Critical NIH Resources to Advance Therapies for Pain: Preclinical Screening Program and Phase II Human Clinical Trial Network.

Opioid-related death and overdose have now reached epidemic proportions. In response to this public health crisis, the National Institutes of Health (NIH) launched the Helping to End Addiction Long-term InitiativeSM, or NIH HEAL InitiativeSM, an aggressive, trans-agency effort to speed scientific solutions to stem the national opioid public health crisis. Herein, we describe two NIH HEAL Initiative programs to accelerate development of non-opioid, non-addictive pain treatments: The Preclinical Screening Platform for Pain (PSPP) and Early Phase Pain Investigation Clinical Network (EPPIC-Net). Global Polio Eradication - Way Ahead.

In 1988, the World Health Assembly resolved to eradicate poliomyelitis by the year 2000. Although substantial progress was achieved by 2000, global polio eradication proved elusive. In India, the goal was accomplished in 2011, and the entire South-East Asia Region was certified as polio-free in 2014. The year 2016 marks the lowest wild poliovirus type 1 case count ever, the lowest number of polio-endemic countries (Afghanistan, Nigeria and Pakistan), the maintenance of wild poliovirus type 2 eradication, and the continued absence of wild poliovirus type 3 detection since 2012. The year also ma

Are Lockdowns More Effective Than Voluntary Measures in Checking Mobility During a Pandemic?

OBJECTIVE: To evaluate the relative effectiveness of voluntary social distancing appeals versus mandated lockdowns in reducing population mobility during the COVID-19 pandemic, using India as a developing country case study. METHODS: This study analyzed daily mobility data from Google's Community Mobility Reports across six location categories between February 16 and May 31, 2020. A geographic-level regression model was employed to isolate voluntary mobility changes during India's 14-hour voluntary Janta Curfew prior to the mandatory nationwide lockdown. The model controlled for pandemic seve

The Intertwined Epidemics of Obesity and Obstructive Sleep Apnea: A Comprehensive Analysis.

The escalating global prevalence of both obesity and obstructive sleep apnea (OSA) represents a major public health challenge due to their combined impact on individual health outcomes and healthcare infrastructure. This review provides an analytical evaluation of the complex, bidirectional relationship between these conditions. Evidence demonstrates that OSA prevalence rises markedly across progressive body mass index (BMI) categories, reaching up to 90% in individuals with Class III obesity. This association is driven by mechanical mechanisms-such as upper airway pharyngeal fat accumulation

Regulatory considerations for messenger RNA vaccines: A comprehensive review on World Health Organization guidelines on development, production, and clinical evaluation.

The messenger RNA (mRNA) vaccines have revolutionized vaccine development by enabling faster production compared to traditional methods. Comprehensive regulatory requirements for mRNA vaccines are required to ensure uniform quality, safety, and efficacy in this rapidly growing technology. Globally accepted World Health Organization (WHO) guidelines provide broad, adaptable recommendations to all national regulatory authorities, whereas others, such as the European Medicines Agencies (EMA's) draft guideline, provide European Union specific legally binding, highly detailed technical requirements

Women blood donation drive in 21st century: a theme 'so near yet so far' for world blood donors day.

Female participation in blood donation remains disproportionately low worldwide, despite decades of efforts to achieve 100 % voluntary and non-remunerated blood donation. Women contribute only around one-third of donations globally, and in some countries, their involvement is less than 5-10 %. This narrative review explores the complex interplay of physiological, psychological, social, and systemic factors that contribute to this disparity. Key challenges include deferral due to low hemoglobin or physiological states like menstruation and pregnancy, a higher incidence of vasovagal reactions, a

Diagnostic algorithms for tuberculosis in Europe: insights from the European Reference Laboratory Network for Tuberculosis (ERLTB-Net).

The reported poor treatment outcomes for extensively drug-resistant tuberculosis (TB) in the European region highlight the urgent need for effective and context-appropriate diagnostic strategies. While the World Health Organisation (WHO) provides model algorithms, these require adaptation to the European Union/European Economic Area (EU/EEA) context, a setting with low TB incidence but high resources. This viewpoint from the European Reference Laboratory Network for TB (ERLTB-Net) proposes a tailored diagnostic algorithm that prioritises the universal use of WHO-recommended molecular rapid dia

A qualitative analysis exploring barriers and enablers to distribution, delivery, and access to COVID-19 vaccines in Botswana.

INTRODUCTION: The COVID-19 pandemic highlighted pre-existing weaknesses, revealing deep-rooted issues in infrastructure, access, and resource allocation that have long impeded African countries' ability to effectively meet population health needs. It also became evident during the pandemic that there were discrepancies in how vaccines were distributed, delivered and accessed in these countries. We aimed to identify vaccine distribution, service delivery processes and related barriers in Botswana to contextually explore practices that either enhance or hinder access and equity in vaccine distri

Strengthening medication safety infrastructure in U.S. community pharmacies: Lessons from national survey data, Ontario's AIMS, and California policy.

Community pharmacies dispense most medications in the United States and represent a high-frequency point of patient interaction, yet medication-safety policy in this setting remains fragmented compared to inpatient pharmacy settings. Hospitals benefit from established safety reporting infrastructures, while comparable system-level mechanisms for learning from medication errors and near misses are absent in community pharmacy practice. This commentary draws on publicly available U.S. data from the Agency for Healthcare Research and Quality (AHRQ) Community Pharmacy Survey on Patient Safety Cult

Economic burden of cancer in India: Evidence from cross-sectional nationally representative household survey, 2014.

With the ongoing demographic and epidemiological transition, cancer is emerging as a major public health concern in India. This paper uses nationally representative household survey to examine the overall prevalence and economic burden of cancer in India. The age-standardized prevalence of cancer is estimated to be 97 per 100,000 persons with greater prevalence in urban areas. The evidence suggests that cancer prevalence is highest among the elderly and also among females in the reproductive age groups. Cancer displays a significant socioeconomic gradient even after adjusting for age-sex speci

US Public Sexually Transmitted Disease Clinical Services in an Era of Declining Public Health Funding: 2013-14.

BACKGROUND: We examined the infrastructure for US public sexually transmitted disease (STD) clinical services. METHODS: In 2013 to 2014, we surveyed 331 of 1225 local health departments (LHDs) who either reported providing STD testing/treatment in the 2010 National Profile of Local Health Departments survey or were the 50 local areas with the highest STD cases or rates. The sample was stratified by jurisdiction population size. We examined the primary referral clinics for STDs, the services offered and the impact of budget cuts (limited to government funding only). Data were analyzed using SA

Advancing real-world evidence in Brazil: regulatory gaps and global lessons.

Regulatory agencies are increasingly incorporating real-world data (RWD) and real-world evidence (RWE) into decision-making frameworks to complement randomized clinical trials. While some regions, such as the U.S. and EU, have developed structured approaches for RWE use, Brazil's regulatory environment remains comparatively limited. This study examines the status of RWE regulatory integration in Brazil through an analysis of normative documents, institutional publications, and selected case studies, using a comparative policy perspective. Although advances have been made in data standardizatio

Pathway-driven assessment of wastewater contamination in drinking water systems: integrating AI with public health risk.

Wastewater contamination in drinking water systems arises from failures across interconnected components. Globally, an estimated 2.2 billion people lack safely managed drinking water services, with contamination risks persisting in regulated systems. Despite advances in treatment technologies, contamination events continue due to infrastructure deterioration, hydraulic disturbances, and cross-connections, particularly in rapidly urbanizing and resource-constrained regions. A critical limitation of conventional monitoring is its reliance on periodic sampling and laboratory analysis, which often

Epidemiology of public transportation use among older adults in the United States.

BACKGROUND: Attending healthcare appointments and participating in social activities are important for older adults, but these activities are often limited by transportation barriers. Public transportation may bridge these gaps, but little is known about older public transportation users. This study compares the characteristics of older adults who use public transportation to those who do not. DESIGN: Cross-sectional analysis of data from Round 5 of the National Health and Aging Trends Study (NHATS). We identified 5696 urban community dwelling older adults, and calculated national estimates o

Echo Chambers: Bias and Representation in Cardiac Imaging Datasets for Artificial Intelligence.

PURPOSE OF REVIEW: We examined the landscape of publicly available cardiac imaging datasets to assess how their distribution and construction shape bias and equity in cardiovascular AI. RECENT FINDINGS: Across 38 publicly available echocardiography, cardiac magnetic resonance imaging, and cardiac computed tomography datasets, nearly 80% originate from high-income countries, with no public cardiac imaging datasets from Africa or South America. Fewer than two-thirds of these public datasets report any demographic information, and only a small minority are linked to clinical outcomes. Studies re

Progress in recombinant DNA-derived vaccines for Lassa virus and filoviruses.

Developing vaccines for highly pathogenic viruses such as those causing Lassa, Ebola, and Marburg hemorrhagic fevers is a daunting task due to both scientific and logistical constraints. Scientific hurdles to overcome include poorly defined relationships between pathogenicity and protective immune responses, genetic diversity of viruses, and safety in a target population that includes a large number of individuals with compromised immune systems. Logistical obstacles include the requirement for biosafety level-4 containment to study the authentic viruses, the poor public health infrastructure …
