Ankle taping can improve proprioception in healthy volunteers.

BACKGROUND: Taping is a common technique to treat inversion ankle sprains, but the effects of tape on proprioception are still not clear. The purpose of this study was to establish whether proprioception would be modified by the use of ankle tape. To do so we studied joint position sense (JPS) in healthy volunteers using three-dimensional (3D) movement analysis systems. MATERIAL AND METHODS: Forty healthy volunteers (mean age and SD = 23.2 ± 4.2) were asked to actively reproduce four different ankle positions: 10 degrees of dorsiflexion, neutral, 10 degrees and 20 degrees of plantarflexion. Hamstring muscle forces prior to and immediately following an acute sprinting-related muscle strain injury.

A thorough understanding of the biomechanics of the hamstrings during sprinting is required to optimise injury rehabilitation and prevention strategies. The main aims of this study were to compare hamstrings load across different modes of locomotion as well as before and after an acute sprinting-related muscle strain injury. Bilateral kinematic and ground reaction force data were captured from a single subject whilst walking, jogging and sprinting prior to and immediately following a significant injury involving the right semitendinosis and biceps femoris long head muscles. Experimental data w

Contributing factors to chronic ankle instability.

BACKGROUND: The development of repetitive ankle sprains and persistent symptoms after initial ankle sprain has been termed chronic ankle instability (CAI). There is no clear indication of which measures are most important in discriminating between individuals with and without CAI. METHODS: Thirty subjects with unilateral CAI and controls had measures of ankle laxity and hypomobility, static and dynamic balance, ankle and hip strength, lower extremity alignments, and flexibility taken on both limbs. RESULTS: Based on comparisons of CAI ankles and side-matched limbs in controls, the measures s

Acute ankle sprain: an update.

Acute ankle injury, a common musculoskeletal injury, can cause ankle sprains. Some evidence suggests that previous injuries or limited joint flexibility may contribute to ankle sprains. The initial assessment of an acute ankle injury should include questions about the timing and mechanism of the injury. The Ottawa Ankle and Foot Rules provide clinical guidelines for excluding a fracture in adults and children and determining if radiography is indicated at the time of injury. Reexamination three to five days after injury, when pain and swelling have improved, may help with the diagnosis. Therap

Altered neuromuscular control and ankle joint kinematics during walking in subjects with functional instability of the ankle joint.

BACKGROUND: The ankle joint requires very precise neuromuscular control during the transition from terminal swing to the early stance phase of the gait cycle. Altered ankle joint arthrokinematics and muscular activity have been cited as potential factors that may lead to an inversion sprain during the aforementioned time periods. However, to date, no study has investigated patterns of muscle activity and 3D joint kinematics simultaneously in a group of subjects with functional instability compared with a noninjured control group during these phases of the gait cycle. PURPOSE: To compare the p

Ankle syndesmosis injuries: anatomy, biomechanics, mechanism of injury, and clinical guidelines for diagnosis and intervention.

Syndesmosis injuries are rare, but very debilitating and frequently misdiagnosed. The purpose of this clinical commentary is to review the mechanisms of syndesmotic injuries, clinical examination methods, diagnosis, and management of the injuries. Cadaveric studies of the syndesmosis and deltoid ligaments are also reviewed for further understanding of stress transmission and the roles of different structures in stabilizing the distal syndesmosis. External rotation and excessive dorsiflexion of the foot on the leg have been reported as the most common mechanisms of injury. The injury is most of

Trampoline exercise vs. strength training to reduce neck strain in fighter pilots.

INTRODUCTION: Fighter pilots' muscular strength and endurance are subjected to very high demands. Pilots' fatigued muscles are at higher risk for injuries. The purpose of this study was to compare the effects of two different training methods in reducing muscular loading during in-flight and cervical loading testing (CLT). METHODS: There were 16 volunteer Finnish Air Force cadets who were divided into 2 groups: a strength training group (STG) and a trampoline training group (TTG). During the 6-wk training period, the STG performed dynamic flexion and extension and isometric rotation exercises

Functional ankle control of rock climbers.

OBJECTIVE: To evaluate whether rock climbing type exercise would be of value in rehabilitating ankle injuries to improve ankle stability and coordination. [figure: see text] METHODS: A group of 25 rock climbers was compared with a group of 26 soccer players. All were male, uninjured, and exercised three to four times a week. Active ankle stability was evaluated by one leg stand stabilometry (measurement of migration of the centre of gravity) and measurements of maximum strength of ankle isokinetic concentric flexion and extension (Cybex). RESULTS: The rock climbers showed significantly bette

Peroneus longus muscle activation pattern during gait cycle in athletes affected by functional ankle instability: a surface electromyographic study.

BACKGROUND: Functional ankle instability is a clinical syndrome that may develop after acute lateral ankle sprain. Although several causes of this functional instability have been suggested, it is still unclear what the activation pattern of the peroneus longus muscle is in patients with functional ankle instability. HYPOTHESIS: Peroneus longus activation patterns differ in the injured side and the uninjured side in subjects with functional ankle instability. STUDY DESIGN: Descriptive laboratory study. METHODS: The authors examined 14 subjects with functional ankle instability by using surf

Comparison of three preventive methods in order to reduce the incidence of ankle inversion sprains among female volleyball players.

BACKGROUND: Ankle sprains are the most common form of acute injury in volleyball. A prevention programme consisting of technical training, proprioceptive training, and external support was previously designed to reduce the rate of ankle sprains in volleyball players. OBJECTIVE: To investigate which of these three interventions is the most effective in preventing ankle sprain in female volleyball players. METHODS: Participants were 52 players who suffered ankle sprains during the season 1998-1999. They were divided randomly into three preventive groups: group 1 (n = 18) followed the technical

Pseudoaneurysm of the anterior tibial artery after ankle arthroscopy.

Ankle arthroscopy is believed to have the least vascular complications compared with other types of arthroscopies. We present a case of traumatic pseudoaneurysm of the anterior tibial artery after ankle arthroscopy, an extremely rare complication that was treated surgically and had an uneventful recovery.

Complex regional pain syndrome type I associated with amyotrophic lateral sclerosis.

BACKGROUND: (CRPS I [formerly called reflex sympathetic dystrophy]) is a syndrome with pain and signs of autonomic dysfunction after trauma or immobilization; the pathophysiologic mechanisms of CRPS I, however, remain unknown. DESIGN: The authors present a case of CRPS I associated with amyotrophic lateral sclerosis. Along with the motor paresis due to amyotrophic lateral sclerosis, pain, swelling, and signs of autonomic disturbance occurred. CONCLUSIONS: This case supports the hypothesis that immobilization is one of the major contributing factors for CRPS I.

Anatomical and possible clinical relationships between the calcaneofibular ligament and peroneus brevis--a pilot study.

The authors have observed in their clinical practice patients presenting with chronic retromalleolar pain following lateral ankle injuries. It has been hypothesized that persistent retromalleolar pain following a supination sprain may be due to peroneus brevis (PB) tendon tears (Boruta et al. 1990). The aims of this study were to investigate whether an anatomical relationship exists between the calcaneofibular ligament (CFL) and PB, and if so, the significance of this relationship in the positions of supination sprain and talar tilt test. Seven out of eight cadaveric ankles demonstrated fibrou

The effect of ankle braces on the prevention of dynamic forced ankle inversion.

BACKGROUND: Athletes often employ prophylactic braces to reduce the risk of ankle injuries. HYPOTHESIS: Ankle braces do not significantly decrease the risk of forced inversion on a standardized one-footed jump landing. STUDY DESIGN: Controlled laboratory study. METHODS: Fourteen healthy men with a mean age of 25.1 years were tested. Three braces, two semirigid (Aircast and Bledsoe) and one lace-up (Swede-O), were fitted to each subject. Forced dynamic ankle inversion of 24 degrees was to be resisted as the subjects landed on one foot with a force of two body weights on a stimulus presented

Mechanically induced ankle inversion during human walking and jumping.

A new method to study sudden ankle inversions during human walking and jumping is presented. Ankle inversions of 25 degrees were elicited using a box containing a trap door. During the gait task, subjects walked at a speed of 4 km/h. At a pre-programmed delay after left heel strike, an electromagnet released the box on the treadmill. This delay enabled the subject to step on the box without having to change the walking cadence. During the jumping task, subjects jumped from a 30 cm high platform on the box in a standardised way. In both tasks 20 stimulus and 20 control trials were presented ran

Clinical perspectives regarding eccentric muscle injury.

Muscle strain injuries occur to predictable muscles at consistent locations during expected sporting maneuvers when a muscle is stretched and then activated, particularly during high intensity bursts of activity. More than 30% of the injuries seen in the clinician's office are injuries to skeletal muscle. The typical location of the injury is just proximal to the distal muscle tendon junction regardless of strain rate or architecture of the muscle. After the injury, the muscle is weaker, continues to weaken, then recovers during the next week. An inflammatory response is seen in the following

Regional distribution of axial strain and circumferential residual strain in the layered rabbit oesophagus.

The oesophagus is subjected to large axial strains in vivo and the zero-stress state is not a closed cylinder but an open circular cylindrical sector. The closed cylinder with no external loads applied is called the no-load state and residual strain is the difference in strain between the no-load state and zero-stress state. To understand oesophageal physiology and pathophysiology, it is necessary to know the distribution of axial strain, the zero-stress state, the stress-strain relations of oesophageal tissue, and the changes of these states and relationships due to biological remodeling of t
