Numerous surgical approaches have been used to treat petrous apex cholesterol granulomas. They are usually treated via the transtemporal- or middle fossa approach; some are managed endoscopically. We present a patient treated by the endoscopic transsphenoidal approach and review the literature.

Pituitary adenomas often present with the symptoms of hormonal hypersecretion, and although medical therapy is available for most hyperfunctioning states, it is not curative. As a result, transsphenoidal pituitary surgery has become a commonly performed neurosurgical procedure with unique challenges for the anesthesiologist due to the distinct medical comorbidities associated with various adenomas. Any type of pituitary tumor may also produce hypopituitarism and local mass effects secondary to the expanding intrasellar mass. Here we review the perioperative concerns surrounding surgery to remo

A seven-year-old child sustained a fracture of her basisphenoid resulting in profuse, life-threatening haemorrhage which could not be controlled with a post-nasal pack. The fracture site was identified using rigid endoscopy and packed with oxidized cellulose, resulting in immediate control of the haemorrhage. The use of the nasal endoscope in the management of posterior epistaxis is discussed.

OBJECTIVE: To examine the relationship of the clinoid segment of the internal carotid artery to the structures in the roof of the cavernous sinus and to determine whether this segment is neither intradural nor intracavernous, as recently proposed.
METHODS: The region of the roof of the cavernous sinus was dissected and examined using 3 to 40x magnification and micro-operative techniques.
RESULTS: The clinoid segment was located within a collar formed by the dura lining the medial surface of the anterior clinoid process, the posterior surface of the optic strut, and the upper part of the caro

Pituitary adenomas are uncommon in childhood. Between 1975 and 1988, 36 patients underwent transsphenoidal resection at the Mayo Clinic before their 17th birthday. Fifteen (41.7%) had prolactin (PRL)-secreting tumors, 16 (44.4%) had adrenocorticotropic hormone (ACTH)-secreting tumors (including two patients with Nelson-Salassa syndrome), and three (8.3%) had tumors secreting growth hormone (GH); two patients (5.6%) had clinically nonfunctioning tumors. Of the 36 patients, 21 (58.3%) were girls, and girls predominated in every group except that with ACTH-secreting tumors, where they accounted f

A case of extreme, recurrent epistaxis secondary to a ruptured aneurysm of the sphenopalatine artery is reported and diagnosis of the condition by angiography and its treatment by Gianturco coil embolization is discussed.

A 32-year-old woman had a history of left temporal and occipital bone defects since her infancy and she developed left exophthalmos in the last three years following a ormin head injury. The left eye ball was displaced downward and rotated nasally, and she was unable to move the left eye voluntarily beyond the mid-line. The left eye ball could not be rotated beyond the mid-line even by muscle traction. Skull x-rays and carotid angiograms showed various abnormalities of skull dysplasia and positional abnormalities of intracranial arteries. Pneumoencephalography demonstrated dilatation of the ar

A patient sustained a severe cranio-facial injury which included a transverse fracture of the middle cranial fossa through the sella turcica producing otorrhoea, rhinorrhoea, a bilateral abducents palsy and a large aero-coele. All gradually remitted spontaneously. The management of this patient and the patterns of cranial base fractures and their associated clinical features, particularly of the middle cranial fossa, are discussed.

In order to improve the current knowledge of the topography and the anatomy of the paranasal sinuses of the one-humped dromedary (Camelus dromedarius), we applied both conventional and advanced imaging techniques, namely, radiography and computed tomography (CT). Twelve heads of healthy dromedaries were used; eight heads were dissected to obtain skulls for the fenestration of the sinuses, two heads underwent anatomical sections, and two heads were imaged respectively by radiography and CT. Sinus fenestration allowed observation of sinuses and their communications. In each dissected dromedary's

The authors reviewed cranial imaging studies (radiographs, computed tomographic scans, and magnetic resonance [MR] images) in 13 infants and children with the autosomal recessive form of osteopetrosis to characterize patterns of skull base, brain, and cranial nerve involvement at presentation and with progression of disease. Marked sclerosis and deposition of osteopetrotic bone was noted along the anterior (but not posterior) occipitomastoid suture (n = 8), at the basioccipital-exoccipital synchondrosis (n = 9), and along the sphenooccipital synchondrosis (n = 8). Endobones, presumably represe

Spheno-orbital (SOM) meningioma is a rare subtype of skull base tumor characterized by the proliferation of arachnoid cap cells with intradural expansion, hyperostosis of calvarial and spheno-orbital bone, and consecutive infiltration of critical surrounding neurovascular structures of the frontotemporal base of the skull. Surgical resection is often challenging due to the infiltrative growth and the anatomically complex localization. In most cases, a microsurgical transcranial approach is performed, resulting in the need to reconstruct the orbit with or without a cranioplasty. This review hig

BACKGROUND: The skull base depicts significant morphological variability, which is frequently described due to its neurosurgical significance. The middle cranial fossa's accessory foramen has rarely been described.
MATERIALS: A 53-year-old female patient's computed tomography (CT) scan was further investigated for its unusual morphology.
RESULTS: On the left-sided middle cranial fossa, an accessory sphenoidal foramen (ASF) was observed, located 3.3 mm posterior to the foramen rotundum (FR) and 5.5 mm anterior to the foramen ovale (FO). Extracranially, the ASF opened into the infratemporal fo

Objective:To study the clinical anatomy of the sphenopalatine foramina by dissecting the sphenopalatine foramina during Vidian nerve branch neurotomy. The anatomy and CBCT images of sphenopalatine foramen were analyzed to facilitate the navigational of clinical operation using CBCT images. Methods:From October 2017 to September 2023, 84 cases(168 sides) of Vidian nerve branch neurotomy in our department were collected. The clinical summary was made according to the anatomy of sphenopalatine foramen during the operation. Preoperative CBCT imaging findings of the sphenopalatine foramina were als

A mixed germ cell tumor (MGCT) in the neurohypophysis is very rare, with only a few reported cases1-4 but none with surgical videos. In this report, the endoscopic endonasal transsphenoidal approach for MGCT in the neurohypophysis is presented (Video 1). A 12-year-old girl with ocular pain, fatigue, and nausea presented with gradual onset of quadrant hemianopsia and left oculomotor palsy. Magnetic resonance imaging showed an enhanced mass in the sella turcica with multiple components involving the pituitary gland and stalk. Her endocrinological examination showed decreased levels of pituitary

OBJECTIVE: Retrospective observation of computed tomographic images to describe an anatomical difference in the shape of the pterygoid bone in French bulldogs (FB) and determine its prevalence and effect on the cross-sectional area of the airway, in comparison with normal conformation in brachycephalic and mesocephalic dogs.
ANIMALS AND PROCEDURE: Computed tomographic studies of FB and cocker spaniels (CS) that underwent examination of the head were reviewed. The shape and cross-sectional area of the pterygoid bone was recorded and compared between groups.
RESULTS: Thirty-six CS and 34 FB we

The aim of this study was to compare optic canal parameters of syndromic craniosynostosis patients with those of normal patients to visit the possibility of optic nerve impingement as a cause of visual impairment. Computed tomography scan images were processed using the Materialise Interactive Medical Image Control System (MIMICS) Research 21.0 software (Materialise NV, Leuven, Belgium). Eleven optic canal parameters were measured: 1) height of optic canal on the cranial side, 2) height of optic canal on the orbital side 3) length of the medial wall of the optic canal, 4) length of the lateral
