The vascular effects of lethal doses of E. coli endotoxin, as observed in the mesentery of the rat, resemble the reactions of traumatic and hemorrhagic shock in the following respects: a profound inhibition of arteriolar and precapillary reactivity to topical epinephrine occurs after an initial stage of hyperreactivity; the small veins show failure to relax completely following constrictor doses of epinephrine; and the terminal vessels develop an unusual sensitivity to fluctuations in temperature of the fluid irrigating the tissue. Rats in which tolerance to bacterial endotoxin is induced, by

The early diagnosis of traumatic pericardial tamponade may be difficult. The transcutaneous oxygen (PtcO2) monitor has been shown to be a useful indicator of low-flow shock. In the case presented, PtcO2 monitoring was the earliest indicator of shock due to pericardial tamponade and led to successful early therapy before other signs of physiologic decompensation were evident. In the management of acutely traumatized patients, PtcO2 monitoring is useful both in identifying patients who are in shock and in helping to guide therapy.

OBJECTIVES: Much attention has been given to prehospital blood use for trauma, but evidence is lacking for non-traumatic hemorrhage. Therefore, we sought to evaluate the epidemiology of patients receiving prehospital whole blood for nontraumatic hemorrhage.
METHODS: We conducted a retrospective observational study of patients receiving prehospital blood transfusions for non-traumatic hemorrhagic shock within two fire-based emergency medical services (EMS) systems, between February 20, 2024, and April 23, 2026. Patients were included if blood products were administered for a non-traumatic caus

The effects of human urinary trypsin inhibitor (UTI) were studied in experimental shock models. Administration of 50,000 U/kg, i.v., of UTI protected against mortality from shock induced by burn, endotoxin or trauma. Aprotinin at a dose of 50,000 U/kg improved only endotoxin shock and showed a moderate but not significant effect on burn and traumatic shock. Administration of 50,000 U/kg, i.v., of UTI protected against the aggravation in systemic hemodynamics in canine hemorrhagic shock. Furthermore, in rat traumatic shock, 50,000 U/kg, i.v., administration of UTI significantly reversed the inc

PURPOSE OF REVIEW: To describe how hemostatic resuscitation can be used in the prehospital phase of resuscitation to reduce preventable deaths after traumatic injury.
RECENT FINDINGS: Hemorrhagic shock is the leading cause of death that is preventable after injury. The National Academy of Sciences, recently, recommended that achievement of zero preventable deaths after traumatic injury should be the goal of a national trauma system. In the United States, there are an estimated 25 000 preventable deaths per year in the prehospital phase of resuscitation because of traumatic hemorrhagic shock.
