These experiments were designed to test the hypothesis that increases in blood flow to the lower brainstem would be greater than forebrain r … icantly greater flow to the cerebrum (0.86 ml/min/g) than either the medulla (0.52 ml/min/g) or the pons (0.49 ml/min/g). When arterial PCO2 was increased average CBF increased 113%, and a significant linear regression was calculated for arterial PCO2 vs CBF [CBF (ml/min/g) = 0.028 PCO2 (torr) - 0.502]. The distribution of blood flow within the brain w …

Auditory evoked potentials (EVP) are an effective and non-invasive way to assess the functional state of the auditory nerve and auditory sensory pathways of the brainstem. An increase in the latent periods of peaks III, IV, V, as well as a lengthening of the time of central sound conduction of III-V and I-V waves in all newborns with hyperbilirubinemia, indicates a pathology of hearing of central origin with impaired conduction along the auditory pathways at the level of the lower and middle third of the brain pons.

A 63-year-old man, with a history of melanoma and basal cell carcinoma, presented with progressive right-sided facial numbness, vertical diplopia, and headache. Brain MRI revealed leptomeningeal enhancement of multiple cranial nerves and an enhancing mass-like lesion along the anterolateral surface of the pons and midbrain. Subsequent brain biopsy demonstrated the final diagnosis. This case highlights the broad differential diagnosis of leptomeningeal disease, emphasizing the role of specific clinical, laboratory, and imaging cues in guiding clinical reasoning.

Osmotic demyelination syndrome, comprised of central pontine and extrapontine myelinolysis, is an important and potentially fatal complication primarily related to rapid overcorrection of serum sodium leading to devastating neurological symptoms. While traditionally presenting in the pons, we report the case of a 43-year-old female patient who recently underwent a liver transplant and developed extrapontine myelinolysis and subsequently central pontine myelinolysis resulting in irreversible spastic quadriparesis. This rare case highlights the variability of presentation of osmotic demyelinatio…

A 53-year-old man with progressive visual loss in the right eye and diplopia manifested dysfunction of the right optic nerve and the right sixth cranial nerve. MRI revealed a markedly enlarged and tortuous basilar artery, its proximal portion compressing the right sixth cranial nerve at the exit from the pons and its distal portion elevating and compressing the right optic nerve. This is the first report of optic neuropathy and sixth cranial nerve palsy caused by a dolichoectatic basilar artery.

Magnetic resonance imaging (MRI) was performed in two patients in whom a clinical diagnosis of central pontine myelinolysis (CPM) had been made. MRI showed lesions in the pons in both cases about 2 years after the illness, at a time when the spastic quadriparesis and pseudobulbar palsy had recovered. The persisting abnormal signals in CPM are likely to be due to fibrillary gliosis. Persistence of lesions on MRI means that the diagnosis of CPM may be electively, after the acute illness has resolved.

Regional cerebral blood flow (rCBF) was measured in rats to define the autoregulatory response at different levels of hypertension. Mean arterial blood pressure (MABP) was raised with IV metaraminol. rCBF was measured using 14C-iodoantipyrine. Autoregulation was intact in normotensive animals and those with MABP of 152 to 158 mm Hg. At higher pressures, autoregulation was abnormal and heterogeneous. Hyperperfusion was most prominent in cerebellum, parietal gray matter, thalamus, striatum, and pons. These anatomic sites are recognized sites of hypertensive hemorrhage in humans.

Brain-stem death is a state in which brain-stem functions have ceased whereas the hemispheres still have electrical activity. We report a patient with a haematoma over the cerebellum and the pons who showed a preserved electroencephalogram and VEPs whereas all clinical signs of cerebral death was present. Normal VEPs in isolated brain-stem death have not yet been reported. We conclude that in any case of infratentorial lesion, an electroencephalogram should be recorded for the diagnosis of brain death.

Tuberculomas located in the brainstem are rare entities. This article presents a case study detailing the diagnosis and treatment process of a patient with a pontine tuberculoma, as well as a literature review of tuberculomas located in the pons. A 39-year-old male patient showed progressive quadriparesis and deficits in VII, XI, and XII cranial nerves. MRI image initially suggested a pontine astrocytoma. A second biopsy revealed the presence of acid-fast bacilli. Following six months of anti-tuberculous treatment, the patient showed successful recovery.

Body lateropulsion is known to be caused commonly by lateral medullary lesions but rarely by pontine lesions. It is also known to be associated with lesions of the dorsal spinothalamic tract or ascending graviceptive pathways. We herein report the case of a 75-year-old woman presenting with contralateral lateropulsion and cerebellar tremor caused by pons infarction. To our knowledge, this is the first case report of pontine infarction causing both lateropulsion and cerebellar tremor. Our case may be helpful in anatomical studies of ascending graviceptive pathways.

We report a patient admitted with severe dyspnea due to bilateral vocal fold immobility (BVFI), associated with hyponatremia and pneumonia that required emergency tracheotomy. Cerebral magnetic resonance imaging showed a pons lesion compatible with central pontine myelinolysis. Hyponatremia is thought to be caused by hypovolemia. The hyponatremia, and not its correction, is speculated to be the cause of central pontine myelinolysis in this patient. To our knowledge, this is the first case reported of a bilateral vocal fold immobility caused by central pontine myelinolysis induced by hyponatrem…

Reversible posterior leukoencephalopathy syndrome associated with hypertension rarely presents with predominant involvement of the brainstem and sparing of the supratentorial regions. In this study, the clinical and neuroimaging features of a 39-year-old woman with hypertensive encephalopathy and magnetic resonance imaging (MRI) findings localized to pons and bilateral middle cerebellar peduncles were described. Reversible posterior leukoencephalopathy syndrome associated with hypertension rarely shows isolated brainstem and cerebellum involvement, and it is important to be familiar with the l…

Lateral rectus muscle (LRM) palsy due to a nuclear or fascicular sixth nerve lesion is rare as the presenting sign of multiple sclerosis (MS). It is more common to find this palsy in the company of other nearby cranial nerves deficits. Facial numbness in association with a LRM palsy or paresis may go unappreciated and therefore underreported. We report an unusual patient with a LRM palsy, facial anesthesia, ataxia and as the initial manifestation of MS. There was a demyelinating lesion in the pons seen on magnetic resonance imaging (MRI).

We report a neonate with brainstem disconnection. Only five similar cases have previously been described, the longest survival was 7 weeks. The newborn exhibited muscular hypertonia, absent suction, insufficient breathing, and seizures. Magnetic resonance imaging showed a disconnection between the upper pons and the medulla oblongata and cerebellar hypoplasia. The basilar artery was not visible. These neuroimaging findings are clearly different from other midbrain or hindbrain malformations with cerebellar hypoplasia. This pattern and a previously reported autopsy point to a malformation, not …

Hemorrhage in the midbrain and/or pons in patients is often associated with increased metabolism, resulting in hyperthermia. We have recently reported that hyperthermia develops in anesthetized rats following prepontine knife-cuts or procaine microinjections into the midbrain or upper pontine region. It was concluded that the hyperthermia in the animals was caused by the removal of a tonic inhibitor mechanism of heat production that exists in the lower midbrain. The present paper proposes a new hypothesis that the hyperthermia in patients with brainstem hemorrhage is caused by disinhibition of…

A case of locked-in syndrome is described and the literature is reviewed. The disorder consists of quadriplegia and mutism with preserved consciousness. The only remaining voluntary movements are usually vertical eye movements and eyelid blinking. Most cases are caused by basilar artery occlusion resulting in a brainstem infarction in the ventral pons. Health care personnel must be aware that: 1) cognition and some cutaneous sensation is intact and 2) while for most patients the prognosis is poor, there is some chance for partial to almost complete recovery.

A young pregnant woman showing clinical symptoms of numbness, vertigo, diplopia, and amnesia with associated hypernatremia ran a fulminant course. At autopsy, an area or extensive demyelination in the center of the pons was found and a pathological diagnosis of central pontine myelinolysis was made. Since definitive etiology and pathogenesis of the lesion have not been established, we investigated the role of humoral immunity in the pathogenesis of this condition by retrospective immunofluorescent study; the result was negative. Possible factors related to the etiology and pathogenesis of the …

Cranial computed tomography (CT) in a 58-year-old man disclosed enhancing lesions in the medial right temporal lobe and the right pons. Biopsy of the sural nerve was positive for Whipple's disease, and one of the temporal lobe lesions showed unprecedented histological findings. This case, to our knowledge, is unique in its demonstration of cranial CT findings in association with Whipple's disease. It is suggested that a cranial CT scan with the use of intravenous contrast material should be performed in patients presenting with the signs or symptoms of Whipple's disease.

A new finding made possible by the use of the operating microscope in neurosurgery is discussed. Nineteen patients with trigeminal neuralgia have been explored, and in 14 of these vascular compression of the fifth nerve at its entry zone to the pons has been found. Relief of this vascular compression has caused cessation of pain in all these patients. In addition, one patient with hemifacial spasm, and one patient with glosso pharyngeal neuralgia, are reported; each of these also had vascular compression of the appropriate nerve causing the symptoms.

We report the cases of two siblings who died at age 21 and 15 years respectively. Both had optic atrophy, pitressinsensitive diabetes insipidus, and insulin-dependent diabetes mellitus, with onset occurring in early childhood. Although there are now 21 patients from 15 families with this syndrome, this is the first time that necropsy findings have become available. They include the expected atrophy of hypothalamic nuclei and degeneration of the optic nerves, chiasm and tract, as well as a totally unexpected degeneration of the pons and cerebellum.
