Hyperbaric oxygen therapy in the battlefield.

Hyperbaric oxygen therapy (HBOT) is increasingly being used in a number of areas of medical practice. It is an accepted adjunctive therapy in conditions such as burns, crush injuries, head injuries, spinal cord injuries, reconstruction surgeries, gas poisonings, radiation injuries, various anaerobic and aerobic infections that are commonly encountered in combat. It is being evaluated as a potential therapy for a variety of illnesses such as Post Traumatic Stress Disorder (PTSD) and High Altitude Cerebral Oedema (HACO) that are typically encountered in a combat scenario. The latest hyperbaric c…

Heart is a heavy burden: cardiac toxicity in radiation oncology.

Over the years, radiotherapy has seen continual improvements and has become a standard treatment for most malignant tumors. Cardiotoxicity is a well-known radiotherapy side effect, leading to the risk of long-term morbidity and mortality in cancer survivors. Therefore, minimizing radiotherapy-related cardiotoxicity remains an important challenge in cancer care management. Indeed, multiple dose constraints were proposed for the heart and its substructures. In addition, sparing techniques were developed to reduce the exposure of the heart to ionizing radiation and are currently used in daily cli

FLASH radiotherapy: A new milestone in the field of cancer radiotherapy.

Radiotherapy plays a pivotal role in the control and eradication of tumors, but it can also induce radiation injury to surrounding normal tissues while targeting tumor cells. In recent years, FLASH-Radiotherapy (FLASH-RT) has emerged as a cutting-edge research focus in the field of radiation therapy. By delivering high radiation doses to the treatment target in an ultra-short time, FLASH-RT produces the FLASH effect, which reduces the toxicity to normal tissues while achieving comparable tumor control efficacy to conventional radiotherapy. This review provides a brief overview of the developme

Protective Attitudes toward Occupational Radiation Exposure among Spine Surgeons in Japan: An Epidemiological Description from the Survey by the Society for Minimally Invasive Spinal Treatment.

Background and Objectives: The global trend toward increased protection of medical personnel from occupational radiation exposure requires efforts to promote protection from radiation on a societal scale. To develop effective educational programs to promote radiation protection, we clarify the actual status and stage of behavioral changes of spine surgeons regarding radiation protection. Materials and Methods: We used a web-based questionnaire to collect information on the actual status of radiation protection and stages of behavioral change according to the transtheoretical model. The survey

Intracranial long-term complications of radiation therapy: an image-based review.

BACKGROUND AND PURPOSE: Radiation therapy is commonly utilized in the majority of solid cancers and many hematologic malignancies and other disorders. While it has an undeniably major role in improving cancer survival, radiation therapy has long been recognized to have various negative effects, ranging from mild to severe. In this manuscript, we review several intracranial manifestations of therapeutic radiation, with particular attention to those that may be encountered by radiologists. METHODS: We conducted an extensive literature review of known complications of intracranial radiation ther

[Experience in health prevention work at the accident at the Chernobyl nuclear power plant].

One of the main conditions for successful completion of liquidation tasks in the consequences of the accident at the Chernobyl nuclearpowerplant is of the swipe-effective sanitary and anti-epidemic (preventive) measures. The main directions of medical services in this area include the prevention of acute radiation injuries of personnel and distant effects of ionizing radiation, prevention of epidemics of acute intestinal, respiratory and other infections in the armedforces, pro-prevention of injuries, poisonings, accidents. Medical preventive measures were performed by the Medical Service, whi…

Acute and Chronic Kidney Injury in a Non-Human Primate Model of Partial-Body Irradiation with Bone Marrow Sparing.

The development of medical countermeasures against acute and delayed multi-organ injury requires animal models predictive of the human response to radiation and its treatment. Late chronic injury is a well-known feature of radiation nephropathy, but acute kidney injury has not been reported in an appropriate animal model. We have established a single-fraction partial-body irradiation model with minimal marrow sparing in non-human primates. Subject-based medical management was used including parenteral fluids according to prospective morbidity criteria. We show herein that 10 or 11 Gy exposures

Cardiovascular complications of radiation exposure.

The cardiovascular sequelae of radiation exposure are an important cause of morbidity and mortality following radiation therapy for cancer, as well as after exposure to radiation after atomic bombs or nuclear accidents. In the United States, most of the data on radiation-induced heart disease (RIHD) come from patients treated with radiation therapy for Hodgkin disease and breast cancer. Additionally, people exposed to radiation from the atomic bombs in Hiroshima and Nagasaki, Japan, and the Chernobyl, Ukraine, nuclear accident have an increased risk of cardiovascular disease. The total dose of

[Radiation-induced cardiac disease].

Radiation-induced effects may damage various cardiac structures chronically and cause heart valve dysfunction as well as occlusive lesions of coronary and other arteries exposed to radiation. A 72-year-old woman with a history of radiation treatment after breast cancer was admitted 25 years later with symptoms of tachycardia and acute dyspnea. We found valvular thickening, medium to severe valvular dysfunction and high grade occlusive coronary artery disease in proximal portions. The left subclavian artery also was affected. Surgical treatment was required immediately. Long-term follow-up card

The microlesion concept in HZE particle dosimetry.

High energy, high-Z (HZE) particles are present in high-altitude and high-inclination satellite orbits. Most of the HZE dose above LET = 200 keV/micrometer is due to Fe nuclei. Individual HZE particles can damage several cells adjacent to one another along the particle track in tissue. The outcome has been described as a "microlesion" by D. Grahn. The present study attempts to define conditions for microlesions in specific tissues, to seek biological evidence that microlesions are produced, and to evaluate the microlesion as a potentially useful unit of dose in assessing hazards to spaceworker

Dirty bomb source term characterization and downwind dispersion: Review of experimental evidence.

Dirty bombs are considered one of the easiest forms of radiological terrorism, a form of terrorism based on the deliberate use of radiological material to cause adverse effects in a target population. One U.S. Government official has even described a dirty bomb attack as "all but inevitable". While people in the vicinity of the blast may experience acute radiation effects, people downwind may unknowingly be contaminated by the radioactive airborne particulate and face increased long-term cancer risk. The likelihood of increased cancer risk depends on the radionuclide used and its specific acti

Neuromuscular complications of radiation therapy.

Neuromuscular late effects of radiation therapy (RT) result from radiation fibrosis (RF) of the treated tissues. The clinical manifestations of this dysfunction have been termed radiation fibrosis syndrome (RFS). Any segment of the central and/or peripheral nervous system can be involved, including the brain, spinal cord, nerve roots, plexus, peripheral nerves, and muscles. Often, multiple levels are damaged, resulting in a constellation of findings named for the affected structures (i.e., radiculo-plexo-neuro-myopathy). Accurately diagnosing RFS requires the clinician to understand the basics

Skin injuries in interventional procedures.

Radiation-induced skin injuries to patients in interventional procedures have been reported since the early 1990s, but the number reported is far less than what might be occurring around the world. There is a gross lack of awareness resulting in patients suffering. A case of severe injury observed in multiple percutaneous coronary interventions for chronic total occlusion is reported in this paper. Further, the paper summarises the existing knowledge on radiation dosimetry in interventional procedures, factors involved in skin injury, guidance on detection, and avoidance and management of inju
