Refractive surgery for the glaucoma suspect.

PURPOSE OF REVIEW: The aim of this study was to provide an update on perioperative considerations in the evaluation and management of patients undergoing primary corneal and intraocular refractive procedures who are at risk for progressive glaucomatous optic neuropathy. RECENT FINDINGS: The necessity of a comprehensive baseline assessment prior to refractive procedures with structural and functional testing, along with documentation of preoperative intraocular pressure (IOP) measurements, is highlighted in recent literature. Substantiation of the increased risk of postoperative IOP elevation

IMI Prevention of Myopia and Its Progression.

The prevalence of myopia has markedly increased in East and Southeast Asia, and pathologic consequences of myopia, including myopic maculopathy and high myopia-associated optic neuropathy, are now some of the most common causes of irreversible blindness. Hence, strategies are warranted to reduce the prevalence of myopia and the progression to high myopia because this is the main modifiable risk factor for pathologic myopia. On the basis of published population-based and interventional studies, an important strategy to reduce the development of myopia is encouraging schoolchildren to spend more

Retinal control of lens-induced astigmatism in chicks.

PURPOSE: Astigmatism is a refractive error due to meridional differences in refractive powers of lens or cornea. The resulting failure to focus image points in a single plane causes blurred vision at all distances. In this study, using an animal model of lens-induced astigmatism, we tested the hypothesis that induced astigmatism is due to processing of astigmatic retinal image information by the brain, which causes distorted growth in the anterior segment via centrifugal neural projections. METHODS: To induce astigmatism, +4.00DS/-8.00DC crossed-cylinder-lens goggles were affixed over the rig

Relation of accommodative microfluctuation with dry eye symptoms in short tear break-up time dry eye.

PURPOSE: To investigate accommodative microfluctuations (AMFs) and visual function in short tear break-up time (BUT)-type dry eye (DE) and non-DE subjects. METHODS: This prospective comparative study included 48 volunteers with DE symptoms (mean age 34.8 ± 5.5 years, age range 25-42 years) and 73 without DE symptoms (mean age 30.6 ± 4.7 years, age range 25-42 years). The eyes were divided into two groups: (1) DE group with DE symptoms and BUT ≤ 5 s and (2) non-DE group without DE symptoms and BUT > 5 s. We excluded eyes with Schirmer score ≤ 5 mm and positive keratoconjunctival epithelial dam

A physiological model to measure effects of age on lenticular accommodation and spherical aberration in chickens.

PURPOSE: To assess physiological accommodative function of intact intraocular lenses, as measured by focal length changes, in chickens of various ages. METHODS: Eyes of white leghorn chickens, aged 0 days (hatchlings), 7 days, 14 days, 6 weeks, 1 year, and 2 years, were enucleated and the backs of the globes, except for the ciliary nerve and ganglion, were removed. The ciliary nerve and ganglion were suctioned into the tip of a suction electrode, and lenses were optically scanned before, during, and after accommodation. Accommodation was elicited by delivery of 30-Hz electrical pulses. RESUL

Effects of brief periods of unrestricted vision on the development of form-deprivation myopia in monkeys.

PURPOSE: To characterize the temporal integration properties of the mechanisms responsible for form-deprivation myopia (FDM), the effects of brief daily periods of unrestricted vision on the degree of FDM were investigated in infant monkeys. METHODS: Starting at approximately 3 weeks of age, unilateral form deprivation was produced in 24 infant rhesus monkeys by securing a diffuser spectacle lens in front of one eye and a clear, zero-powered lens in front of the fellow eye. During the treatment period (17 +/- 2 weeks), six infants wore the diffuser lenses continuously. In the other experiment

Good visual function in posterior microphthalmos.

Posterior microphthalmos is a rare condition in which the anterior segment is normal in size and configuration, but the posterior segment is reduced in size; this results in axial hyperopia and retinal folding. Patients have decreased vision that is caused by posterior segment abnormalities, high refractive error, and amblyopia. We present a case of posterior microphthalmos in which retinal function was relatively intact and visual loss was believed to be primarily caused by refractive error and amblyopia. After treatment, the child's visual acuity and school performance improved. This case em

The influence of interactions between accommodation and convergence on the lag of accommodation.

Several models of myopia predict that growth of axial length is stimulated by blur. Accommodative lag has been suggested as an important source of blur in the development of myopia and this study has modeled how cross-link interactions between accommodation and convergence might interact with uncorrected distance heterophoria and refractive error to influence accommodative lag. Accommodative lag was simulated with two models of interactions between accommodation and convergence (one with and one without adaptable tonic elements). Simulations of both models indicate that both uncorrected hypero

Consensual accommodation.

Consensual accommodation, like other consensual responses, may be a useful diagnostic indicator. The consensual interocular lag (direct accommodative response minus consensual accommodative response) was measured under a variety of accommodative conditions in four normal subjects, one of whom later underwent cryosurgery for a retinal tear. There was no consensual interocular lag when subjects were orthophoric , but when slightly exophoric at near distances a small lag does occur. A subject with large exophorias at near showed a more complex accommodative pattern. The consensual interocular lag

The AC/A ratio and convergence in hypermetropia corrected with spectacles or contact lenses.

Some hypermetropes, when corrected with contact lenses, appear to have a much smaller AC/A ratio than when corrected with spectacle lenses. Ocular convergence measured with contact lenses is greater than that measured with spectacles. Studies were carried out to establish the cause of these effects. Theoretical considerations are compared with the experimental results obtained. This study emphasizes the importance of the different amounts of magnification induced by contact lenses as compared with spectacles.

Heterophoria--its influence on stereopsis and the importance of cycloplegia in refraction testing of pilot applicants.

Heterophoria, its influence on stereopsis, and the importance of cycloplegia in refraction testing of pilot applicants were studied. Distant heterophoria was found in 94.5-100% while near heterophoria was found in 86.9-98.1% of the cases, depending on the test used. Hyperphoria of over 1 prism diopter was extremely rare. No statistically significant correlation between distant heterophoria and the degree of stereopsis could be demonstrated at a range of up to at least 6 prism diopters. Cycloplegia induced a mean difference of +0.75 diopter in refraction before and during cycloplegia. Eight per

Ocular Anatomical and Functional Characteristics in Anisometropic Chinese Children.

SIGNIFICANCE: This research found that anterior and posterior biometrics differ in many aspects between fellow eyes of anisometropic children. This might shed light on the mechanisms underlying the onset and progression of anisometropia and myopia. PURPOSE: This study aimed to investigate the ocular biometric parameters, peripheral refraction, and accommodative lag of fellow eyes in anisometropic children. METHODS: Anisometropic children were recruited. Axial length (AL), vitreous chamber depth (VCD), central corneal thickness, anterior chamber depth (ACD), lens thickness (LT), simulated K r

[Biomechanical condition of the cornea as a new indicator for pathological and structural changes].

AIM: Several methods permit the measurement of geometric parameters of the cornea, but until now biomechanical conditions of the cornea have been ignored (e.g. in refractive corneal surgery). Besides the geometric condition, biomechanical properties of the cornea have been shown to influence applanation measurement of intra-ocular pressure (IOP) and epidemiological studies have identified corneal thickness as an independent risk factor for the development and progression of glaucoma. The aim of this investigation was to characterize the biomechanical properties of the cornea using the ocular r

Myopia and astigmatism in adolescents with autism spectrum disorder.

Of 894,375 Israeli adolescents undergoing standardized medical and ocular assessments in our nationwide cross-sectional study, 2,622 (0.3%) had autism spectrum disorder (ASD). Adolescents with ASD had significantly higher adjusted odds of both myopia and astigmatism compared with peers who did not have ASD. The relative odds of refractive error in ASD increased with both myopia severity and astigmatism cylinder power. Associations were consistent across all astigmatism axis orientations, most notably with-the-rule. These findings suggest that early and routine vision screening may be beneficia

A prospective study of ocular abnormalities in hearing impaired and deaf students.

In a prospective study of 505 hearing impaired and deaf students conducted at Children's Hospital Eye Clinic, 48.7% were found to have significant ocular abnormalities. The prevalence of myopia, astigmatism, and pathological intraocular changes was found to be significantly increased over the general population. In addition, the prevalence of ocular abnormalities generally increased with the severity of the hearing loss. Of importance was the high prevalence of rubella-consistent abnormalities in the hearing impaired and deaf student populations. Rubella syndrome may be responsible for a major
