Sniffing position combined with mouth opening improves facemask ventilation in children with adenotonsillar hypertrophy.

BACKGROUND: This study evaluates the influence of sniffing position combined with mouth opening on the effectiveness of facemask ventilation in paralyzed pediatric patients undergoing adenotonsillectomy during sevoflurane-N(2)O anesthesia. METHODS: After Institutional Ethics Committee approval, 40 children 5-11 years of age who were scheduled for an elective adenotonsillectomy operation were enrolled in this prospective randomized study. After routine monitoring and pre-oxygenation, anesthesia was induced with sevoflurane 8% in a mixture of 50% N(2)O-O(2). Three minutes after the administrati

Acupuncture for Pain Management in Pediatric Patients with Sickle Cell Disease.

Pain management in an acute vaso-occlusive episode for pediatric patients with sickle cell disease (SCD) is challenging and often is focused on opioids, IV fluids, regional anesthesia, ketamine infusions, and non-steroidal anti-inflammatory drugs (NSAIDs). Acupuncture has long been studied as an effective method of pain relief, although the use of acupuncture in pediatric patients with SCD during an acute vaso-occlusive pain episode is vastly understudied. This article provides a review of current research regarding the use of acupuncture as a pain treatment strategy for pediatric patients wit

Histopathology and ultrastructural findings of pediatric sialolithiasis: a brief communication.

Sialolithiasis is a condition in which the salivary gland excretory duct is obstructed due to the formation of calcareous deposits and is uncommon in children compared to adults. The treatment modalities range from a conservative approach involving hydration to a surgical approach. Though several studies have analyzed the sialolith micromorphology structures, studies on pediatric sialoliths remain scarce. This brief communication aims to describe the sialolith micromorphology to understand the mechanism of mineralization and growth of pediatric sialoliths. A 6-year-old Korean female presented

Repeated neonatal sevoflurane exposure causes adolescent anxiety/depression via microglial P2Y12R-Wnt/β-catenin pathway-mediated neurogenesis deficits.

Sevoflurane is the most commonly used inhaled anesthetic in pediatric and obstetric practice. However, early-life multiple sevoflurane expos … l neurogenesis and adolescent emotional dysfunction. These findings have important implications for pediatric anesthesia safety and identify the microglial P2Y12R-Wnt/β-catenin axis as a potential therapeutic target for preventing anesthesia-related neurodevelopmental risks in young children.

An uncommon presentation of a large vulval wart in a child.

INTRODUCTION AND IMPORTANCE: Ano-genital warts are rare presentation in children. The main route of transmission is by sexual intercourse however horizontal transmission, self-inoculation and vertical transmission are also documented. Establishing the route of transmission when there is no evidence of sexual abuse may be difficult in pediatric and female virgins such as in our case report. Surgical excision remains one of the best options for treatment of large warts in pediatrics. To the best of our knowledge, this is the first report of novel case of large genital warts in pediatrics in Tanz

Surgical Repair of a Partial Penile Amputation From a Hair Tourniquet Under Local Anesthesia.

Penile hair tourniquet syndrome is a rare but serious condition necessitating prompt diagnosis. It is defined as a circumferential strangulation of the penis secondary to a hair coil and is predominantly encountered in the pediatric population and seldom in adults. The severity of presentation is governed by the duration of hair strangulation, with prolonged cases often necessitating surgical reconstruction. This case report describes the surgical approach in managing a near-complete penile amputation secondary to a penile hair tourniquet. Due to the patient's comorbidities, the reconstruction

Drug selection for sedation and general anesthesia in children undergoing ambulatory magnetic resonance imaging.

The demand for drug-induced sedation for magnetic resonance imaging (MRI) scans have substantially increased in response to increases in MRI utilization and growing interest in anxiety in children. Understanding the pharmacologic options for deep sedation and general anesthesia in an MRI environment is essential to achieve immobility for the successful completion of the procedure and ensure rapid and safe discharge of children undergoing ambulatory MRI. For painless diagnostic MRI, a single sedative/anesthetic agent without analgesia is safer than a combination of multiple sedatives. The tradi

[Emergency axillary brachial plexus block in a patient allergic to atropine].

Peripheral nerve blocks can provide a valid alternative for anesthetizing patients with allergy to drugs such as atropine that are often used for general anesthesia. We report the case of a 7-year-old boy (physical status ASA II, weight 28 Kg). Relevant medical history included generalized skin rash and breathing difficulties due to bronchospasm after administration of atropine eyedrops. The boy had never undergone skin testing for allergy. With a diagnosis of distal metaphyseal fracture of the right radius, he was proposed for urgent closed reduction and osteosynthesis of the lesion. After pr

Anesthetic Management of an Asian Pediatric Patient With Rubinstein-Taybi Syndrome for Dental Surgery.

Rubinstein-Taybi syndrome (RTS) is a rare genetic disorder that is associated with dysmorphism, moderate to severe intellectual disability and abnormalities involving multiple organ systems. For anesthetists, these patients pose unique challenges in the administration of anesthesia and airway management. We describe the anesthetic conduct of a pediatric patient with RTS coming for dental surgery as a day surgery case and will discuss the anesthetic considerations for such patients.
