Polysomnographic characteristics of excessive daytime sleepiness phenotypes in obstructive sleep apnea: results from the international sleep apnea global interdisciplinary consortium.

STUDY OBJECTIVES: Excessive daytime sleepiness (EDS) is a major symptom of obstructive sleep apnea (OSA). Traditional polysomnographic (PSG) measures only partially explain EDS in OSA. This study analyzed traditional and novel PSG characteristics of two different measures of EDS among patients with OSA. METHODS: Sleepiness was assessed using the Epworth Sleepiness Scale (>10 points defined as "risk of dozing") and a measure of general sleepiness (feeling sleepy ≥ 3 times/week defined as "feeling sleepy"). Four sleepiness phenotypes were identified: "non-sleepy," "risk of dozing only," "feelin

SARS-CoV-2 Causing Transient Central Sleep Apnea in Patients Treated for Obstructive Sleep Apnea with Continuous Positive Airway Pressure.

BACKGROUND SARS-CoV-2 has globally affected humanity and devastated many families. Here, we attempt to identify which diseases are independent risk factors for severe SARS-CoV-2. There have been multiple studies that have evaluated the impact of obstructive sleep apnea (OSA) on SARS-CoV-2 outcomes, suggesting that OSA is an independent risk factor. SARS-CoV-2 has also been suggested to invade the central nervous system and be responsible for neurological signs and decreasing central respiratory drive. Central sleep apnea (CSA) is defined when apneas and hypopneas are associated with absent or

Meta-Analysis of Obstruction Site Observed With Drug-Induced Sleep Endoscopy in Patients With Obstructive Sleep Apnea.

OBJECTIVE: To perform a meta-analysis on the distribution and characteristics of the obstructive site in patients with obstructive sleep apnea (OSA) using data from a variety of published studies that evaluated the obstruction with drug-induced sleep endoscopy (DISE). METHODS: A literature search was performed to identify studies in which DISE was used to identify the obstruction site in adult patients with OSA, and the obstruction site was described in sufficient detail. Four items were evaluated in the meta-analysis: the obstruction site, closing direction of the soft palate, degree of clos

Analysis of upper airway obstruction by sleep videofluoroscopy in obstructive sleep apnea: a large population-based study.

OBJECTIVES/HYPOTHESIS: To analyze the pattern of the upper airway obstruction in a large cohort of obstructive sleep apnea (OSA) patients using sleep videofluoroscopy (SVF). STUDY DESIGN: Retrospective analysis. METHODS: This study included 922 OSA patients who underwent both polysomnography and SVF. Their mean age, apnea-hypopnea index, and body mass index were 46.8 years, 34.2 per hour, and 26.2 kg/m2, respectively. Sleep was induced by intravenous injection of midazolam, and the obstruction pattern was determined on SVF when oxygen saturation dropped by more than 4% in pulse oxymetry. RE

Exploring the genetic overlap between obstructive sleep apnea and prostate cancer.

BACKGROUND: Studies have highlighted the possible association between obstructive sleep apnea (OSA) and an increased risk of cancer, especially prostate cancer. However, the extent to which genes that are found in both conditions contribute to their comorbidity is unclear. The aim of this study was to identify risk genes present in both conditions. METHODS: Genes common to both conditions were identified and the statistical significance of the overlap was assessed. Pathway enrichment and protein-protein interaction analyses were performed on the intersecting gene set. RESULTS: There were 68

Obesity and Obstructive Sleep Apnea: Impact on Medical and Surgical Management.

Obstructive sleep apnea (OSA) and obesity are increasingly prevalent conditions for which weight loss can have a profound impact. Management strategies for OSA include lifestyle, medical, and surgical interventions. Obesity has been shown to influence adherence to and effectiveness of non-surgical therapies, such as positive pressure therapy. Surgical interventions such as pharyngeal surgery and hypoglossal nerve stimulation are less effective in individuals with obesity, while maxillomandibular advancement is considered efficacious regardless of obesity status. Referral to bariatric surgery s

Tirzepatide: A Breakthrough Therapy for Obstructive Sleep Apnea and Metabolic Dysfunction.

Obstructive sleep apnea (OSA) is a breathing disorder characterized by repeated, complete, and partial upper airway blockage, which results in disturbances in sleep patterns, neurocognitive functions, and hypoxemia. It is strongly linked to obesity and metabolic dysfunction, contributing to cardiovascular and neurocognitive complications. Tirzepatide, a dual GIP/GLP-1 receptor agonist, has shown significant potential for weight loss with metabolic benefits, making it a potential therapeutic strategy for OSA by reducing fat deposition around the upper airway, improving insulin sensitivity, and

Neurophysiologic tongue protrusion characteristics$ in obstructive sleep apnea: A comparative study.

The essential role of upper airway muscles in maintaining patency has led to the hypothesis that alterations in tongue muscle properties may contribute to the development of obstructive sleep apnea (OSA). Prior studies comparing the mechanical characteristics of tongue muscles in OSA patients and healthy controls have yielded inconsistent results. In this study, we evaluated tongue muscle mechanics using an objective, non-volitional method: surface electrical stimulation of the genioglossus (GG) muscle. Tongue protrusion force, whether generated volitionally or by electrical stimulation, was s

High night-to-night variability in childhood obstructive sleep apnea severity.

STUDY: Overnight in-laboratory polysomnography is the gold standard test for childhood obstructive sleep apnea (OSA). Night-to-night variability is a well-documented phenomenon in adult literature but is poorly characterized in children. We evaluated the test-retest reliability of polysomnography in children with moderate-severe OSA. METHODS: This is a secondary analysis of two pediatric trials evaluating OSA therapies for children aged 4-18 years old with moderate-severe OSA. The baseline diagnostic polysomnography data were compared to repeat polysomnography data in trial. RESULTS: Twenty-

Hidden oral inflammation and biomarker misclassification in obstructive sleep apnea trials.

Inflammatory biomarkers are used as surrogate endpoints in obstructive sleep apnea trials, assuming the measured signal originates from the airway or systemic response to intermittent hypoxia. Periodontitis-a common, extrapulmonary inflammatory reservoir-elevates systemic active matrix metalloproteinase-8 independently of obstructive sleep apnea. This Perspective examines whether unmeasured oral inflammation confounds biomarker attribution in obstructive sleep apnea trials. Epidemiological and interventional evidence indicates that periodontitis is highly prevalent, is more common in individua

[Obstructive sleep apnea and epilepsy comorbidity: possible complications and treatment options].

Epilepsy is one of the most prevalent neurological disorders, affecting approximately 50 million individuals globally. Sleep plays a crucial role in the progression of epilepsy, particularly concerning serious complications such as Sudden Unexpected Death in Epilepsy (SUDEP). The sleep patterns of individuals with epilepsy can be significantly disrupted by obstructive sleep apnea (OSA), which affects around 40% of adult epilepsy patients. The most effective treatment for OSA is continuous positive airway pressure (CPAP). Additionally, carbonic anhydrase inhibitors, commonly used to manage foca

Nocturnal Hypoxemia Burden Correlates With Vestibular Dysfunction in Obstructive Sleep Apnea.

INTRODUCTION: Obstructive sleep apnea (OSA) is a potential contributor to vestibular dysfunction, though specific correlating parameters remain unclear. This study aimed to determine whether hypoxemia severity indices correlate quantitatively with caloric slow-phase eye velocity (SPV), a marker of vestibular function. METHODS: In a retrospective cohort of 82 adults undergoing both full-night PSG and bithermal caloric testing, the primary outcome was total caloric SPV (°/s; sum of four irrigations). Fifteen prespecified predictors were evaluated, including hypoxemia burden (apnea-hypopnea inde

Untreated obstructive sleep apnea and accelerated cognitive decline over 10 years.

INTRODUCTION: Obstructive sleep apnea (OSA) is associated with cognitive decline, but short-term studies show limited cognitive benefits of its treatment with continuous positive airway pressure (CPAP). We examined whether longer follow-up exhibits greater cognitive differences associated with CPAP use. METHODS: We analyzed 777 participants from the 2011 National Health and Aging Trends Study (NHATS) with linked Medicare claims, with one or more claims for OSA and no baseline cognitive impairment. CPAP treatment was defined by one or more CPAP claims. Cognitive trajectories from 2011 to 2021

Gestational Obstructive Sleep Apnea: From Pathophysiological Mechanisms to Maternal-Fetal Outcomes.

Obstructive sleep apnea (OSA) during pregnancy is increasingly recognized as a clinically significant condition associated with maternal cardiovascular and metabolic complications, as well as adverse fetal outcomes. Physiological changes in pregnancy, including weight gain, airway edema, reduced functional residual capacity, and altered ventilatory control, can exacerbate or unmask sleep-disordered breathing. Emerging evidence suggests associations between gestational OSA and hypertensive disorders of pregnancy, gestational diabetes mellitus, cesarean delivery, and preterm birth. Continuous po
