Assessment of children with developmental apraxia of speech: a rationale.

A rationale to guide assessment and subsequent management of children with developmental speech disorders is described. Hypotheses about the nature of praxis, the role of oral-verbal praxis in linguistic processing and speech development, and the effects of disturbances in praxis on speech behavior are presented. Implications of a diagnosis of developmental apraxia of speech (DAS) for treatment planning and expected speech, language, and social communication outcomes are discussed. Misconceptions about children diagnosed with DAS and assumptions underlying the author's approach to assessing ch

Communication competence and disability secondary to laryngectomy and tracheoesophageal puncture voice restoration.

BACKGROUND: There is a large body of research showing the negative impact of a total laryngectomy on the resultant voice signal across multiple outcomes (e.g., speech intelligibility/acceptability, listener comprehension, voice quality). However, there is limited information on the effects of laryngectomy on broader communication acts, particularly in the area of pragmatics, commonly referred to as the social use of language. Considering that individuals with a laryngectomy (IWL) frequently report reduced quality of life as well as reduced communicative competency, expanding the current resear…

Perception and encoding of narrative events during continuous speech listening in background noise.

Everyday speech comprehension involves interpreting overarching themes as they unfold continuously overtime and structuring experiences for effective encoding and future recall. Characterizing downstream cognition, particularly how the brain perceptually organizes and encodes information, can help elucidate some of the mechanisms underlying listening effort. Recent advances in memory research highlight event segmentation, the process of identifying distinct events within dynamic environments, as a core mechanism to how we perceive, encode, and recall experiences. In the current study, we exami

Automated Vowel Articulation Analysis in Connected Speech Among Progressive Neurological Diseases, Dysarthria Types, and Dysarthria Severities.

PURPOSE: Although articulatory impairment represents distinct speech characteristics in most neurological diseases affecting movement, methods allowing automated assessments of articulation deficits from the connected speech are scarce. This study aimed to design a fully automated method for analyzing dysarthria-related vowel articulation impairment and estimate its sensitivity in a broad range of neurological diseases and various types and severities of dysarthria. METHOD: Unconstrained monologue and reading passages were acquired from 459 speakers, including 306 healthy controls and 153 neu

Speech and swallow following Staffieri voice restoration procedure.

The Loyola University and Hines Veterans Administration Surgical Service completed the Staffieri procedure to construct a pharyngoesophageal flap at the time of total laryngectomy in 12 patients. Following surgery, 11 patients produced speech that was highly intelligible. Ultimately, seven continued to rely on this method of speech as their primary means of communication. Voice quality was judged to be rough, strained, wet, and breathy in a majority of patients. Aspiration of liquids was evident in 11 of 12. Four underwent revisions to reduce the size of the fistula and to correct swallowing p

[Investigations on the incidence of partial agnosia of sounds (author's transl)].

Stammering as a frequent speech disorder in infancy results from many causes. For instance, it is possible that a child with normal peripheral hearing may be unable to distinguish sounds by tone and therefore will repeat them incorrectly. A testing method is presented in which paired pictures are used which differ by the phonologic opposition of some stammered sounds. The results of the test in 100 children who stammer are reported. Further investigations give evidence that other perceptual disorders can be found in supposed agnosias.

What is the optimal assessment of speech? A multicentre, international evaluation of speech assessment in 2500 patients with a cleft.

OBJECTIVES: Speech problems in patients with a cleft palate are often complex and multifactorial. Finding the optimal way of monitoring these problems is challenging. The International Consortium of Health Outcomes Measurement (ICHOM) has developed a set of standardised outcome measures at specific ages for patients with a cleft lip and/or palate, including measures of speech assessment. This study evaluates the type and timing of speech outcome measures currently included in this ICHOM Standard Set. Additionally, speech assessments in other cleft protocols and initiatives are discussed. DESI

Tongue dysmorphology in craniofacial microsomia.

BACKGROUND: Craniofacial microsomia is one of the most common and well-characterized craniofacial anomalies. Tongue dysmorphism, however, has been neither thoroughly investigated nor reported in the context of this disease. This review focuses on the true prevalence of tongue dysmorphology in craniofacial microsomia and its relation to the deformities seen in this condition. METHODS: A 20-year retrospective study was performed to determine the number of patients who had a documented tongue anomaly and any relation to the development of abnormal speech. In recognition of the limitations of thi

Speech ratings by speech clinicians, parents and children.

Ratings of speech samples of children with cleft palate were obtained from speech clinicians in a Cleft Palate Clinic, speech clinicians in the public schools, parents of children with clefts, parents of children without clefts, children with clefts and children without clefts. Analyses of the obtained ratings suggest that nasality and articulation ratings obtained from adult groups do not differ appreciably. Correlations between ratings of nasality and articulation are interpreted as suggesting that speech clinicians are more likely to differentiate between these two variables than other list

Prescriptive hearing aid fitting by parameter adjustment and selection.

We describe the rationale for the development of a psychophysical procedure that allows individual listeners to adjust the frequency-gain characteristic of a programmable hearing aid on a unidimensional scale, based on a target-gain reference value. The listener's task is to adjust the frequency response and select the parametric value judged to optimize the intelligibility or quality of speech. Originally described as the Sliding Scale, the procedure has been modified for use in reliability and validity studies of hearing aid fitting at Michigan State University. The modified approach is refe
