Primary Axillary Venous Aneurysm Associated with Local Neurological Complications.

Primary axillary venous aneurysms (VAs) are rare, and there are only a handful of cases reported previously. The patient can be either asymptomatic or symptomatic and can present with local axillary swelling, dilated venules in overlying skin associated with local discomfort, peripheral neuropathy, or pulmonary embolisms. A 30-year-old man presented with a 4-month history of an uncomfortable lump in his left armpit that was associated with paresthesia radiating down to his left forearm and hand. He had no history of arm swelling and had a normal motor neurological examination. The clinical exa

Intra- and Extracranial MR Venography: Technical Notes, Clinical Application, and Imaging Development.

Scientific debate over chronic cerebrospinal venous insufficiency (CCSVI) has drawn attention to venous system involvement in a series of pathologic brain conditions. In the last few decades, the MRI venography (MRV) field has developed a number of valuable sequences to better investigate structural anatomy, vessel patency, and flow characteristics of venous drainage in the intra- and extracranial systems. A brief two-tier protocol is proposed to encompass the study of intra- and extracranial venous drainage with and without contrast administration, respectively. Contrast-enhanced protocol is

Carotid Artery Dissection and Ischemic Stroke After Severe Coughing.

Vigorous coughing leading to internal carotid artery dissection has been described in the adult literature but has not been reported in the pediatric population. We describe an 8-year-old right-handed girl who had a 1-week history of increased work of breathing and vigorous coughing. She initially presented to her pediatrician's office for her respiratory symptoms which improved with steroid therapy and albuterol treatments. However, she developed evolving left-sided motor weakness over the course of 2 days and presented to our pediatric emergency department where she was found to have an isch

Nutcracker syndrome due to left renal vein compression by an aberrant right renal artery.

Classic nutcracker syndrome is caused by left renal vein compression between the superior mesenteric artery and aorta, leading to retrograde venous hypertension associated with such urinary abnormalities as hematuria or proteinuria. We describe a case of symptomatic nutcracker syndrome treated by means of stent placement in which hypertension in the left renal vein was caused by stenosis of this vein compressed by an aberrant right renal artery at a point closer to the inferior vena cava.

Pseudotumor--like presentation of neurobrucellosis.

Brucellosis is bacterial zoonoses. In endemic areas brucellosis can present with clinical features of nearly any neurological illness. Meningitic presentation is most common, with patient presenting with either acute or chronic meningitis. Pseudotumor--like presentation is also documented and accounts for only 4% of cases of neurobrucellosis. Here we are documenting a case of neurobrucellosis with presentation similar to pseudotumor cerebri but with abnormal CSF. This highlights the fact that being a potentially treatable condition brucellosis should always come in the differential diagnosis o

Testicular varicoceles.

A testicular varicocele represents an abnormal degree of venous dilatation of the pampiniform plexus. It is a relatively common condition and may present at scrotal pain and swelling. An association with male subfertility is an area of debate. This article describes the present day radiological criteria and imaging techniques to aid accurate diagnosis of varicoceles. In addition, the role of the interventional radiologist in treating this condition is discussed.

MR contrast agent at high-field MRI (3 Tesla).

Tumor-to-brain contrast after gadolinium administration using MP-RAGE and T1-SE scans in patients with primary and secondary brain tumors was significantly higher at 3 T than at 1.5 T. The subjective assessment of cumulative triple-dose 3 Tesla images obtained the best results in the detection of brain metastases compared with other sequences followed by 1.5 T cumulative triple-dose enhanced images. In macroadenomas of the hypophysis, contrast-enhanced 3 T MRI was superior to standard MRI in the diagnosis of cavernous sinus infiltration and in visualization of cranial nerves within the caverno

Persistent left superior vena cava: a reason for pseudodisplacement of a port catheter.

Port catheters have been increasingly used in recent years. Using one case as an example, we demonstrate how an anatomic variation can easily become a pitfall when inserting a central venous catheter, port catheter, pacemaker or Swan-Ganz catheter. We report a case of persistent left superior vena cava, the most common variation in the thoracic venous system. Since these procedures are increasingly being performed, surgeons, anesthesiologists and radiologists should be aware of the possible anatomic variations.

Pelvic venoablation with ethanol for the treatment of erectile dysfunction due to veno-occlusive dysfunction.

OBJECTIVES: To perform pelvic venoablation with ethanol injection into the deep dorsal vein for the treatment of 10 patients with venogenic erectile dysfunction. This procedure was easily performed without any selective embolization technique. The efficacy and safety of this technique are discussed. METHODS: A total of 10 patients with veno-occlusive dysfunction, severe enough to make vaginal insertion impossible, underwent pelvic venoablation with ethanol. The mean patient age was 67.1 years. Under spinal anesthesia, after the venous leaks were identified by cavernosography, a 20-gauge flexi

Challenging the role of cremasteric reflux in the pathogenesis of varicocele using a new venographic approach.

PURPOSE: Cremasteric or extrafunicular reflux is considered by many a major cause of primary and recurrent varicocele. Therefore, surgical techniques that allow ligation of the intrafunicular and extrafunicular veins are often performed. We evaluated the incidence of cremasteric reflux in patients with primary or recurrent varicocele with a new and simple venographic technique. MATERIALS AND METHODS: A series of 73 patients with primary (54) or recurrent (19) varicocele underwent venography of the left iliac vein while standing and performing Valsalva's maneuver to reveal the possible presenc

Klippel-Trénaunay syndrome: clinical features, complications, and management.

Klippel-Trénaunay Syndrome (KTS) is rare and not well recognized among most physicians, which has lead to inappropriate management and delayed referral in many cases. Presented herein is the largest series ever reported from a single center in Saudi Arabia, comprised of 18 patients seen between 1990 and 1996, whose clinical features, complications, and management are discussed. All 18 patients had large angiomatous navei, hypertrophy of the soft tissue with bone overgrowth in the lower limbs, and extensive lower limb varicosities, which extended to the buttocks and lower abdomen in 2 patients.

[Venography and percutaneous embolization of the spermatic vein with spring coil as treatment of the varicocele in childhood].

The treatment of varicocele is controversial in boys and it is a contributing factor to male infertility. Recently it has been proposed that earlier therapy of a varicocele during this period may improve the prognosis of infertility. We review our experience with the diagnosis and management of left varicocele in 30 pediatric patients 6 to 15 years old. All patients had clinically palpable varicocele. All were managed under local anesthesia by spermatic venography and percutaneous transcatheter embolization of the internal spermatic vein with spring coils. All achieved satisfactory occlusion,

[Treatment of infantile varicocele with percutaneous embolization].

Nine boys, aged 11-13 years with left varicocele grade II or III, were treated by percutaneous embolization of internal spermatic vein at the General Yagüe hospital in the last four years. The procedure was performed under local anesthesia. Percutaneous selective retrograde left spermatic venography was done via the right transfemoral vein. For embolization, sclerosing agents were used in eight cases and coil in one. Follow-up ranged from six months to four years. The control clinical examination and ultrasonographic evaluation showed treatment was successful in eight cases (89%) and persisten

Pseudothrombophlebitis in an adolescent without rheumatic disease. A case report.

Pseudothrombophlebitis syndrome is the occurrence of calf pain and swelling caused by extrinsic compression of the popliteal vessels by an enlarging Baker's cyst or by calf inflammation that occurs as the result of a ruptured Baker's cyst. Few cases of pseudothrombophlebitis syndrome have been reported in patients less than 18 years of age, and nearly all these young patients had juvenile rheumatoid arthritis. Reported here is the case of a 17-year-old male patient without rheumatic disease who presented to the outpatient clinic with a 1-week history of an increasingly painful swelling of the

Budd-Chiari syndrome: successful treatment by percutaneous transluminal angioplasty.

Budd Chiari syndrome is a rare disorder. It is caused by obstruction of the hepatic veins. The prognosis is usually dismal, except in the rare case where the obstruction is due to either webs in the hepatic veins or membranous obstruction of the inferior vena cava. Such cases, more common in the Orient, can be treated by percutaneous transluminal angioplasty. We report a case of Budd Chiari syndrome, due to a membranous obstruction in the right hepatic vein, successfully treated by percutaneous transluminal angioplasty.
