Comparison of four diagnostic systems for the diagnosis of somatization disorder.

This article compares the Feighner criteria, the DSM-III criteria for somatization disorder and a modified version of the proposed ICD-10 criteria. Working with a data set collected from the charts of 250 patients considered likely to have unexplained somatic symptoms, the kappa statistic and percentage agreement was calculated. The kappa between the DSM-III and DSM-III-R criteria is 0.93. Between the modified ICD-10 and DSM-III it was 0.71, but between Feighner and the modified ICD-10 it was 0.44. However, the different criteria identify the same patient population based on mental co-morbidit

Vertigo and dizziness in children.

PURPOSE OF REVIEW: Vertigo and dizziness occur with considerable frequency in childhood and adolescence. Most causes are benign and treatable. This review aims to make physicians more alert to the frequent causes of dizziness in the young. RECENT FINDINGS: Epidemiological data confirm that migraine-related syndromes are the most common cause of vertigo in children. Vestibular migraine and benign paroxysmal vertigo have now been defined by the International Classification of Headache Disorders. About half of the adolescents with vertigo and dizziness show psychiatric comorbidity and somatizati

Intergenerational transmission of somatization behaviour: 2. Observations of joint attention and bids for attention.

BACKGROUND: Somatoform disorders may have their roots in childhood through processes that involve an enhanced parental focus on health. The aim of this study was to test the hypothesis that somatizing mothers will show less joint involvement than other mothers during play but greater responsiveness when this play involves a 'medical' theme. METHOD: Cross-sectional observational study of 42 chronic somatizers, 44 organically ill and 50 healthy mothers and their 4-8 year-old children during structured play and a meal. Tasks comprised boxes containing tea-set items, 'medical' items and a light s…

High CSSI-24 scores in paediatric headache suggest a functional disorder and call for a neurodevelopmental biopsychosocial framework: A prospective cohort study.

BACKGROUND AND OBJECTIVES: Primary headache is the most frequent neurological symptom in childhood and often coexists with psychosomatic risk factors. This study investigated functional (psychosomatic) features and anxiety in children and adolescents with primary headache, testing the hypothesis that a subgroup with high Children's Somatic Symptoms Inventory (CSSI-24) scores may present characteristics consistent with a functional disorder. METHODS: In this prospective observational cohort study, 148 patients aged >6 years with primary headache, diagnosed according to ICHD-3 criteria by child

Abnormalities in the absolute power of Delta and Alpha rhythms in the frontal lobe of patients suffering from psychosomatic disorders.

OBJECTIVES: Psychosomatic disorders (somatic symptom disorder - SSD) constitute a heterogeneous group of medical conditions characterized by somatic symptoms without explained somatic causes. Due to the complexity of symptoms, they present a significant medical, social and economic challenge, and diagnosing and treating these disorders remains a substantial clinical challenge. The aim of the conducted research was an attempt to identify characteristic patterns in the surface-level brain bioelectrical activity of individuals experiencing psychosomatic disorders. The obtained results may contrib

The Predictors of Somatic Symptoms in a Population Sample: The Lifelines Cohort Study.

OBJECTIVE: Multiple bodily symptoms predict poor health status, high healthcare use, and onset of functional somatic syndromes. This study aimed to identify the predictors of somatic symptoms with special reference to mental disorders. METHODS: The prospective, population-based Lifelines cohort study included 80,888 adults who were followed up for a mean of 2.4 years. At baseline sociodemographic status, self-reported medical disorders, functional somatic syndromes, stress, neuroticism, mental disorders (assessed with Mini-International Neuropsychiatric Interview), health perception, and anal

Stress and emotional distress: their associations with somatic and temporomandibular disorder-related symptoms.

Although Asians are posited to be predisposed to somatization, studies on the inter-relationships between emotional and physical symptoms in Asian youths are still limited. This study determined the associations between emotional, somatic, and Temporomandibular Disorder (TMD)-related symptoms in Asian youths. Youths were recruited from a local polytechnic and the prevalence/severity of emotional distress was ascertained with the Depression, Anxiety, and Stress Scale-21. Somatic and TMD-related symptoms were appraised with the Patient Health Questionnaire-15 and Fonseca Anamnestic Index. Data w

A therapy-led, multidisciplinary programme for treatment-resistant functional fixed dystonia.

Functional neurological disorder (FND) describes various neurological symptoms that are not explained by an organic aetiology. The condition has a poor prognosis. Despite this, there is sparse research that informs clinical interventions for FND, particularly when treating functional fixed dystonia. Our article has outlined an intervention for a patient with a treatment-resistant functional fixed dystonia that was informed by a biopsychosocial model, which aimed to rehabilitate the patient's functional motor symptoms. This led to favourable outcomes including restoring full range of movement i

Chronicity factors of temporomandibular disorders: a critical review of the literature.

Facial pain often persists long after any identifiable organic pathology has healed. Moreover, in a subgroup of patients with temporomandibular disorder (TMD), no treatment is effective. Knowledge of factors associated with persistent pain in TMD could help identify personalized treatment approaches. Therefore, we conducted a critical review of the literature for the period from January 2000 to December 2013 to identify factors related to TMD development and persistence. The literature findings showed that chronic TMD is marked by psychological distress (somatization and depression, affective

Depression and anxiety in patients repeatedly referred to secondary care with medically unexplained symptoms: a case-control study.

BACKGROUND: One third of referrals from primary to secondary care are for medically unexplained symptoms (MUS). We aimed to determine the association of depression and anxiety disorders with high use of specialist services by patients with MUS. We did this by comparing their prevalence in patients who had been repeatedly referred with symptoms for which they had received repeated specialist diagnoses of MUS with that in two control groups. We also determined the adequacy of treatment received. METHOD: A case-control study in five general practices in Edinburgh, UK. Data collection was by case

Treatment of tunnel phobia: an experimental field study.

The opening of the deepest undersea tunnel in the world (264 m below sea level, 5600 m in length) replaced the ferry from the island of Hitra to the mainland in Norway. This event provoked phobic anxiety for traveling through the undersea tunnel in a number of individuals in the area. A treatment program for tunnel phobia was designed to test whether such a phobia could be mitigated by procedures previously proven effective in the treatment of other phobias. The program was presented to 18 persons with a specific phobia for tunnels and included a general discussion on the construction of under

Depression and associated organic diseases: are there any specific depressive symptoms? Results from the dialogue-2 survey.

The objective of the survey was to compare depressive symptoms in depression with and without a concomitant organic disease. The results based on the HAD and CES-D scales showed that, compared to those with an isolated depression, the patients with an associated chronic organic disease have a higher score on two items on the HAD. Three variables on the CES-D scale also helped to differentiate the two groups of depressed patients. No significant difference was found between the two groups in terms of anxiety or cognitive symptoms, fatigue or feelings of disability. Our results do not indicate a
