Cloning, functional expression, and chromosomal localization of the human pancreatic islet glucose-dependent insulinotropic polypeptide receptor.

Glucose-dependent insulinotropic polypeptide (GIP) is a hormone secreted by the endocrine K-cells from the duodenum that stimulates glucose-induced insulin secretion. Here, we present the molecular characterization of the human pancreatic islet GIP receptor. cDNA clones for the GIP receptor were isolated from a human pancreatic islet cDNA library. They encoded two different forms of the receptor, which differed by a 27-amino acid insertion in the COOH-terminal cytoplasmic tail. The receptor protein sequence was 81% identical to that of the rat GIP receptor. When expressed in Chinese hamster lu

Rearrangements at the 11p15 locus and overexpression of insulin-like growth factor-II gene in sporadic adrenocortical tumors.

Little is known about the pathophysiology of sporadic adrenocortical tumors in adults. Because loss of heterozygosity at the 11p15 locus has been described in childhood tumors, particularly, in adrenocortical tumors, associated with the Beckwith-Wiedemann syndrome and because insulin-like growth factor-II (IGF-II) is a crucial regulator of fetal adrenal growth, we looked for structural analysis at the 11p15 locus and IGF-II gene expression in 23 sporadic adrenocortical adult tumors: 6 carcinomas (5 with Cushing's syndrome and 1 nonsecreting) and 17 benign adenomas (13 with Cushing's syndrome,

Religious leaders as mental health gatekeepers in Nigeria: Belief-attitude typologies and implications for faith-health system integration: Findings from the CLERIC study.

In Nigeria, religious leaders are de facto mental health gatekeepers, yet their explanatory models and attitudes remain poorly characterised at the population level. This cross-sectional study maps causal beliefs and attitudes among 207 Christian and Muslim religious leaders in Lagos using validated, culturally adapted instruments, forming the attitudinal and typological component of the CLERIC research programme. An integrated conceptual model posits that explanatory models structure causal attributions, which constitute the cognitive foundation of stigma-related attitudes clustering into typ

Diffusion-weighted magnetic resonance imaging of cerebrospinal fluid in patients with and without communicating hydrocephalus.

BACKGROUND: Recent concepts about cerebrospinal fluid (CSF) circulation in communicating hydrocephalus (CoHy), which is also termed "restricted arterial pulsation hydrocephalus," suggest reduced arterial pulsations of subarachnoid vessels with a smaller amount of CSF shifted in subarachnoid spaces during the early systole. The postulated restriction of subarachnoid arterial pulsations in CoHy should induce a smaller motion artifact and reduced local stream effects in CSF in magnetic resonance (MR) diffusion-weighted imaging (DWI). PURPOSE: To investigate the maximum diffusivity in CSF in pati

Differential viral protein expression in Kaposi's sarcoma-associated herpesvirus-infected diseases: Kaposi's sarcoma, primary effusion lymphoma, and multicentric Castleman's disease.

Kaposi's sarcoma (KS)-associated herpesvirus (KSHV) is linked to KS, primary effusion lymphomas (PEL), and a subset of multicentric Castleman's disease (MCD). Transcript mapping studies using PEL cell lines have allowed preliminary classification of viral gene expression into constitutive (class I) and inducible (class II/III) categories. To determine whether viral gene expression differs in vivo, we examined tissue sections of KSHV-infected disorders, using specific antibodies against proteins that are representative of the different expression classes of KSHV genes. ORF73/LANA appears to be

Differential generation of class I H-2D- versus H-2K-restricted cytotoxicity against a demyelinating virus following central nervous system infection.

Despite the fact that both H-2K and D molecules are up-regulated in the central nervous system (CNS) following Theiler's murine encephalomyelitis virus (TMEV) infection, resistance in this virus model of multiple sclerosis maps exclusively to D. To address this paradox, we examined the ability of the K and D molecules to present viral antigens to cytotoxic T lymphocytes (CTL). Whereas no virus-specific CTL were detected in the CNS of susceptible B10.Q and B10.S mice 7 days post-infection, D-restricted CTL were identified readily in the CNS of resistant B10 animals. There was no evidence of K-r

Recent Issues in Varicella-Zoster Virus Latency.

Varicella-zoster virus (VZV) is a human herpes virus which causes varicella (chicken pox) as a primary infection, and, following a variable period of latency in neurons in the peripheral ganglia, may reactivate to cause herpes zoster (shingles) as well as a variety of neurological syndromes. In this overview we consider some recent issues in alphaherpesvirus latency with special focus on VZV ganglionic latency. A key question is the nature and extent of viral gene transcription during viral latency. While it is known that this is highly restricted, it is only recently that the very high degree

Evaluation of the common variants of the ABCA4 gene in families with Stargardt disease and autosomal recessive retinitis pigmentosa.

Stargardt disease (STGD) is one of the most common autosomal recessive retinal dystrophies with an estimated incidence of one in 10,000. It affects the central retina (macula). Retinitis pigmentosa (RP) comprises a large and exceptionally heterogeneous group of hereditary disorders of the retina. It is caused by the loss of photoreceptors. The condition is a degenerative disorder characterized by retinal pigment deposits and has an estimated incidence of one in 4,000. Although, to date, 45 known loci have been identified, none of them independently account for a substantial portion of RP. Rece

Genetic variability and molecular epidemiology of human and simian T cell leukemia/lymphoma virus type I.

In the past few years, numerous investigators have demonstrated that human T cell leukemia/lymphoma virus type I (HTLV-I) possesses a great genetic stability, and recent data indicate that viral amplification via clonal expansion of infected cells, rather than by reverse transcription, could explain this remarkable genetic stability. In parallel, the molecular epidemiology of HTLV-I proviruses showed that the few nucleotide changes observed between isolates were specific for the geographical origin of the patients but not for the type of the associated pathologies (adult T cell leukemia/lympho

High diversity of poliovirus strains isolated from the central nervous system from patients with vaccine-associated paralytic poliomyelitis.

To establish the etiology of vaccine-associated paralytic poliomyelitis (VAPP), isolates from the central nervous system (CNS) from eight patients with VAPP were compared with stool isolates from the same patients. The vaccine (Sabin) origin was checked for all of the available isolates. Unique and similar strains were recovered from paired stool and CNS samples for five of the eight VAPP cases and the three wild-type cases included in the study. In the remaining three VAPP cases, the stool samples and, in one case, the CNS samples contained mixtures of strains. In two of these cases an equiva

Molecular survey of a prevalent mutation, 985A-to-G transition, and identification of five infrequent mutations in the medium-chain Acyl-CoA dehydrogenase (MCAD) gene in 55 patients with MCAD deficiency.

Medium-chain acyl-CoA dehydrogenase (MCAD) deficiency is an inborn error of fatty-acid oxidation that is characterized by fasting intolerance and recurrent episodes of hypoglycemic coma which can be fatal. Its incidence is one of the highest among genetic metabolic disorders. Using a modified PCR and NcoI digestion method, we have surveyed 46 additional, unrelated MCAD-deficient patients for a prevalent mutation, an 985A-to-G transition (985A----G), that we previously identified in nine MCAD-deficient patients. Among the total of 55 studied, 44 were homozygous and 10 were heterozygous for the

Antenatal subpial hemorrhage diagnosed by fetal magnetic resonance imaging: imaging spectrum and autopsy findings.

Subpial hemorrhage is a rare intracranial hemorrhage typically described in neonates. We report the first prenatal diagnosis of subpial hemorrhage in a 28-year-old primigravida, defined on fetal magnetic resonance imaging (MRI) by its hallmark cortical inward depression ("cortical buckling") and restricted diffusion on the apparent diffusion coefficient map, and later confirmed by autopsy. This case implicates intrinsic fetal factors-rather than birth trauma or neonatal asphyxia-in subpial hemorrhage pathogenesis and highlights the critical role of fetal MRI in distinguishing subpial hemorrhag

SNP variations in the 7q33 region containing DGKI are associated with dyslexia in the Finnish and German populations.

Four genes, DYX1C1, ROBO1, DCDC2 and KIAA0319 have been studied both genetically and functionally as candidate genes for developmental dyslexia, a common learning disability in children. The identification of novel genes is crucial to better understand the molecular pathways affected in dyslectic individuals. Here, we report results from a fine-mapping approach involving linkage and association analysis in Finnish and German dyslexic cohorts. We restrict a candidate region to 0.3 Mb on chromosome 7q33. This region harbours the gene diacylglycerol kinase, iota (DGKI) which contains overlapping

Temporal dynamics of EEG microstates during postural control in Parkinson's disease.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterised by motor impairments extending to balance and postural regulation. Although EEG abnormalities in oscillatory activity and functional connectivity are well documented in PD, large-scale brain dynamics during tasks directly engaging postural control remain poorly understood. To address this gap, we examined EEG microstate organisation during the BioVRSea virtual-reality postural-control task in early-stage PD patients (n = 30) and matched healthy controls (HC; n = 26). EEG microstates, brief quasi-stable scalp top

CELL FREE DNA-BASED PREDICTION OF PROGNOSIS FOR PATIENTS ON VENO-ARTERIAL EXTRACORPOREAL MEMBRANE OXYGENATION.

Introduction : Elevated cell-free DNA (cfDNA) was observed in patients receiving venoarterial (VA) extracorporeal membrane oxygenation (ECMO), but the clinical relevance of cfDNA is still not clear. We aimed to establish a predictive model based on the cfDNA to predict the prognosis for patients on ECMO, and reveal the values of cfDNA for complications of limb ischemia and bleeding/thromboembolic events. Methods : Single-center, retrospective evaluation of patients with ECMO support from 2018 through 2023. The derivation cohort included 133 adults diagnosed with cardiogenic shock who received

Family-centred care in adult intensive care units: A scoping review protocol of secondary evidence with a focus on the nursing role.

Admission to an adult intensive care unit (ICU) is a profoundly disruptive experience for both patients and their families. Family-Centred Care (FCC) is the predominant framework for addressing their needs, yet the secondary evidence base is heterogeneous and the specific nursing contribution remains insufficiently theorised. This protocol describes a scoping review designed to systematically map the secondary evidence on FCC in adult ICUs, with particular attention to the nursing role, the nurse-family relationship, and the instruments used to measure FCC-related constructs. Conducted in acco

Post-elimination trachoma surveillance and integration into primary health systems: a scoping review protocol.

OBJECTIVES: This scoping review will map the global evidence on post-elimination trachoma surveillance, with a particular focus on integration models within primary health care, aiming to draw lessons for Nigeria. INTRODUCTION: Trachoma, the leading infectious cause of preventable blindness, has been eliminated as a public health issue in 27 countries. Maintaining these achievements requires strong post-elimination surveillance, yet evidence on incorporating trachoma monitoring into primary health care and health information systems remains scattered and incompletely synthesized.
