How technology influences the therapeutic process: evaluation of the patient-therapist relationship in augmented reality exposure therapy and in vivo exposure therapy.

BACKGROUND: New technologies have slowly become a part of psychologists' therapeutic office. However, many therapists still have doubts about the possibility of creating a good therapeutic relationship with patients in the presence of technology. AIMS: This study evaluates the development of the therapeutic alliance in individuals with small animal phobia disorder who were treated with Augmented Reality Exposure Therapy or In Vivo Exposure Therapy. METHOD: Twenty-two participants received an intensive session of cognitive behavioural therapy in either a technology-mediated therapeutic contex

The course of the working alliance during virtual reality and exposure group therapy for social anxiety disorder.

BACKGROUND: Psychoanalytic theory and some empirical research suggest the working alliance follows a "rupture and repair" pattern over the course of therapy, but given its emphasis on collaboration, cognitive behavioral therapy may yield a different trajectory. AIMS: The current study compares the trajectory of the working alliance during two types of cognitive behavioral therapy for social anxiety disorder - virtual reality exposure therapy (VRE) and exposure group therapy (EGT), one of which (VRE) has been proposed to show lower levels of working alliance due to the physical barriers posed

[Chronic brucellosis: a rare cause of septic loosening of arthroplasties with high risk of laboratory-acquired infections].

Brucella spp. are a main contributor to morbidity in endemic countries, especially in the Near and Middle East and the Mediterranean. However, in most western industrialised countries, cases are rare. Here, we report the case of a previously unrecognised chronic brucellosis with subsequent laboratory-acquired infection. A 64-year-old female presented to the orthopaedic department with loosening of her knee prosthesis after total knee arthroplasty five years earlier. Bacterial cultures were obtained in a preoperative puncture of the affected joint. About two weeks after replacement of the prost

[Child abuse].

It is estimated that in the Netherlands over 100.000 children per year are victims of child abuse. In more than 50% of the cases of physical abuse there is a trauma of the head and neck area. Therefore, it is likely that (without realizing it) dentists are regularly confronted with cases of child abuse. Dentists have an ethical duty to take positive action in cases of suspected child abuse. They may refer the patient to an oral surgeon, consult the family physician or ask the advice of the 'Advies- en Meldpunt Kindermishandeling' (Advice and Report Centre for Child Abuse). The Dutch Dental Ass

Pediatric residents' perceptions of communication competencies: Implications for teaching.

BACKGROUND: Medical regulatory organizations worldwide require competency in communication skills. Pediatric communication competencies are unique, and little is known about pediatric residents' perceptions regarding these skills. AIM: The purpose of this study was to examine pediatric residents' attitudes about communication skills, their perceptions of the importance of learning 15 specific communication skills relevant to pediatrics, confidence in these skills, and relevant program supports. METHODS: We developed a 47-item cross-sectional questionnaire to study pediatric residents' attitu

Optimising clinical practice in cancer genetics with cultural competence: lessons to be learned from ethnographic research with Chinese-Australians.

Hereditary cancer is about families, and clinicians and genetic counsellors need to understand the cultural beliefs of patients and families about cancer and inheritance. In the light of their kinship patterns Chinese-Australians were chosen for the present study, which aims to determine the explanatory models of inheritance, cancer, and inherited cancer, with a view to identifying the relationship between these culture-specific lay attributions and help-seeking behaviour, and to identify possible barriers to genetic counselling and testing. Qualitative ethnographically informed methodology in

Barriers to home care and social support for an adolescent with disability.

Home care for children with disability has expanded in the last twenty years in order to support the child and family at home and avoid hospitalization or long term care facilities. A case story is presented to show several barriers to home care for a 15 year old boy with Hunter's syndrome and severe disability. The case showed a need for further education and coordination of professionals in order for pediatric home care to be more efficient and supportive in Israel.

Camptomelic dysplasia: a case study.

Camptomelic dysplasia (CMD) is a congenital short-limb skeletal dysplasia characterized by prenatal bowing of the lower limbs in association with additional anomalies of the tracheobronchial tree or genitourinary tract. Perinatal and early neonatal death from respiratory failure is common. Diagnosis and long-term management of the infant with CMD require a coordinated effort among many specialists. This article presents a general overview of skeletal dysplasias and camptomelic dysplasia. It concludes with a case study illustrating the many problems infants with CMD may have and the complex tre

Prenatal care and delivery in an agoraphobic woman. A case report.

Agoraphobia is a psychiatric illness that can cause patients to become functionally homebound. Although the obstetric care of women with agoraphobia has not been described previously, it is likely that the problem is more prevalent than recognized. A pregnant woman initially believed she would be unable to travel to the hospital for delivery. She was given prenatal care and psychotherapy at home; a safe hospital delivery was achieved. The management plan included the in-depth involvement of two physicians, physician preparation for a potential home delivery, patient education about obstetric r

Use of mobile phones in the behavioral treatment of driving phobias.

Results of two case studies are presented to illustrate the use of mobile phones with in vivo exposure treatment of refractory driving phobias. Number of miles driven and subjective ratings of anxiety were recorded during a baseline phase and 8 weeks of treatment involving a total of 24 driving practices. One subject's use of a mobile phone increased the number of miles driven alone, but the second subject made little progress and regressed following removal of the phone. These two cases and our experience with other patients suggest that mobile phones can benefit many individuals whose therap

The treatment of elopement behavior in a retardate using a graduated levels program.

The present study investigated whether a levels program would effectively reduce the amount of elopement (running-away) behavior demonstrated by an adult with profound mental retardation. The subject resided in an Alternative Intermediate Services (AIS) group home. During baseline data collection, he demonstrated an average of 131 elopements per month. After implementation of the levels program, elopements decreased to 0 within 7 months. At the end of the study, the subject's elopements continued to be maintained at a very low rate.

Staff performance: do attitudinal "effectiveness profiles" really assess it?

Performance differences were examined for mental hospital staff with divergent profiles on the Opinions About Mental Illness (OMI) that have been purported to reflect differential staff effectiveness. Two staff groups (N = 40 each) were selected, in pairs, from 19 treatment programs serving adult mental patients to provide extremes of "OMI-effective" vs. "OMI-ineffective" profiles while equating groups on demographic, experiential, and program characteristics that had previously been found to influence OMI scores or staff performance. Actual on-the-floor performance was assessed by direct hour

Responses given by A and B quasi-therapists to differing sexual roles in an intropunitive-neurotic prototype: an analogue study.

Twenty-four male undergraduate students were selected on the basis of their A--B status. Six Ss of each type were assigned randomly to one of two videotaped stimulus conditions, an adequate or an inadequate sexual role variant of an intropunitive neurotic prototype. Two dependent measures, response time and number of confrontations, were used and conceptualized in terms of an approach-avoidance continuum. Differential responding on the basis of the Ss' A--B status was not supported in this study.

Consistent Use of Assistive Devices for Patient Transfer Is Associated With Less Patient-Initiated Violence: Cross-Sectional Study Among Health Care Workers at General Hospitals.

This study investigated whether factors related to bodily contact between health care workers and patients were associated with patient-initiated violence. This cross-sectional study surveyed 496 Danish health care workers measuring patient-initiated violence, use of assistive devices, body mass index, physical exertion, frequency of patient transfers, psychosocial work environment, gender, age, and seniority. Associations were modeled using logistic regression analyses using patient-initiated violence as the outcome. Twenty-five percent of the respondents had experienced physical or verbal vi

Transitions in the lives of young people with complex healthcare needs.

BACKGROUND: The disabled child population now incorporates a group of children and young people with complex healthcare needs, many of whom are supported by medical devices and technologies. Little is known about their experiences and perspectives, particularly in relation to transitions. METHODS: Twenty-eight young people aged between 8 and 19 years old were recruited via Community Children's Nursing Teams. Data were collected by in-depth qualitative interviews and analysed using Grounded Theory principles and procedures. RESULTS: Young people with complex healthcare needs experience multip

Women's views and outcomes of an educational intervention designed to enhance psychosocial support for women during pregnancy.

BACKGROUND: Identification of psychosocial issues in pregnant women by screening is difficult because of the lack of accuracy of screening tools, women's reluctance to disclose sensitive issues, and health care practitioner's reluctance to ask. This paper evaluates if a health professional education program, a new (ANEW) approach, improves pregnant women's ratings of care and practitioner's listening skills and comfort to disclose psychosocial issues. METHODS: Midwives and doctors from Mercy Hospital for Women, Melbourne, Australia, were trained from August to December 2002. English-speaking
