A chromatographic procedure has been developed for the characterization of ACTH- and lipotrophin- (LPH) related peptides in human plasma under acid-dissociating conditions to minimize artifacts of protein binding. Plasma fibronectin (PFN) is associated with cryoglobulins of the polyclonal type. Cryoglobulins from two patients with essential mixed cryoglobulinaemia were analysed immunoelectrophoretically to determine a possible association between PFN and immunoglobulin complexes in the formation of cryoprecipitates. In cryoglobulins from each patient, distinctive alterations in electrophoretic migration patterns of both PFN and immunoglobulin were associated with the formation of a cryoprecipitate. Plasma testosterone was shown to be a very sensitive index of panhypopituitarism in women. In 18 women, testosterone levels were found to be 0.06 +/- 0.04 ng/ml (mean +/- SD; normal, 0.40 +/- 0.10 ng/ml). In all patients, the values were below 2 SD of testosterone levels found in normal women. Dihydrotestosterone concentrations were similarly modified. Such an important decrease in plasma testosterone was probably due to the additional effect produced by the functional suppression of both sources of testosterone precursors: the adrenals and the ovaries.

Effects of mild and severe hypervitaminosis A on retinol transport and clearance were studied in mature ewes. Excessive vitamin A intake was assoicated with high concentrations of vitamin A in plasma and liver, with elevated serum enzymes, with decreased feed consumption and packed cell volume, and with epithelial hyperplasia. Plasma prolactin response to postural change and variation in dietary sodium was evaluated in five normal volunteers and 15 patients with essential hypertension. Values at 0800 hours (11.9 +/- 3.5 ng/ml, mean +/- 1 S.D.) were uninfluenced by the duration of recumbency (10 or 34 hr) and were significantly higher than those obtained at noon (6.9 +/- 3.5 ng/ml, mean +/- 1 S.D., p less than 0.001). The effect of ethanol intoxication and hangover on immunoreactive plasma arginine vasopressin (AVP) concentration was studied in 7 healthy supine men in controlled clinical conditions. In 6 subjects plasma AVP increased above control values at the time of maximal blood ethanol concentration. The indication for the infusion of plasma substitutes has been considerably limited as the result of a series of grave incidents. Prior to each application, differential-therapeutic deliberations must decide on the choice of plasma substitute bringing about the desired success in accordance with its physico-chemical characteristics and clinical values. The type and severity of a probable anaphylactoid incident and its therapy should also be considered.

A simple, rapid GLC method for the determination of lidocaine in plasma is described. After addition of mepivacaine as an internal standard, the plasma sample is deproteinated and centrifuged. The supernate is alkalinized, and the lidocaine and internal standard are extracted into a microvolume of carbon disulfide. By using GLC with a flame-ionization detector, linear concentration-response curves were obtained in the 1-6 microgram/ml range. The method can easily determine plasma concentrations at the 250-ng/ml level when a 1-ml plasma sample is analyzed.

Cryoglobulins were removed from the plasma of two patients with mixed cryoglobulinaemia (IgM(k)-IgG type) associated with severe membrano-proliferative glomerulonephritis and angiitis. The removal of about half the circulating cryo-Igs ('cryo-Igpheresis') was followed by a brisk rebound in cryo-Igs due to the rapid synthesis rate of monoclonal IgM(k) with anti-IgG activity.
