Small ethmoid mucocele with intracranial extension associated with fibrous dysplasia: A rare case report.

Ethmoid mucoceles are expansile, benign cystic lesions characterized by slow growth, predominantly affecting the frontal sinus, followed by the ethmoid sinus, and infrequently the sphenoid and maxillary sinuses. Intracranial involvement is rare and is typically associated with large or longstanding lesions. We present a case involving a 34-year-old immunocompetent woman who experienced non-specific facial pain subsequent to an influenza infection without exhibiting typical sinonasal or neurological symptoms. Computed tomography and magnetic resonance imaging revealed a small ethmoid sinus lesi

Adaptive Treatment Strategy: Adjuvant Chemoradiotherapy for a Complex SMARCB1 (INI-1)-Deficient Sinonasal Carcinoma With Intracranial Extension.

SWI/SNF-related matrix-associated actin-dependent regulator of chromatin subfamily B member 1 (SMARCB1) (integrase interactor 1)-deficient sinonasal carcinoma (SDSNC) is a rare and aggressive malignancy of the head and neck. It is characterised by the absence of nuclear SMARCB1 expression and typically presents at advanced stages due to non-specific symptoms resembling benign conditions such as nasal polyps or sinusitis. This report describes a 64-year-old male who presented with a sphenoidal sinus mass causing bony erosion and intracranial extension, associated with headaches and diplopia. In

Exophthalmos and hemiheadache caused by osteoma in the greater wing of sphenoid bone: an extremely rare case report.

UNLABELLED: Osteoma is the most common benign tumor of the nose and paranasal sinuses. It is usually asymptomatic and diagnosed accidentally. In our case, the tumor formed in an unusual location and led to unexpected symptoms, which created a big challenge in diagnosis and treatment. CASE PRESENTATION: A 53-year-old woman complained of hemiheadache, exophthalmos in the right eye, and limitation in lateral eye movements progressing to diplopia in the past 2 months. The physical examination of the rest systems was unremarkable. The radiological investigations revealed a hyperdense lesion arisin

Repair of internal carotid artery injury with aneurysm clip during endoscopic endonasal surgery: illustrative case.

BACKGROUND: One of the most feared and dangerous scenarios that can appear during an endoscopic endonasal surgery (EES) is the iatrogenic injury of the internal carotid artery (ICA). Several methods, along with a variety of outcomes, have been described to deal with this complication. To the authors' knowledge, this is the first report on the use of a Yasargil-type aneurysm clip to solve an ICA injury, preserving the artery's patency and having a long-term follow-up. The authors discuss the advantages and disadvantages of other vessel preservation techniques compared with clipping. OBSERVATIO

IgG4-related intracranial disease.

IgG4-related disease (IgG4-RD) is a multi-organ chronic inflammatory process caused by infiltration of IgG4-positive plasma cells in one or more organs. Intracranial involvement has only recently become better recognized. Our case series adds to the growing literature on the varying presentations of intracranial IgG4 by describing the clinical and imaging findings of three patients who presented to our institution with intracranial involvement. Our first patient presented with a mass-forming IgG4 pachymeningitis mimicking a sphenoid wing meningioma, which is to our knowledge the largest mass-f

Nasal meatus plasty: a contribution to plastic reconstruction of the nasal valve during midfacial degloving.

Midfacial degloving is a proven method for easily accessing the midface, the nasal pyramid, the maxillary and ethmoidal sinuses, the orbits, as well as the anterior skull base. Indications for this method of access mainly include tumour resections in the area of the midface, the septum, the maxillary sinus, the paranasal to the sphenoidal sinus as well as the clivus. In addition, this method of access allows for the exposure of the bony structures of the midface in the event of extensive fractures. In general, this method of access combines an incision in the oral vestibule and circular incisi

Computed tomography of Paget disease of the skull versus fibrous dysplasia.

OBJECTIVE: Radiologists are often challenged to review CT examinations of the skull without pertinent clinical information or plain radiographs. Skull lesions of fibrous dysplasia (FD) may often be confused with Paget disease (PD). The purpose of this article is to evaluate radiographic similarities and to find the signs that can differentiate PD from FD of the skull on head CT and to describe the CT imaging features of PD and FD. DESIGN AND PATIENTS: CT scans of the skull in eight cases of PD, 18 cases of FD (13 cases of skull and facial bones, five cases of only facial bones) and 10 normals

A novel technique for skull base reconstruction in craniofacial fibrous dysplasia surgery using three-dimensional printing and a dental alginate mold: illustrative case.

BACKGROUND: Fibrous dysplasia is a rare and benign skeletal lesion characterized by fibrous tissue proliferation due to an abnormal osteogenesis replacing normal bone. OBSERVATIONS: An 18-year-old male with fibrous dysplasia of the left sphenoid, ethmoid, orbit, and frontal bones was managed with excision and skull base reconstruction. After complete removal of the tumor, skull base reconstruction was commenced by making a reverse temporalis flap and placing it over the opened paranasal sinuses for a robust vascularized graft, followed by an abdominal fat graft, and then a pedicled pericranal

Investigation of the relationship between facial injuries and traumatic brain injuries using a realistic subject-specific finite element head model.

In spite of anatomic proximity of the facial skeleton and cranium, there is lack of information in the literature regarding the relationship between facial and brain injuries. This study aims to correlate brain injuries with facial injuries using finite element method (FEM). Nine common impact scenarios of facial injuries are simulated with their individual stress wave propagation paths in the facial skeleton and the intracranial brain. Fractures of cranio-facial bones and intracranial injuries are evaluated based on the tolerance limits of the biomechanical parameters. General trend of maximu

Infraorbital canal variants and its clinical and surgical implications. A systematic review.

BACKGROUND: Recent literature highlights anomalous cranial nerves in the sinonasal region, notably in the sphenoid and maxillary sinuses, linked to anatomical factors. However, data on the suspended infraorbital canal (IOC) variant is scarce in cross-sectional imaging. Anatomical variations in the sphenoid sinuses, including optic, maxillary, and vidian nerves, raise interest among specialists involved in advanced sinonasal procedures. The infraorbital nerve's (ION) course along the orbital floor and its abnormal positioning within the orbital and maxillary sinus region pose risks of iatrogeni

A mucopyocele of the clivus: case report.

OBJECTIVE AND IMPORTANCE: There are increasing reports of nasal sinus mucopyoceles eroding the base of the cranium and causing the rapid onset of neurological complications. Uncommon presentations can mimic tumors, but the infectious nature mandates an urgent surgical decompression. It is important to document these uncommon presentations. CLINICIAL PRESENTATION: A mucopyocele of the clivus is reported in a patient with a frequent history of sinusitis. Diplopia caused by an acute sixth nerve palsy prompted the radiological diagnosis with computed tomography and magnetic resonance imaging. INT

[Spontaneous rhinorrhea of a rare etiology (apropos of empty sella turcica)].

The authors report a case of non traumatic rhinorrhea due to an empty sella turcica. Such an intra sellar arachnoidocele compress the pituitary gland against the posterior wall, erode the floor and lead to CSF rhinorrhea through the sphenoidal sinus. Diagnosis and treatment are best accomplished by a team approach involving otolaryngologist, neurosurgeon and neuroradiologist. CT Scan and IRM explain the diagnosis. Treatment can be accomplished by a transeptal transphenoidal approach with localization and repair of the leak in the absence of increased intracranial pressure.

The sphenoidal emissary foramen and the emissary vein: Anatomy and clinical relevance.

Although the sphenoidal emissary foramen (SEF) and its content are anatomically and clinically relevant, accurate description of them in the modern literature is lacking. This study aimed to examine and describe the SEF and its content (the sphenoidal emissary vein [SEV]). We analyzed 1,000 computed tomography (CT) images, 170 dry skulls, 50 formalin-fixed specimens, and three specimens (heads) following guidelines proposed by Dr. Albert L. Rhoton Jr. MD for latex injection. SEV morphology was determined by histological staining and electron microscopy. The SEF was observed in 46.8% of the CTs
