A 20-year-old male undergoing mechanical aortic valve replacement suffered from prosthetic valve endocarditis by Coagulase negative Staphylococci 3 months postoperatively. Echocardiography revealed left ventricular distention, severe paravalvular leakage, valve detachment and large periannular abscess adjacent to the left main coronary artery. Redo-AVR was successfully performed with a cryopreserved aortic allograft by cylinder technique. The orifice of abscess was covered with the aortic wall of the graft. Postoperative echocardiography showed trivial central regurgitation and the abscess cav

A case is described of a 49-year-old man with rheumatic aortic valve disease who developed endocarditis seven years after valvular replacement. Trichosporon beigelii was isolated from the blood, a peripheral thrombus, and the removed prosthesis. After two valve prosthesis replacements and prolonged antifungal therapy, the patient survived for four years, but eventually died as a consequence of multiple septic complications due to the same organism. To the authors' knowledge, this is the longest survival time of any reported case of Trichosporon prosthetic valve endocarditis.

BACKGROUND: The survivorship of tibial metaphyseal cones following revision total knee arthroplasty (rTKA) has been reported. However, literature comparing cone midterm survivorship (from five to 10 years after rTKA) between septic (infection history on the joint) and aseptic revisions is scarce. Therefore, we sought: (1) to assess the overall tibial cone survivorship rate after rTKA; and (2) to compare cone survivorship between septic and aseptic revisions.
METHODS: A multicenter retrospective chart review of 231 patients who underwent rTKA using highly porous titanium tibial metaphyseal con

OBJECTIVE: The aim of the present study was to perform a comparative evaluation of septic and aseptic interface membranes, assessing histological features, inflammatory infiltrate, and expression of inflammatory cytokines.
METHODS: Septic and aseptic interface membranes from 102 patients were examined by histology, histochemistry, and immunohistochemistry (tissue arrays). The cell subpopulations were characterized by quantification of CD3, CD4, CD8, CD20, and CD163 positive cells. Additionally, a semiquantitative evaluation of inflammatory cytokines (TNFalpha, TGF-beta1, IL-1, IL-6, CRP, MMP-

Thirty to fifty percent of patients with infective endocarditis are operated on during the active phase of the disease; this percentage is higher in case of some valvular localizations (aortic), in case of early prosthetic valve endocarditis, in case of some microorganisms (Staphylococcus aureus, gram-negative, fungus, intracellular microorganism). Operative death (at 30 days) is below 10% in native valve endocarditis, close to 50% in early prosthetic valve endocarditis, and below 20% in late prosthetic valve endocarditis. When active infective disease has been healed by medical treatment alon

RATIONALE: Some dental implant failures may result from bacterial contamination at implant insertion, and biomaterial-related infections are difficult to treat. Antibiotic prophylaxis is generally indicated for patients at risk of infective endocarditis or with reduced host response, surgery at infected sites, extensive or prolonged procedures, or implantation of large foreign materials. Various prophylactic regimens, including short-term protocols, have been proposed to reduce post-implant infections. However, concerns about adverse events and antibiotic resistance make prophylactic antibioti

The direct attachment of the osseointegrated prosthesis to the skeleton avoids the inherent problems of socket suspension. It also permits physiological weight bearing, improved range of motion in the proximal joint, and osseoperceptive sensor feedback, enabling better control of the artificial limb by amputees. This article describes the osseointegration program in Sweden based on the use of bone-anchored prostheses for transfemoral amputation rehabilitation. The authors discuss in detail the patient-centered evaluation, surgical technique, and postoperative rehabilitation protocol. The outco

The case presented is of prosthetic aortic valve endocarditis in a 23-year-old male patient, complicated by an inflammatory periprosthetic valvular leak, and fistula formation between the aorta and the right atrium. The fistulous tract was diagnosed intraoperatively. Closure of the fistula was performed successfully in concordance with re-replacement of the aortic valve.

BACKGROUND AND AIMS OF THE STUDY: Prosthetic valve endocarditis (PVE) is an infrequent but serious complication of cardiac valve replacement. PVE is a foreign body infection predominantly based in the sewing cuff of a prosthetic heart valve leading to thromboembolism, ring abscess, paravalvular leakage, and eventual invasion of the myocardium. Mortality rates as high as 75% have been reported. A silver-coated sewing cuff is now available (St. Jude Medical mechanical heart valve SJMR Masters Series with Silzone coating) intended to inhibit the colonization and attachment to the sewing cuff of t

BACKGROUND: Cardiovascular Disease (CVD) is one of the leading causes of mortality and morbidity and significantly impacts the health-related quality of life. Oral infections have been linked to cardiovascular diseases such as thrombosis, cardiac infarction, stroke, and peripheral vascular disease. This study aims to evaluate the effects of oral health on the quality of life in cardiovascular patients.
METHODS: The oral health-related quality of life was measured using the OHIP-14 questionnaire. Demographic information, questions regarding smoke consumption, wearing removable prostheses, nine

The left ventricular assist device (LVAD) is a mechanical pump that supplements or replaces the function of a damaged left ventricle. Although LVAD support is associated with improved survival and quality of life, infectious complications remain a major limitation. The authors examine the epidemiology of LVAD-associated infections and review current diagnostic, treatment, and management strategies. Novel evidence-based approaches to infection prevention remain critical because the number of patients receiving long-term mechanical support continues to burgeon.

BACKGROUND: This video article describes the use of bone-anchored prostheses for patients with transtibial amputations, most often resulting from trauma, infection, or dysvascular disease. Large studies have shown that about half of all patients with a socket-suspended artificial limb experience limited mobility and limited prosthesis use because of socket-related problems. These problems occur at the socket-residual limb interface as a result of a painful and unstable connection, leading to an asymmetrical gait and subsequent pelvic and back pain. In almost all of these cases, a bone-anchored

Staphylococcal infections are one of the main causes of complications in patients with implanted foreign prosthetic material. Implants are associated with a significant reduction of the threshold at which contaminating Gram-positive bacteria, particularly Staphylococcus epidermidis, become infectious and develop a biofilm with phenotypic resistance to almost all antibiotics. A 1000-fold increase in minimal bactericidal levels against most antibiotics except rifampin has been repeatedly observed. Since only removal of the foreign material reverses these phenomena, the clinical challenge consist

BACKGROUND: The Harris-Galante (HG) prosthesis is a first-generation, cementless total hip arthroplasty (THA) prosthesis. Considering the recent increase in the demand for THA in young patients and their life expectancy, a study with a follow-up duration of longer than 20 years in a young population is needed. Therefore, we evaluated the long-term clinical and radiographic results after cementless THA using the HG prosthesis in patients younger than 50 years.
METHODS: A total of 61 THAs performed using the HG with a minimum follow-up of 10 years were included. There were 38 men and 11 women w

BACKGROUND: Prosthetic joint infection (PJI) is a devastating complication of arthroplasty surgery, mostly caused by Gram-positive pathogens, including Staphylococcus aureus and coagulase-negative staphylococci. Multidrug resistance is of major concern in this setting: (a) it can negatively impact outcome, restricting the use of the most effective antimicrobials; (b) it may influence the choice of surgical strategies; and (c) it restrains the therapeutic options to newly labelled antimicrobials with limited experience in PJI.
OBJECTIVES: To provide a comprehensive overview of the clinical imp
