Next-generation precision medicine for mood disorders: reproducible blood biomarkers enable objective diagnostics, subtyping, and targeted therapeutics.

Mood disorders are highly prevalent in society, often disabling, and can lead to a reduced healthspan and lifespan, including by suicide. Depression and bipolar disorders are currently deemed to be chronic and/or treatment-refractory in up to half the patients treated. Additionally, depression and bipolar disorders need to be better distinguished, especially upon initial depressive presentations, as treatments can be quite different. Lack of widespread use of objective and/or quantitative information has hampered treatment and prevention efforts. We sought to uncover the underlying genomic and

Precision Medicine Study of Post-Exertional Malaise Epigenetic Changes in Myalgic Encephalomyelitis/Chronic Fatigue Patients During Exercise.

Post-exertional malaise (PEM) is a defining symptom of Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS), yet its molecular underpinnings remain elusive. This study investigated the temporal-longitudinal DNA methylation changes associated with PEM using a structured two-day maximum repeated effort cardiopulmonary exercise testing (CPET) protocol involving pre- and two post-exercise blood samplings from five ME/CFS patients. Cardiopulmonary measurements revealed complex heterogeneous profiles among the patients compared to typical healthy controls, and VO2 peak indicated all patients

"Precision Medicine" and the Failed Search for Binary Brain Sex Differences to Address Gender Behavioral Health Disparities.

Human brain imaging took off in the 1980s and has since flooded the zone in the analysis of gender differences in behavior and mental health. Couched in the aims of "precision medicine," the vast majority of this research has taken a binary approach, dividing participants according to the M/F box at intake and asserting that the sex differences found in neuroimaging will lead to important advances for treating neuropsychiatric disorders. However, the actual findings from this 40-year project have not lived up to its promise, in part because of the over-binarization of sex and general ignorance…

The precision medicine strategy to treat COPD pulmonary traits in clinical practice: The role of N-acetylcysteine.

Chronic obstructive pulmonary disease (COPD) is a progressive lung condition and a leading cause of physical decline and death. COPD prevalence is expected to increase steadily in the coming years, and as a result, the healthcare and social burden of this condition will intensify. In this scenario, a patient-centric approach, the treatable trait (TT) strategy, based on the identification of traits that are clinically relevant, identifiable, monitorable and treatable, has emerged. The TT strategy, which considers behavioral/risk factors, as well as pulmonary and extrapulmonary traits, has shown

Cell and Tissue Specific Metabolism of Nucleoside and Nucleotide Drugs: Case Studies and Implications for Precision Medicine.

Many clinically used antiviral drugs are nucleoside or nucleotide analog drugs, which have a unique mechanism of action that requires intracellular phosphorylation. This dependence on intracellular activation presents novel challenges for the discovery and development of nucleoside/nucleotide analog drugs. Contrary to many small molecule drug development programs that rely on plasma pharmacokinetics and systemic exposures, the precise mechanisms that result in efficacious intracellular nucleoside triphosphate concentrations must be understood in the process of nucleoside/nucleotide drug develo
