence is scientifically robust, ethically just and representative of the populations it is intended to serve. In Australia, culturally and linguistically diverse (CALD) populations remain underrepresented in clinical trials despite increasing national policy attention to equity and inclusion. This review synthesized empirical evidence and Australian policy and practice documents to examine factors influencing CALD participation in clinical trials. OBJECTIVES: To synthesize evidence on CALD attitu

Understanding shared decision-making among patients with limited health literacy: a realist review protocol.

BACKGROUND: Over a quarter of adults in Europe have limited health literacy (LHL). LHL hampers their ability to find, understand, and implement medical information, resulting in poorer health outcomes. Shared decision-making (SDM), a process that enhances communication and personalised care and encourages patient involvement, can mitigate the challenges individuals with LHL face. However, healthcare providers often struggle to recognise and effectively engage these patients in SDM. This realist review aims to understand how, why, for whom, and under what circumstances SDM does or does not work

Public and patient involvement (PPI) in the design, execution and dissemination of a trial: the BISTRO trial.

BACKGROUND: For people receiving haemodialysis, a balance has to be struck between removing sufficient but not too much fluid during a treatment session and maintaining any remaining kidney function they might have. In the BISTRO trial, this study sought to establish if getting the balance right might be improved by the additional use of bioimpedance, a device that measures body fluid composition to help decide how much fluid to remove during dialysis. Designing and executing this trial, which incorporated complex and repeated trial procedures that would be dependent on participant engagement,

Innovations in randomized clinical trials in Nephrology: from study design to patient's activism.

Current clinical practice is guided by novel robust evidence from randomized clinical trials (RCT). These studies usually follow a rigorous prospective design with pre-defined visits and planned exams during follow-up. In the context of Nephrology, RCTs have been mainly designed to test the efficacy of new therapies in reducing the risk for kidney failure, estimated Glomerular Filtration Rate decline and mortality or cardiovascular events. However, the progress of informatics is reflected in an improvement and expansion of technologies and innovations in present and future RCTs. We here presen

Exploring Diet-Based Treatments for Atrial Fibrillation: Patient Empowerment and Citizen Science as a Model for Quality-of-Life-Centered Solutions.

Atrial fibrillation (AF) is the most common heart rhythm disorder in the Western world. Between the years 2010 and 2019, the global prevalence of AF rose from 33.5 million to 59 million, highlighting the importance of developing equitable treatments for patients. The disease is associated with symptoms such as palpitations, dizziness, fatigue, shortness of breath, and cognitive dysfunction. In addition, AF increases the risk of developing a stroke and heart failure. Despite new insights into risk factors that can lead to the development of AF, the success of current treatments is suboptimal. N
