Rosai-Dorfman disease: unusual cause of diffuse and massive retroperitoneal lymphadenopathy.

Rosai-Dorfman disease (RDD), originally described as sinus histiocytosis with massive lymphadenopathy, is a rare histiocytic proliferative disorder with a distinctive microscopic appearance. Patients usually present with massive cervical lymphadenopathy. However other nodal sites (mediastinal and inguinal) are also frequently involved. The retro-peritoneum is an infrequent site of involvement. This uncommon entity should also be included in the list of differential diagnoses of diffuse and massive lymphadenopathy, which includes includes infectious/granulomatous conditions, lymphomatous and me

Innovative technique for ureteral stenting during retroperitoneal laparoscopic ureterolithotomy.

Large calculi in the upper and mid ureter are best managed by laparoscopic ureterolithotomy. Stenting after laparoscopic ureterolithotomy is … has been extracted by incising the ureter, the straightened stent on the guidewire is passed in the retroperitoneal space through a 5-mm port. The stent is passed down the ureter into the bladder first and then up the ureter to the renal pelvis. The portion of the guidewire lying outside the stent is positioned at the site of ureterolithotomy. The guidewire is pulled o …

Case report of a difficult venous access: retroperitoneal approach of inserting an improved non-heparinised port for long-term use.

This is a case report of a patient with difficult venous access following thrombosis of major superficial and deep veins of the limbs as documented by ultrasound doppler and venography. The insertion of a few central ports were infected and the vein thrombosed. Venography revealed that central venous access was no longer feasible. The previous laparotomies had resulted in dense intra-peritoneal adhesions, and rendered further laparotomy virtually impossible. The patient had occasional adhesion colics, vomiting and hypoglycemic episodes. A rarely performed retro-peritoneal approach of inserting

Retroperitoneoscopic pyelolithotomy as initial treatment for upper urinary tract large stone.

We report a case in which retroperitoneoscopic pyelolithotomy was the procedure selected to treat a large stone in the upper urinary tract. A 71-year-old woman who had multiple cerebral infarction and dementia was admitted with a persistent high fever unresponsive to antibiotics. The diagnosis was pyelonephritis and urosepsis associated with ureteral calculus. A large calculus(3.0 x 2.0 cm)was found in the left ureter at the L3 level. She underwent nephrostomy of the left side. After the patient's general condition had improved, surgery was performed successfully with an uneventful recovery. T

Enoxaparin-associated retroperitoneal bleeding in two patients with renal insufficiency.

Two patients with chronic kidney disease experienced a major bleeding event, retroperitoneal hematoma, requiring a blood transfusion after the administration of enoxaparin. The first patient was a 61-year-old Caucasian woman with multiple comorbidities, including chronic kidney disease stage 4. She received subcutaneous enoxaparin 45 mg every 12 hours, along with antiplatelet agents. On the seventh day, she developed a large retroperitoneal hematoma and her hematocrit had decreased, requiring a transfusion of packed red blood cells. The second patient was a 74-year-old, obese Caucasian woman w

Retroperitoneoscopic ureterocalicostomy for congenital proximal ureteral stenosis.

We report the first case of retroperitoneoscopic ureterocalicostomy in a 17-year-old male patient with severe left hydronephrosis caused by a long congenital upper ureteral stenosis. With a retroperitoneoscopic approach, the stenotic segment was resected, the thin renal parenchyma overlying the lower calyx was fenestrated by a modest excision, and the proximal ureter was anastomosed to the lower pole in an end-to-end manner. At 2 years postoperatively, the patient was asymptomatic, with a significant reduction in hydronephrosis and a patent upper ureter. Retroperitoneoscopic ureterocalicostomy

[Cystic space-occupying lesion with involvement of the pelvis: unusual site of echinococcosis. Case report and surgical treatment].

A case of an unusual localization of cystic echinococcosis is presented. Initially it appeared as a retropertioneal tumor with bone involvement and the diagnosis was only recognized intraoperatively. The patient was treated by radical surgery and chemotherapy according to the guidelines of the WHO. We conclude that echinococcosis should be considered in extrahepatic cystic tumors, especially in patients from endemic areas.

Iatrogenic ejaculation disorders and their prevention.

Ejaculation is mediated by sympathetic fibers originating from the D10-L2 medullar center. These nerves rise from the lumbar ganglia of the paravertebral sympathetic trunk and travel posteriorly to the vena cava and then to the interaortocaval space, on the right side, and laterally to the aorta, on the left side. They are the principal constituents of the superior hypogastric plexus. Many surgical operations can cause an ejaculation disorder, but the most important is retroperitoneal lymphadenectomy (RL) for testis cancer, because it involves young patients and it has been the subject of impo

[Is it able to simplify the classifications of staging and curability for pancreatic cancer?].

This study was undertaken to simplify the classifications of the staging and curability of pancreatic cancer by the Japan Pancreas Society (JPS). A total of 461 patients who underwent surgical resection from 1968 to 1997 were analyzed in this study in accordance with our new classification of staging and curability. This staging mainly consists of three factors based on tumor extent:invasion to the retroperitoneum (rp); invasion to the extrapancreatic nerve plexus (pl); and lymph node metastasis (n). Each factor is simply judged as + (positive) or - (negative). Curability is also judged simply

The anatomical approach to inguinal orchiopexy.

PURPOSE: Achieving a scrotally positioned testis with no iatrogenic atrophy is the primary goal of the surgical treatment of cryptorchidism. The success rate is associated with preoperative testicular position and the technique used. Our success rate with inguinal orchiopexy for abdominal testis is higher than those previously published and may be due to our anatomical approach to retroperitoneal dissection. We review the relevant anatomy and discuss the surgical technique of inguinal orchiopexy. MATERIALS AND METHODS: We researched and reviewed the relevant technical and anatomical literatur

Pediatric retroperitoneoscopic nephrectomy using 2 mm. instrumentation.

PURPOSE: We describe several modifications of the retroperitoneoscopic approach to nephrectomy for benign renal disease, including the use of 2 mm. instrumentation and prone patient positioning. MATERIALS AND METHODS: A total of 14 children underwent retroperitoneoscopic nephrectomy in the prone position. An inflatable dissecting device was inserted into the retroperitoneum after a small muscle splitting incision was made at the lateral border of the sacrospinalis muscle approximately 1 cm. below the costovertebral angle. After inflation the dissecting device was replaced with a 5 mm. cannula

Surgical anatomy and dissection techniques for laparoscopic surgery.

All anatomically important pelvic structures lie embedded in the fatty-fibrous connective tissue of the retroperitoneum from which they may be freed by blunt dissection in the correct tissue planes. By relying on fixed laparoscopic landmarks, the correct surgical planes of dissection can be found, and all vital structures freed and identified by a systematic dissection consisting of a precise sequence of operative steps. Once the retroperitoneal dissection has been completed and all vital structures identified, most gynecologic operations can be carried out safely and without much difficulty l

Retroperitoneal radical hysterectomy.

A surgical technique is presented which combines both the abdominal and vaginal approaches to radical hysterectomy with significant modifications to allow it to be named "retroperitoneal" radical hysterectomy. Small oblique skin incisions are made 1 cm superior to the inguinal ligament exposing the retroperitoneal structures. The round ligament, uterine artery, ovarian vessels, and cardinal ligament are divided without entering the peritoneal cavity. The remainder of the procedure is completed by a modified Schauta technique. Eight patients with Stage I cancer of the cervix underwent this oper

[The localization of the regional ovarian lymph nodes in girls and mature women].

The method of interstitial injections followed by preparation was used to investigate the regional lymph nodes of the ovaries located in the retroperitoneal space in 18 preparations from corpses of women of different age. The permanently occurring (in all the cases) precaval lymph node, regional for the right ovary and permanent lateral aortal lymph node, regional for the left ovary were determined. The shape, syntopy, skeletotopy of these nodes are described. Their sizes are determined. It was established that the level of localization of both the precaval and lateral aortal lymph nodes displ…

Case report: MR appearance of a retained surgical sponge.

A case of a retained surgical sponge found in the retroperitoneum is presented with findings on magnetic resonance (MR) imaging, computed tomography (CT), ultrasonography (US) and angiography. Among all the performed modalities, a characteristic internal structure of the gauze granuloma was best visualized on MR imaging. If no radio-opaque marker is seen on plain radiography or CT, the folded fabric inner structure visualized on T2-weighted images can be a most important clue to the correct diagnosis of this iatrogenic mass.
