BACKGROUND: Vestibuloocular and vestibulospinal reactions following the bilateral loss of vestibular function are designated as Bechterew's phenomenon. In vestibular physiology, under experimental conditions, this phenomenon has played an important role for understanding the mechanisms underlying vestibular compensation. However, clinical reports about this phenomenon in humans are extremely rare.
PATIENTS AND METHODS: Bechterew's phenomen was observed throughout every stage of its development in a patient with consecutive bilateral loss of vestibular function. The disorder was documented by

The subjective visual horizontal and vestibular-evoked myogenic potentials are simple, robust, and reproducible tests of otolith dysfunction that can prove clinically useful diagnostic information in patients with vertigo and other balance disorders. While they appear to have high specificity for unilateral otolith dysfunction, further clinical research will be required to establish their sensitivity.

Modern neurootology has proved that many vertigo patients suffer from central dysequilibrium states. As vertigo patients are very frequent amongst our patient population we have to increase our diagnostic as well as therapeutical efforts. Since 1965, eliciting vestibular evoked cortical potentials has been possible. A method of evoked responses based upon computation of several stimulus responses has been used since then. Besides we are also using the frequency analysis of representative epochs of ongoing vestibular-ocular or retino-ocular reactions has been used since then. For diagnostic pur

Postural stability in patients with bilateral vestibular deficits from aminoglycoside toxicity was characterized by examining their ability to use different sensory cues to maintain balance and by recording their automatic postural responses to sudden translational and rotational (pitch) perturbations of the support surface. We found our patients had increased sway on sensory tests in which either visual or somatosensory cues were altered and were unable to maintain their balance when both visual and somatosensory cues were altered compared to age-matched normal subjects. The amount of vestibu

The pathophysiology of idiopathic spasmodic torticollis is still unknown. In addition to genetic and other factors, one of the possible causes under discussion is a disturbance of the vestibular system. In order to analyse vestibular influences we investigated 40 patients with idiopathic torticollis using electronystagmography, fundus photography, measurement of the subjective visual vertical and posturography. The results of the neuroophthalmological investigations and the measurement of the visual vertical were within the normal range. 53% of the patients examined showed a slight central ves

OBJECTIVES: This study aimed to assess balance deficits in children at risk due to hearing loss or dizziness. The specific objectives were to: (1) measure the prevalence of poor balance in children presenting with these risks; (2) determine whether vestibular test results can predict balance deficits in these children.
BACKGROUND: While vestibular impairment is a known predictor of poor balance, poor balance may also occur for reasons unrelated to vestibular impairment. Poor balance in some children with hearing loss relates directly to the risks to the vestibular system due to its shared ana

BACKGROUND: Cranio-cervico-mandibular disorders can cause disturbances in posture. Stabilimetry measures spatio-temporal variations in the center of body pressure and evaluates the mechanisms of maintenance of balance. The study used a stabilimetric platform to evaluate posture changes in patients with cranio-cervico-mandibular disorders before and after treatment for malocclusion.
METHODS: Between February 1998 and December 2000, 60 patients with cranio-cervico-mandibular disorders were recruited from the Dentistry Clinic of the University of Sassari. Each patient underwent two stabilimetric

OBJECTIVES: Recently, OTOG and OTOGL were identified as human deafness genes. Currently, only four families are known to have autosomal recessive hearing loss based on mutations in these genes. Because the two genes code for proteins (otogelin and otogelin-like) that are strikingly similar in structure and localization in the inner ear, this study is focused on characterizing and comparing the hearing loss caused by mutations in these genes.
DESIGN: To evaluate this type of hearing, an extensive set of audiometric and vestibular examinations was performed in the 13 patients from four families

BACKGROUND: Dizziness is common in older adults, and often presents a diagnostic challenge. Vestibular function declines with age, but the prevalence and diagnostic explanatory value of mild vestibular loss in isolation (i.e., the clinical diagnosis of "presbyvestibulopathy") remains uncertain.
METHODS: We retrospectively analyzed consecutive patients aged ≥ 60 years in a subspecialty dizziness clinic who underwent clinical assessment (history, exam, videonystagmography) and horizontal canal video head impulse testing. We examined (i) the prevalence of vestibular loss (by laterality/degree of

INTRODUCTION: The Romberg Test (RT) is a traditional bedside test of static balance. However, its nonspecific original description and focus on proprioceptive deficits have limited its diagnostic utility for vestibular dysfunction. We used gyroscopic measurements of trunk sway at the body's center of mass, time to fall, and a Subjective Instability score (SI) to gain more detailed insights than the binary outcomes typically used in clinical practice.
OBJECTIVE: To evaluate whether specific patterns of sway and sway perception during the RT and its variants differentiate individuals with vesti

The vestibular system contributes to postural control, eye movements, spatial perception, and autonomic regulation. Its dysfunction causes vertigo, nausea, postural instability, which gradually improve via central plasticity known as vestibular compensation. Experimental vestibular compensation studies commonly use unilateral vestibular neurectomy or chemical/labyrinthectomy models, with vestibular function assessed by vestibulo-ocular reflex (VOR) analysis, vestibular evoked myogenic potentials, and vestibular sensory evoked potentials. However, these methods require specialized equipment. In

The vestibular system of the inner ear provides head motion and orientation information required for maintaining balance and spatial orientation. Each of the five vestibular sensory organs contains type I and type II hair cells (HCs). Type I HCs are particularly notable for their evolutionary adaptability and unique calyceal synapses, in which the vestibular afferent nerve ending envelopes the HC body. In vitro studies indicate that calyceal synapses can transduce signals from HCs to afferents via nonquantal transmission, a mechanism proposed to be faster than conventional bouton synaptic tran

Background: The video head impulse test (vHIT) is a standard tool for assessing semicircular canal function through vestibulo-ocular reflex (VOR) gain and corrective saccades (CS). Although reduced gain accompanied by CS typically indicates vestibular hypofunction, a paradoxical pattern-normal gain with CS-is occasionally observed, and its clinical significance remains unclear. Methods: We retrospectively analyzed 1573 patients who underwent both vHIT and bithermal caloric testing between September 2022 and September 2025. Among them, 1174 subjects with normal bilateral horizontal VOR gains (0

ObjectivesThis study aims to develop a prognostic model, based on the video Head Impulse Test (vHIT)-Vestibulo-Ocular Reflex (VOR) gain, of vestibular loss after an acute Unilateral Vestibulopathy (aUVP) predicting the probability of an objective and significant recovery.DesignData gathered prospectively in patients with aUVP were re-analyzed. After an exploratory cluster analysis, a multivariate PROBIT regression model was used to objectively pinpoint the value of vHIT-VOR gain at baseline that predicts a significant recovery after 10 weeks. Significant recovery was defined as a vHIT-VOR gain

BACKGROUND: Acute vestibular syndrome usually represents either vestibular neuritis (VN), an innocuous viral illness, or posterior circulation stroke (PCS), a potentially life-threatening event. The video head impulse test (VHIT) is a quantitative measure of the vestibulo-ocular reflex that can distinguish between these two diagnoses. It can be rapidly performed at the bedside by any trained healthcare professional but requires interpretation by an expert clinician. We developed machine learning models to differentiate between PCS and VN using only the VHIT.
METHODS: We trained machine learni

PURPOSE: When a dizzy patient with episodic vertigo has an abnormal caloric and a normal video head impulse test (vHIT), this caloric-vHIT dissociation provides vital information for a diagnosis of Ménière's disease (MD). Endolymphatic hydrops (EH), a histological marker of MD, is hypothesized to be involved in the caloric-vHIT dissociation in MD through hydropic duct distension of the horizontal semicircular canal (SC). This study was designed to determine the impact of EH on the function of horizontal SC during caloric stimulation.
METHODS: Caloric test and vHIT were used to evaluate the fu

Background. The vestibular system is vital for gaze stability via the vestibulo-ocular reflex, which generates compensatory eye motion in the direction opposite to head motion. Consequently, individuals with peripheral vestibular loss demonstrate impaired gaze stability that reduces functional capacity and quality of life. To facilitate patients' compensatory strategies, two classes of gaze stabilization exercises are often prescribed: (i) transient (eg, ballistic) and (ii) continuous. However, the relative benefits of these two classes of exercises are not well understood. Objective. To quant

The mechanisms of vestibular migraine and motion sickness remain unknown. The aims of this study were to determine interictal vestibular dysfunction in migraineurs according to associated dizziness/vertigo and motion sickness, and to find out whether impaired uvulonodular inhibition over the vestibular system underlies the vestibular symptoms and signs by measuring tilt suppression of the vestibulo-ocular reflex (VOR). One hundred and thirty-one patients with migraine [65 with vestibular migraine (MV), 41 with migrainous dizziness (MD), and 25 with migraine only (MO)] and 50 normal controls un
