We produced experimental renovascular malignant arterial hypertension by modified Goldblatt's procedures, in 60 rhesus monkeys. Hypertensive retinopathy was studied in detail (by ophthalmoscopy, and stereoscopic color fundus photography and fluorescein fundus angiography on long-term follow-up). Cotton-wool spots (CWSs) were found to be an important, early retinal lesion. On ophthalmoscopy, they had a characteristic appearance. Fluorescein fundus angiography of these lesions revealed focal retinal capillary nonperfusion. The CWSs usually lasted for over 3 weeks and resolved within 6 weeks, lea

PURPOSE: To describe a case of multifocal branch retinal artery occlusions due to a traumatic internal carotid artery pseudoaneurysm and endovascular treatment after a balloon sphenoid sinuplasty procedure.
OBSERVATIONS: A 62-year-old female with no significant past medical history underwent an elective outpatient balloon sinuplasty of the right frontal sinus and bilateral sphenoid sinuses. Shortly after the balloon sinuplasty device was inflated in the left sphenoid sinus, acute, severe, pulsatile bleeding was encountered from the posterior wall of the sinus. The patient underwent emergent c…

We have developed an AO-OCT-based method to characterize microscopic vasodilatory responses of peripapillary major retinal artery (ppRA) to the focal luminous flicker stimulation of peripheral retinal neurons (>15° temporal to the fovea) with various mean illuminance levels, frequencies, and spatial patterns in the living human eye. The results have demonstrated that our proposed method can effectively measure transient, microscopic vasodilatory responses of the ppRA to the focal flicker stimulation, which increased monotonically with mean illuminance levels (1, 5 and 11lx), and exhibited freq…

PURPOSE: To report a case of combined cilioretinal artery and central retinal vein occlusion in a patient with extremely large, asymptomatic optic disk drusen.
METHODS: A patient with sudden vision loss underwent a comprehensive examination, including multimodal imaging.
RESULTS: A 45-year-old man with no previous visual symptoms presented with a combined cilioretinal artery and central retinal vein occlusion in the right eye. The optic nerve in the right eye was slightly hyperemic and the nerve appeared normal in the left eye. His optic nerves did not have a nodular appearance but showed su…

OBJECTIVES: To investigate the association between quantitative retinal vascular parameters and coronary artery disease (CAD) and to evaluate the efficacy of a retinal phenotype-based diagnostic model as a non-invasive tool for early CAD screening.
DESIGN: A retrospective cross-sectional study.
SETTING: A single-centre study conducted at the Cardiovascular Center of Beijing Tongren Hospital, Capital Medical University, China, between January and October 2024.
PARTICIPANTS: 417 patients with suspected angina undergoing their first coronary angiography (CAG) were enrolled. Inclusion criteria

BACKGROUND: Cardiac papillary fibroelastoma are rare benign tumours of the endocardium, often arising from heart valves. Despite their benign histology, papillary fibroelastoma carry high embolic potential and can lead to ischaemic events. In contrast to stroke, ocular embolization including central retinal artery occlusion is a rather rare first manifestation.
CASE SUMMARY: We report a 61-year-old female with sudden painless monocular vision loss due to central retinal artery occlusion. Initial stroke and cardiovascular work-up revealed a very small floating mass (0.6 × 0.4 × 0.2 cm) on the …

RATIONALE: Essential thrombocythemia (ET) is a myeloproliferative neoplasm characterized by sustained thrombocytosis, which can lead to thrombotic complications including retinal artery occlusion. We report a rare case of recurrent central retinal artery occlusion (CRAO) as the initial manifestation of ET.
PATIENT CONCERNS: A 37-year-old female presented with recurrent transient monocular blindness in the right eye, progressing to persistent amaurosis. Ophthalmic examination revealed findings consistent with acute CRAO, accompanied by significantly elevated platelet counts. Emergency treatmen…

PURPOSE: Retinal artery occlusion (RAO) is frequently associated with cerebrovascular and cardiovascular events, and guidelines highlight the need for secondary prevention. Since 2019, our hospital has implemented collaborative care between ophthalmologists and vascular neurologists for acute RAO, and here, we report the clinical outcomes before and after this approach.
STUDY DESIGN: Retrospective observational study.
METHODS: Eighty-seven patients (mean age: 72.2 ± 13.4 years; 40 male) diagnosed with central retinal artery occlusion (CRAO), branch retinal artery occlusion (BRAO), or ciliore…

BACKGROUND: Low retinal blood flow and/or vasospasm represent major risk factors for the development of glaucomatous optic neuropathy (GON), a potentially blinding eye disease. Bradykinin (BK), a nonapeptide, is endogenously produced and released, which can cause smooth muscles to contract and relax in different tissues depending on the physiological/pathological situation and the presence or absence of vascular tone. Several reports have shown the presence of BK receptor mRNAs, and in some cases, B1- and B2-receptor proteins, in ocular tissues, including the retina. However, the function of t

BACKGROUND: Central retinal artery occlusion (CRAO) is a vision-threatening emergency with no established effective treatment. Prostaglandin E1 (PGE1), known for its vasodilatory and cytoprotective properties, may offer therapeutic benefits for retinal ischemia.
METHODS: In this retrospective study, we compared visual outcomes between CRAO patients who received intravenous PGE1 within 24 hours of symptom onset (followed by oral administration) and those who received conventional therapy. PGE1 was administered intravenously for 5 days.
RESULTS: At one month, the PGE1 group showed significantl…

PURPOSE: To evaluate the association of retinal ischemic perivascular lesions (RIPLs) with myocardial infarction (MI) among patients diagnosed with coronary artery diseases (CAD).
DESIGN: Retrospective cross-sectional study.
METHODS: Consecutive patients (317 patients) with CAD who underwent macular spectral domain optical coherence tomography (SD-OCT) were captured. Patients with CAD who developed MI were compared to those without MI. SD-OCT were reviewed by 2 independent and masked graders for the presence of RIPLs. Medical records were reviewed. Central retinal artery occlusion (CRAO) is an acute retinal ischaemic disease, but early diagnosis is challenging due to a lack of biomarkers. Blood samples were collected from CRAO patients and cataract patients. Gene expression profiles were distinct between arterial/venous CRAO blood (A-V group) and venous CRAO/control blood (V-C group) samples. Differentially expressed genes (DEGs) were subjected to GO and KEGG enrichment analyses. Hub genes were identified by Cytoscape and used to predict gene interactions via GeneMANIA. Immune cell infiltration was analysed by CIBERSORT. More than 1400 D…

OBJECTIVES: Central retinal artery occlusion (CRAO) is a stroke of the retina potentially amenable to intravenous thrombolysis (IVT). We aimed to determine feasibility of an emergency treatment protocol and risk profile of IVT for CRAO in a comprehensive stroke center (CSC).
METHODS: We performed a retrospective, observational cohort study including patients with acute CRAO admitted to a CSC over 4 years. Patients are offered IVT if they present with acute vision loss of ≤ 20/200 in the affected eye, have no other cause of vision loss (incorporating a dilated ophthalmologic exam), and meet cr…
