Serotyping and antibiotic resistance of Streptococcus pneumoniae isolated from pediatric infections in central Greece.

OBJECTIVE: To determine the distribution of serogroups/serotypes and antibiotic resistance pattern of Streptococcus pneumoniae isolated from pediatric infections in central Greece. METHODS: In total, 306 S. pneumoniae strains isolated from children, aged from 18 days to 14 years (median 18 months), during a 21-month period, from different specimen sources, were studied. Susceptibility testing was carried out by the Kirby---Bauer method and by the Etest, and serotyping by the Quellung reaction. RESULTS: Of the S. pneumoniae isolates, 3.9% were highly resistant to penicillin (PR), while 17.6% we…

Moxifloxacin: a comparison with other antimicrobial agents of in-vitro activity against Streptococcus pneumoniae.

Two hundred representative isolates, including 26 strains of Streptococcus pneumoniae with intermediate resistance to penicillin, were selected from a collection obtained from blood cultures of patients with bacteraemic pneumococcal pneumonia. The MICs of moxifloxacin (BAY 12-8039), grepafloxacin, sparfloxacin, levofloxacin, ofloxacin, ciprofloxacin, erythromycin, tetracycline and penicillin G were determined by a standard agar dilution technique. Moxifloxacin had the highest in-vitro activity against S. pneumoniae (MIC90 = 0.25 mg/L; MIC range 0.06-0.25 mg/L). The MIC90 values were one diluti…

[Prevalence of antimicrobial resistance among clinical isolates of Streptococcus pneumoniae in a children's hospital].

Eleven hundreds and seventy-six strains of Streptococcus pneumoniae were isolated from pediatric clinics of Chiba Children's Hospital during 1990 through 1995. Annual penicillin-resistant rates of these strains were as follows; 24.0% (1990), 29.0% (1991), 36.2% (1992), 55.8% (1993), 58.6% (1994), and 59.3% (1995). Overall penicillin-resistance during these 6 years was 45.8%. Nine out of 11 cases of systemic pneumococcal infections were due to penicillin-resistant S. pneumoniae (PRSP) during the same period. Of PRSP strains, their PCG-MIC levels had become higher and their spectra of resistance…

[Invasive extrapulmonary infections due to Streptococcus pneumoniae. Report of three fatal cases and review].

Streptococcus pneumoniae is a most important patogen in respiratory and meningeal infections. It is also a relevant agent of septic arthritis, pericarditis, acute endocarditis and spontaneous peritonitis in cirrhotic patients with ascites. It is less well known as a cause of infections in many other sites, including abdominal organs and soft tissues, which may be very severe. In this report, we describe three fatal cases (phlegmonous gastritis, cellulitis and primary peritonitis without pre-existing ascites) due to Streptococcus pneumoniae. In the last years it has become clear that this agent…

Meropenem susceptibility of Neisseria meningitidis and Streptococcus pneumoniae from meningitis patients in The Netherlands.

In-vitro susceptibility of 299 Neisseria meningitidis and 157 Streptococcus pneumoniae strains from meningitis patients in The Netherlands in 1993 and 1994 to meropenem was determined using the Etest. Susceptibility to penicillin, ceftriaxone, and chloramphenicol was also determined. Rifampicin susceptibility was additionally tested for N. meningitidis. Of the meningococci, 4.3% were of intermediate resistance to penicillin and 0.3% were resistant to rifampicin. One pneumococcal isolate (0.6%) was of intermediate resistance to penicillin. All strains were susceptible to meropenem. We conclude …

Comparative serum bactericidal activities of ceftizoxime and cefotaxime against intermediately penicillin-resistant Streptococcus pneumoniae.

In a randomized crossover study involving 12 healthy volunteers, 1 g of ceftizoxime or cefotaxime was administered intravenously every 12 h for a total of three doses on two separate weekends. The duration of serum bactericidal titers (SBTs) greater than 1:2 and the time serum drug concentrations remained above the MIC (T > MIC) were determined against three clinical isolates of Streptococcus pneumoniae with intermediate resistance to penicillin. The duration of SBTs and T > MIC for both antimicrobial agents exceeded 50% of the dosing interval for all isolates. Ceftizoxime's T > MIC was statis…

Susceptibility of Streptococcus pneumoniae strains isolated in Italy to penicillin and ten other antibiotics.

The susceptibility of 312 Streptococcus pneumoniae strains isolated during 1993 in Italy has been studied. Overall resistance to penicillin, erythromycin and co-trimoxazole was 5.5, 6 and 18% respectively. The penicillin resistant isolates (nine displaying low- and eight high-level resistance) were further tested against cefotaxime, ceftriaxone, cefepime, cefodizime, ceftazidime, imipenem, meropenem and rifampicin. The low-level penicillin resistant strains were inhibited by all these drugs. Among the eight high-level penicillin resistant pneumococci, four were resistant to one or more cephalo…

The spread of multiply resistant Streptococcus pneumoniae at a day care center in Ohio.

Streptococcus pneumoniae, type 23F, resistant to penicillin (MIC, 2 micrograms/mL) and multiple other antimicrobic agents, was isolated from middle ear fluid of a child with otitis media attending a day care center in Ohio. To determine the extent of spread of this strain, nasopharyngeal culture surveys were done, and 52 carriers were identified among 250 children attending the index day care center. No carriers were found among 121 children at two other day care centers in the same urban area. Use of prophylactic doses of antibiotics (P < .001) and frequent use of antibiotics (P < 0.001) were…

Trimethoprim-sulfamethoxazole vs. ampicillin in the treatment of experimental meningitis due to Streptococcus pneumoniae.

When given in combination, both trimethoprim and sulfamethoxazole penetrated well into the cerebrospinal fluid of rabbits with experimental pneumococcal meningitis, reaching concentrations that should have been adequate for synergistic killing of Streptococcus pneumoniae. However, trimethoprim-sulfamethoxazole was less effective than ampicillin in the treatment of this experimental infection. The effect of the combination on pneumococci in vivo was bacteriostatic rather than bactericidal, possibly because the number of pneumococci in the cerebrospinal fluid of the rabbits at the start of treat…

Mosaic pbpX genes of major clones of penicillin-resistant Streptococcus pneumoniae have evolved from pbpX genes of a penicillin-sensitive Streptococcus oralis.

Penicillin-resistant clinical isolates of Streptococcus pneumoniae contain mosaic penicillin-binding protein (PBP) genes that encode PBPs with decreased affinity for beta-lactam antibiotics. The mosaic blocks are believed to be the result of gene transfer of homologous PBP genes from related penicillin-resistant species. We have now identified a gene homologous to the pneumococcal PBP2x gene (pbpX) in a penicillin-sensitive Streptococcus oralis isolate M3 from South Africa that diverged by almost 20% from pbpX of penicillin-sensitive pneumococci, and a central sequence block of a mosaic pbpX g…

GpoA glutathione peroxidase links oxidative stress response, antibiotic persistence, and virulence in Streptococcus pneumoniae.

Glutathione peroxidases are conserved enzymes found in prokaryotic and eukaryotic organisms that reduce H2O2 to protect cells from oxidative stress damage. Here, we identified and characterized the GpoA glutathione peroxidase of Streptococcus pneumoniae, one of the most important human bacterial pathogens. We demonstrated that recombinant GpoA exhibits glutathione peroxidase activity and that the Cys36 residue is essential for this function. We conducted a comparative analysis of the phenotypic characteristics linked to GpoA and those linked to other peroxidases, including the TpxD thiol perox…

Fulminant Bilateral Streptococcus pneumoniae Endophthalmitis in a Kidney Transplant Patient With Prior Herpetic Keratopathy.

Streptococcus pneumoniae endophthalmitis is an uncommon but highly aggressive intraocular infection, particularly in immunocompromised patients. Rapid clinical deterioration with irreversible visual loss and systemic complications is frequent. This report describes a fatal case of bilateral pneumococcal endophthalmitis in a kidney transplant recipient with chronic herpetic keratopathy. A 58-year-old woman with stable renal graft function and a history of recurrent herpetic keratitis underwent left corneal transplantation. Fifty days postoperatively, she presented with worsening ocular pain and…

Hyperbaric Oxygen Therapy for Sensorineural Hearing Loss from Streptococcus pneumoniae Meningitis: A Case Report.

Sudden bilateral sensorineural hearing loss (SNHL) is a rare but debilitating complication of bacterial meningitis, which is most commonly caused by Streptococcus pneumoniae . While corticosteroids are a standard intervention to reduce inflammation and protect auditory function, adjunctive hyperbaric oxygen therapy (HBOT) may offer additional benefit, particularly in steroid-refractory cases. We report the case of a 57-year-old woman who developed profound bilateral SNHL following S. pneumoniae meningitis originating from a lumbar epidural abscess. After initiating HBOT, she experienced signif…

Epidemiological, immunological and virulence characteristics of persistent Streptococcus pneumoniae vaccine serotypes following vaccine introduction.

Streptococcus pneumoniae vaccine serotypes can persist despite pneumococcal conjugate vaccine (PCV) introduction. To examine serotype persistence, we leverage 6,545 nasopharyngeal swabs collected from children hospitalised with pneumonia before and after PCV13 introduction in Mongolia. DNA microarray was used to identify changes in lineage composition within serotypes 6A, 6B, 14, 19F and 23F over the six-year surveillance period. Children carrying post-PCV lineages are less likely to have severe pneumonia (serotypes 6B, 14 and 19F) and be multi-drug resistant (serotypes 6A, 6B and 23F) than pr…

Commensal to pathogen switch in Streptococcus pneumoniae is influenced by a thermosensing master regulator.

Opportunistic pathogens switch from a commensal to pathogenic state by sensing and responding to a variety of environmental cues, including temperature fluctuations. Minor temperature oscillations can alert the pathogen to a changing niche ecosystem, necessitating efficient sensing and rapid integration to trigger behavioral change. This is typically achieved through master regulators, dictating pleiotropic phenotypes. Here, we uncover a pivotal role of minor temperature shifts in transition of Streptococcus pneumoniae (SPN) from commensal to virulent lifestyles, mediated via an RNA thermosens…

A case of infectious endocarditis and vertebral discitis caused by Streptococcus pneumoniae serotype 23A.

Streptococcus pneumoniae, is a Gram-positive diplococcus, causes various community-acquired infections. Although pneumococcal vaccination has reduced the incidence of invasive pneumococcal disease, relative incidence of non-vaccine serotype-associated infections has increased. Herein, we report a case of pneumococcal infectious endocarditis and vertebral discitis diagnosed concurrently. A 73-year-old man presented with a 1-week history of fever and back pain (day 0). Laboratory tests revealed elevated inflammatory markers and magnetic resonance imaging confirmed vertebral osteomyelitis. Empiri…
