Primary cardiovascular preventive effect of thiazolidinedione in adults with type 2 diabetes: A nationwide cohort study in Korea.

BACKGROUND: Thiazolidinediones (TZDs) are antidiabetic agents recognized for their effectiveness in secondary cardiovascular prevention; however, their role in primary prevention remains uncertain. This study evaluated the primary cardiovascular preventive effect of TZDs in patients with type 2 diabetes mellitus (T2DM). METHODS: Using the nationwide health claims database, we conducted a cohort study including 1,958,643 adults with T2DM and no prior history of cardiovascular disease. TZD users were compared with dipeptidyl peptidase-4 inhibitor (DPP-4i) users as active comparators. After 1:1 …

A systematic study of mitigation strategies to prevent the emission of malodours from primary settlers in wastewater treatment plants.

Odor emissions from wastewater treatment plants (WWTPs), primarily driven by sulfur-containing compounds, represent a major environmental and operational challenge that requires effective mitigation strategies. The present study assessed the effectiveness of different strategies of sulfur-borne odor mitigation in the primary settler of a pilot-scale WWTP. The most effective technique was the dosing of the precipitating agent FeCl3, which showed removal efficiencies (REs) of 88.5 ± 7.3% for H2S and 63.3 ± 3.5% for methyl mercaptan (CH3SH), using an optimal Fe (III): S molar ratio of 2:1 based o

Impact of Aspirin on Primary Prevention of Cardiovascular Events in Patients with Elevated Lipoprotein(a): A Systematic Review and Meta-analysis.

INTRODUCTION: Elevated lipoprotein(a) (Lp(a)) and Lp(a)-raising genetic variants (e.g. rs3798220) are independent cardiovascular risk factors lacking preventive strategies. Given the prothrombotic properties attributed to high Lp(a), aspirin was hypothesized to confer benefit in primary prevention. We performed a systematic review and meta-analysis to evaluate the impact of aspirin on cardiovascular and bleeding outcomes in this population. METHODS: MEDLINE, Web of Science and CENTRAL were searched (November 2025) for randomized and observational studies assessing aspirin use in primary preve…

Eligibility-Identification Difficulty and Self-Reported Preventive Care Implementation Among Japanese Primary Care Physicians.

BACKGROUND: Primary care is an important setting for prevention, but implementation gaps may arise before clinicians can identify patients eligible for preventive services. We described self-reported preventive care implementation among Japanese primary care physicians and examined whether lower implementation co-occurred with eligibility-identification difficulty. METHODS: We conducted a nationwide web-based cross-sectional survey of physician members of the Japan Primary Care Association from May to June 2025. Respondents reported recommendation consistency for 30 preventive services, diffi

Antiepileptic drugs failed to prevent initial seizures or improve survival outcomes in patients with primary CNS lymphoma.

This study examined the prevalence, risk factors, and prognosis of epilepsy in patients with primary central nervous system lymphoma (PCNSL) and explored the necessity of prophylactic antiepileptic treatment in this population. In this retrospective, observational, single-center study, we analyzed clinical data from PCNSL patients who were diagnosed at our institution between January 2018 and April 2023. The cohort comprised 214 patients with PCNSL (with a median age of 62 years), of which 128 (47.6%) patients were male. Epilepsy was observed in 16.82% (36/214) of the patients, with 9.35% (20/

Preemptive Conversion to mTOR Inhibition to Prevent Primary Cytomegalovirus Infection in Kidney Transplantation with High-Risk Serostatus.

BACKGROUND: Kidney transplantation (KTx) is the treatment of choice for patients with end-stage renal disease. Cytomegalovirus (CMV) infection remains a serious complication of KTx. The most vulnerable patients are naïve recipients (R-) transplanted from a CMV-seropositive donor (D+). Mammalian target of rapamycin inhibitors (mTOR-I) have been shown to have advantages over mycophenolate in terms of CMV infections. In this study, we addressed the effect of preemptive conversion to mTOR-I before ending prophylaxis with valganciclovir in high-risk patients with a D+/R- CMV serostatus. METHODS: T

Nanostructured Lipid Carriers Loaded with Dexamethasone Prevent Inflammatory Responses in Primary Non-Parenchymal Liver Cells.

Liver inflammation represents a major clinical problem in a wide range of pathologies. Among the strategies to prevent liver failure, dexamethasone (DXM) has been widely used to suppress inflammatory responses. The use of nanocarriers for encapsulation and sustained release of glucocorticoids to liver cells could provide a solution to prevent severe side effects associated with systemic delivery as the conventional treatment regime. Here we describe a nanostructured lipid carrier developed to efficiently encapsulate and release DXM. This nano-formulation proved to be stable over time, did not

Number needed to treat with ursodeoxycholic acid therapy to prevent liver transplantation or death in primary biliary cholangitis.

OBJECTIVE: The clinical benefit of ursodeoxycholic acid (UDCA) in primary biliary cholangitis (PBC) has never been reported in absolute measures. The aim of this study was to assess the number needed to treat (NNT) with UDCA to prevent liver transplantation (LT) or death among patients with PBC. METHODS: The NNT was calculated based on the untreated LT-free survival and HR of UDCA with respect to LT or death as derived from inverse probability of treatment weighting-adjusted Cox proportional hazard analyses within the Global PBC Study Group database. RESULTS: We included 3902 patients with a

Thoracoscopic posterior tracheopexy during primary esophageal atresia repair: a new approach to prevent tracheomalacia complications.

BACKGROUND: Esophageal atresia (EA) is usually accompanied by some form of tracheomalacia (TM). During the early phases in life, excessive dynamic collapse of the trachea can cause a wide spectrum of symptoms ranging from mild complaints to apparent life-threatening events (ALTE's) or brief resolved unexplained events (BRUE's). Therapeutic strategies for severe TM include aortopexy to lift the anterior weakened cartilaginous rings or posterior tracheopexy of the floppy membranous tracheal intrusion. In this study, we describe the development of a new approach in which the posterior tracheopexy

Plant-based nutrition for healthcare professionals: implementing diet as a primary modality in the prevention and treatment of chronic disease.

Cardiovascular disease remains the world's leading cause of death. Yet, we have known for decades that the vast majority of atherosclerosis and its subsequent morbidity and mortality are influenced predominantly by diet. This paper will describe a health-promoting whole food, plant-based diet; delineate macro- and micro-nutrition, emphasizing specific geriatric concerns; and offer guidance to physicians and other healthcare practitioners to support patients in successfully utilizing nutrition to improve their health.

Clinical Updates in Women's Health Care Summary: Ischemic Heart Disease: Primary and Preventive Care Review.

Cardiovascular disease is the leading cause of death among women in the United States. Obstetrician-gynecologists often are the sole health care providers for otherwise healthy women. Therefore, they must be aware of risk factors, signs, and symptoms of cardiovascular disease and be prepared to recognize and treat this condition in patients as well as provide referrals when specialized care is indicated. Women with cardiac problems typically present with chest discomfort; however, they also are more likely than healthy women to present with other common symptoms, such as shortness of breath, b

Monoclonal anti-claudin 1 antibodies prevent hepatitis C virus infection of primary human hepatocytes.

BACKGROUND & AIMS: Hepatitis C virus (HCV) infection is a challenge to prevent and treat because of the rapid development of drug resistance and escape. Viral entry is required for initiation, spread, and maintenance of infection, making it an attractive target for antiviral strategies. The tight junction protein claudin-1 (CLDN1) has been shown to be required for entry of HCV into the cell. METHODS: Using genetic immunization, we produced 6 monoclonal antibodies against the host entry factor CLDN1. The effects of antibodies on HCV infection were analyzed in human cell lines and primary human

The use of antiviral drugs in the prevention and treatment of Kaposi sarcoma, multicentric Castleman disease and primary effusion lymphoma.

Kaposi sarcoma-associated herpesvirus [KSHV, also known as human herpesvirus 8 (HHV-8)] is the most recently identified member of the human herpesvirus family. Kaposi sarcoma (KS), primary effusion lymphoma, and multicentric Castleman disease are all associated with KSHV infection. Although the incidence of KS has declined dramatically in areas with access to highly active antiretroviral therapy, it remains the most common AIDS-associated malignancy in the developed world and is one of the most common cancers in developing nations. Current treatment options for KSHV-associated disease are inef
