Chemokine receptor expression and function in childhood acute lymphoblastic leukemia of B-lineage.

Scanty information is available on chemokine receptor expression and function in childhood B-lineage acute lymphoblastic leukemia (ALL). Thirteen pro-B, 17 early pre-B, 12 pre-B, and 9 B-ALL/Burkitt lymphoma (BL) pediatric cases were tested for CXCR1 to CXCR5 and CCR1 to CCR7 expression. CXCR2, CXCR3, and CXCR4 were expressed in the majority of cases, while the other receptors were variably expressed or absent. CXCR4 mediated chemotaxis of all leukemic cell subtypes. Freshly isolated CCR7(+) early pre-B-ALL cells migrated to CCL19, whereas CCR7(+) pro-B- and pre-B-ALL cells were attracted by C

CAR T-cell therapy in patients with acute lymphoblastic leukemia: a systematic review and meta-analysis.

Chimeric antigen receptor (CAR) T-cell therapy has revolutionized the treatment landscape of relapsed/refractory (R/R) B-cell precursor acute lymphoblastic leukemia (B-ALL), with high remission rates across various CAR T-cell constructs. However, the durability of these responses remains a major challenge, with many patients experiencing relapse after an initial remission. This systematic review and meta-analysis aimed to compare the efficacy and safety of different CAR T-cell constructs across 40 clinical trials, including a total of 1540 R/R B-ALL patients. We assessed the impact of patient

Reliable leukemia detection via transfer-enhanced Bayesian CNNs.

Accurate and early detection of Acute Lymphoblastic Leukemia (ALL) is critical for timely intervention and improved patient outcomes. However, the development of reliable deep learning models for hematological image analysis is challenged by limited data availability, dataset bias, and the need for trustworthy predictions in clinical settings. In this study, we propose a Bayesian deep learning framework that integrates transfer learning, data augmentation, and uncertainty quantification for robust classification of leukemic and healthy lymphocytes from peripheral blood smear images. Three wide

CD3 negative T-ALL - A case study of three cases and review of the literature.

T-cell acute lymphoblastic leukemia/lymphoma (T-ALL/LBL) is a neoplasm of lymphoblasts that is usually TdT-positive and variably expresses CD1a, CD2, cytoplasmic CD3 (cytoCD3), CD4, CD5, CD7, and CD8. Acute undifferentiated leukemia (AUL) is a rare entity and is defined by the expression of not more than one membrane marker for any given lineage. Blasts express HLA-DR, CD34, and/or CD38 and may be positive for terminal deoxynucleotidyl transferase (TdT). Mixed-phenotype acute leukemia (MPAL) is a hematologic malignancy with immunophenotypic co-expression of markers from at least two cell linea

[Omega-3 polyunsaturated fatty acids in pediatric patients with acute lymphoblastic leukemia].

Interest in omega-3 long-chain polyunsaturated fatty acids has increased in recent years. Several studies suggest that supplementation with these fatty acids decreases metabolic risk, plasma triglyceride levels and attenuates the loss of lean body mass, modulating the inflammatory response. In adults with cancer, the results of different studies that have evaluated the effects of the administration of these fatty acids suggest that they can be used as adjuvants during treatment acting as receptor agonists, modulating different molecular pathways, reducing the inflammatory response, increasing

Toward Equitable Risk Classification for All Patients with T-ALL.

In this issue of Blood Cancer Discovery, Newman and colleagues investigated the impact of genetic ancestry on molecular subtypes, genomic alterations, and survival outcomes in a diverse cohort of patients with T-cell acute lymphoblastic leukemia. They demonstrated that the prognostic utility of genomic alterations varied by ancestry, in particular, with NOTCH1 mutations having no prognostic utility in patients of predominantly African ancestry, and their findings highlight the importance of considering genetic ancestry in the risk stratification of patients with T-ALL. [Recent advances in the treatment of acute lymphoblastic leukemia].

Acute lymphoblastic leukemia (ALL) is a hematologic neoplastic disease characterized by monoclonal proliferation of lymphoid progenitor cells that have ceased to differentiate, primarily in the bone marrow. Although outcomes of adult ALL remain poorer than those of pediatric ALL, they have dramatically improved in the past decade with better understanding of prognostic factors and the advent of novel therapies. In particular, BCR-ABL1 tyrosine kinase inhibitors and targeted agents against cell surface antigens (CD19, CD20, and CD22) have revolutionized the treatment of ALL. Clinical adoption o

Successful Resolution of Compartment Syndrome in a Pediatric Patient With B-cell Acute Lymphoblastic Leukemia.

We report a case of a 17-year-old male presenting with acute compartment syndrome (CS) of the lower extremity as the initial manifestation of CRLF2-positive, Ph-like B-cell acute lymphoblastic leukemia (B-ALL), without evidence of leukemic infiltration or hematoma. Emergent fasciotomy was performed, followed by cytoreduction with hydroxyurea to allow wound healing before induction chemotherapy. The patient fully recovered and completed induction without complications. This case highlights the importance of recognizing CS as a rare presenting feature of leukemia, and supports hydroxyurea bridgi

Granulomatous Eccrine Hidradenitis. A New Histopathological Entity?

The term "hidradenitis" designates diseases with inflammatory infiltrate around a sweat gland. Neutrophilic eccrine hidradenitis (NEH) is a reactive disorder usually induced by malignancy or chemotherapeutics, characterized by typically erythematous, edematous papules and plaques on the extremities, trunk, face, and palmar and plantar regions. In histopathology, dense neutrophilic infiltration around the eccrine gland can be seen. In this case, we report a rare case of granulomatous eccrine hidradenitis in a 3-year-old girl with acute lymphoblastic leukemia (ALL), with intense granulomatous in

Rapid in vivo Drug Response Prediction Using Leukemia Cell Grafts in Zebrafish Embryos.

Zebrafish xenotransplantation is a pivotal technique for investigating human cancer pathogenesis and predicting individual drug responses. This document introduces a streamlined protocol (ZefiX) for expanding primary B-cell precursor acute lymphoblastic leukemia (BCP-ALL) patient samples or immortalized cell lines in transiently immunosuppressed zebrafish embryos, utilizing flow cytometry for high-resolution single-cell analysis of treatment responses. Compared to solid tumor engraftments, leukemia cells profit significantly from a morpholino antisense oligonucleotide-based suppression of macr

It's "Blin" a Long Time Coming: The Rise of Blinatumomab in Philadelphia Chromosome-Negative Acute Lymphoblastic Leukemia.

Blinatumomab is a bispecific T-cell engager that has recently transformed front-line treatment for many patients with Philadelphia chromosome (Ph)-negative B-cell acute lymphoblastic leukemia (B-ALL). It was originally studied in relapsed/refractory disease, then moved to targeting measurable residual disease (MRD), and has since been shown to improve outcomes for almost every age group when added to consolidation chemotherapy. The evidence supporting blinatumomab is most robust in adult and standard-risk pediatric age groups, but its benefit in adolescents and young adults and high-risk pedia

Emerging genomic biomarkers in diagnosis and classification of T-cell acute lymphoblastic leukemia.

Contemporary chemotherapy protocols have improved cure rates for children, adolescents, and young adults (CAYA) with T-lineage acute lymphoblastic leukemia (T-ALL) to greater than 80%. Unfortunately, outcomes for CAYA with relapsed and refractory disease, as well as older adults, remain poor. A key goal in the treatment of T-ALL therapy is preventing relapse; however, it is challenging to identify high-risk patients. Recently, several genomic initiatives have identified distinct biological subtypes of T-ALL and have correlated disease biology, including mutational status, transcriptional pheno

The efficacy and safety of nelarabine in relapsed or refractory T-cell acute lymphoblastic leukemia: a systematic review and meta-analysis.

T-cell Acute Lymphoblastic Leukemia (T-ALL) is a subtype of acute lymphoblastic leukemia characterized by the proliferation of abnormal T-cell precursors. Nelarabine, a purine analog, has been approved as a targeted therapy for patients with refractory or relapsed T-ALL. This study aims to evaluate the efficacy and safety of Nelarabine, either as monotherapy or in combination with other therapies, in treating T-ALL. A systematic review and meta-analysis were conducted following PRISMA guidelines. We searched Cochrane CENTRAL, PubMed, and Google Scholar up to August 2024 for studies evaluating

"Just let me go"; When suicidal ideation and goals of care collide in adolescent cancer at the end of life.

Cancer in adolescents and young adults is associated with an increased risk for suicidal ideation (SI). There are no reported pediatric oncology cases describing management of SI during end of life. We present the case of a 14-year-old male with relapsed, high-risk, B-cell acute lymphoblastic leukemia who received a haploidentical stem cell transplant and was suicidal at various points in his treatment. We discuss how to manage acute suicidality in this patient population, the importance of giving a voice to the adolescent patient, the impact of discordant goals of care, and potential preventi

A robust and validated integrated prognostic index for defining risk groups in adult acute lymphoblastic leukemia: an EWALL collaborative study.

Risk stratification is crucial to the successful treatment of acute lymphoblastic leukemia (ALL). Although numerous risk factors have been identified, an optimal prognostic model for integrating variables has not been developed. We used individual patient data from 4 contemporary academic national clinical trials, UKALL14, NILG-ALL10/07, GIMEMA-LAL1913, and PETHEMA-ALL-HR2011, to generate and validate the European Working Group for Adult ALL prognostic index (EWALL-PI), which is based on white blood cell count, genetics, and end of induction minimal residual disease (MRD). Individual patient r
