Polyethylene Glycol microspheres for uterine artery embolization for the treatment of symptomatic uterine fibroids.

BACKGROUND: Uterine fibroids are prevalent benign pelvic tumors, often causing debilitating symptoms that impair quality of life. Uterine fibroid embolization (UFE) is a consolidated minimally invasive treatment option. The purpose of this study is to report our experience with polyethylene glycol microspheres (HydroPearl) in UFE for symptomatic patients. METHODS: This single-center retrospective study evaluated 37 consecutive patients with symptomatic uterine fibroids referred to our institution since November 2016 to February 2020 for UFE with HydroPearl microspheres. All patients included

Disposable peel-away sheaths for pleural space access: a minimally invasive conduit for medical thoracoscopy and chest tube insertion.

The global rise in lung diseases has renewed interest in thoracoscopy, underscoring its growing role in modern clinical practice. As this field evolves, the need for practical, adaptable, and cost-effective tools becomes increasingly important. This report explores the use of peel-away sheaths to access the pleural space during medical thoracoscopy, serving as conduits for both flexible and rigid instruments. Their use enables precise placement of small-bore or surgical chest tubes in targeted anatomical locations and allows for the selection of sheath sizes based on procedural needs. Between

Comparing the Safety and Effectiveness of Ketamine Versus Benzodiazepine/Opioid Combination for Procedural Sedation in Emergency Medicine: A Comprehensive Review and Meta-Analysis.

Procedural sedation is essential in the ED to conduct painful procedures effectively. Ketamine and benzodiazepines/opioids are commonly used, with ketamine providing adequate analgesia and preserving airway muscle tone. However, ketamine is associated with adverse effects while benzodiazepines/opioids can lead to respiratory depression. This study compares the safety and efficacy of ketamine and midazolam/fentanyl. Two search methods were used to identify studies related to our topic, including a database search and a manual search involving screening reference lists of articles retrieved by t

Development of a preoperative neuroscience educational program for patients with lumbar radiculopathy.

Postoperative rehabilitation for lumbar radiculopathy has shown little effect on reducing pain and disability. Current preoperative education programs with a focus on a biomedical approach feature procedural and anatomical information, and these too have shown little effect on postoperative outcomes. This report describes the development of an evidence-based educational program and booklet for patients undergoing lumbar surgery for radiculopathy using a recently conducted systematic review of neuroscience education for musculoskeletal pain. The previous systematic review produced evidence for

Risk of Pain and Gastrointestinal Complaints at 6Months After Elective Abdominal Surgery.

The incidence of chronic postoperative abdominal pain (CPAP) after abdominal surgery is substantial and decreases overall quality of life. One in 3 patients report pain-related interference with mood, sleep, and enjoyment of life and 12% visit the emergency department for pain-related symptoms. Previous studies lack data on preoperative health and pain status or are limited by small patient samples. The aim of this study was to assess risk factors for CPAP and gastrointestinal complaints 6 months after surgery. A prospective cohort study was performed including patients undergoing an elective

Inhaled methoxyflurane for fracture reduction in prehospital extremity trauma: an observational review of HEMS clinical practice.

BACKGROUND: Prehospital manipulation and reduction (M&R) of traumatic limb injuries may be required before transfer to restore alignment, reduce pain and bleeding, and limit secondary neurovascular compromise. In many systems this requires intravenous procedural sedation, which is resource-intensive and not universally available. Inhaled methoxyflurane (IMF) is a portable, self-administered analgesic used within UK HEMS, but its utility as the primary agent for prehospital M&R is not well described. METHODS: We performed a retrospective observational review of trauma patients attended by East

Effects of Virtual Reality on Postoperative Pain Management Following Minimally Invasive Gynecologic Surgery: Randomized Controlled Trial.

BACKGROUND: Postoperative pain and anxiety remain common concerns after minimally invasive gynecologic surgery despite advances in surgical techniques and analgesic strategies. Virtual reality (VR) has been investigated as a potential nonpharmacological intervention for pain management; however, evidence in gynecologic postoperative settings is limited. OBJECTIVE: This study aims to evaluate the efficacy and safety of VR technology compared with standard postoperative analgesia for pain and anxiety management in patients undergoing minimally invasive gynecologic surgery. METHODS: This random

A Novel Fluoroscopic Methodology for Accurate and Precise Lumbar Facet Medial Branch Radiofrequency Ablations: An Educational Report.

Although fluoroscopic-guided lumbar facet medial branch radiofrequency ablations (LFMBRFAs) are commonly performed for lumbar spondylosis, literature describing techniques and methods focused on accuracy and precision is scarce. In this case report, we describe an innovative fluoroscopic methodology to help improve LFMBRFA procedural accuracy and precision. A 74-year-old man with a pertinent history of lumbar spondylosis presented with chronic bilateral low axial back pain underwent successful, and uncomplicated bilateral LFMBRFAs. The patient reported 90% to 100% analgesic relief at 6 months

Preoperative Psychosocial and Psychophysical Phenotypes as Predictors of Acute Pain Outcomes After Breast Surgery.

The severity and impact of acute pain after breast surgery varies markedly among individuals, underlining the importance of comprehensively identifying specific risk factors, including psychosocial and psychophysical traits. In this prospective observational study, women (n = 234) undergoing breast-conserving surgery, mastectomy, or mastectomy with reconstruction completed a brief bedside quantitative sensory testing battery, along with measures of psychosocial characteristics. Postoperative pain severity, impact, and opioid use at 2 weeks were assessed using Brief Pain Inventory and procedure

Rare case of Lingual Nerve Schwannoma Presenting as Submandibular Area Swelling with Parapharyngeal Extension.

This article presents a comprehensive overview of perioperative pain medicine based on recently published recommendations by the Swiss Society of Anaesthesiology and Perioperative Medicine (SSAPM). As it has long been known that improvements in postoperative pain management depend more on organisational structures than on new drugs or techniques, these recommendations focus on the structures and processes of perioperative pain management. The introduction of Acute Pain Services (APS) by anaesthesiology departments in the 1980s marked significant progress. However, APS coverage remains limited

Techniques and strategies for regional anesthesia in acute burn care-a narrative review.

Burn injuries and their treatments result in severe pain. Unlike traumatic injuries that are characterized by a discrete episode of pain followed by recovery, burn-injured patients endure pain for a prolonged period that lasts through wound closure (e.g. background pain, procedural pain, breakthrough pain, neuropathic pain and itch). Regional anesthesia, including peripheral nerve blocks and neuraxial/epidural anesthesia, offers significant benefits to a multimodal approach in pain treatment. A 'regional-first' approach to pain management can be incorporated into the workflow of burn centers t…

Indications for neurosurgical treatment of chronic pain.

Guidelines are presented for the neurosurgical treatment of chronic pain. In these guidelines a distinction is made between the pain of cancer and neurogenic pain. In cancer pain the survival time and the location of the lesion are the important guidelines. Possible procedures are: opioids via CSF route, lesions in nociceptive pathways and PV-PAG stimulation of the thalamus. In neurogenic pain, neurostimulation procedures, tailored to the location of the pain are procedures of first choice. There are however specific indications for other procedures depending on the aetiology of the pain. Caus

Person-Centred Pain Measurement in the ICU: A Multicentre Clinimetric Comparison Study of Pain Behaviour Observation Scales in Critically Ill Adult Patients with Burns.

Pain in critically ill adults with burns should be assessed using structured pain behavioural observation measures. This study tested the clinimetric qualities and usability of the behaviour pain scale (BPS) and the critical-care pain observation tool (CPOT) in this population. This prospective observational cohort study included 132 nurses who rated pain behaviour in 75 patients. The majority of nurses indicated that BPS and CPOT reflect background and procedural pain-specific features (63-72 and 87-80%, respectively). All BPS and CPOT items loaded on one latent variable (≥0.70), except for c…

Image-guided modified Hartel approach for percutaneous balloon compression in treating trigeminal neuralgia: a comparative cohort study.

BACKGROUND: Percutaneous balloon compression (PBC) effectively treats trigeminal neuralgia (TN), though balloon misplacement beyond the Meckel's cave (MC) may reduce long-term efficacy. To optimize the balloon placement success rate (BPSR) within the MC, we developed an image-guided modified Hartel approach and conducted a comparative study to evaluate its potential to improve BPSR and clinical outcomes. METHODS: Patients with TN were treated using either the conventional Hartel approach (n = 39) or the image-guided modified Hartel approach (n = 38). Patients' demographic data, procedural cha

Pain management after thoracic surgery or chest trauma.

Accidental or surgically induced thoracic trauma is responsible for significant pain that can impact patient outcomes. One of the main objectives of its pain management is to promote effective coughing and early mobilization to reduce atelectasis and ventilation disorders induced by pulmonary contusion. The incidence of chronic pain can affect more than 35% of patients after both thoracotomy and thoracoscopy as well as after chest trauma. As the severity of acute pain is associated with the incidence of chronic pain, early and effective pain management is very important. In this narrative revi
