The last 15 years have witnessed a resurgence of the role of surgical options for prevention of ischemic stroke. The landmark randomized trials including NASCET and ECST were published and explored the role of carotid endarterectomy in this regard. The authors report the case of a woman who developed a peripheral polyneuropathy after heat stroke. All the classical aetiologies of neuropathy were excluded. The final diagnostic was residual peripheral neuropathy provoked by heat stroke. The sequella of heat stroke are dominated by cerebellar compromise, but this case shows that peripheral polyneuropathies exist even if they are rare. They raise the issue of care because of severe neurological sequella.

The objective of this study was to highlight the gastrointestinal problems that occur in stroke survivors, which may also reduce their quality of life. Stroke patients admitted over an 18-month period were evaluated for common gastrointestinal symptoms as well as type and site of stroke. The advent of new imaging techniques, such as diffusion-weighted sequences, has a great impact on the diagnostic imaging in stroke. The aim of the present paper is to review the brain imaging patterns after stroke, highlight the usefulness of imaging technique for the diagnosis and etiological work-up after stroke. Intracerebral haemorrhage (ICH) is much less common than ischaemic stroke (15% versus 85% in most Western studies), but is associated with a significantly worse prognosis. ICH is much more common in Asian populations, probably reflecting higher rates of small vessel disease, hypertension and genetic factors. There have been major advances in stroke treatment based on robust evidence from large clinical trials. This review covers both acute treatment and secondary prevention. Cerebellar syndromes and radiologic cerebellar atrophy after hyperpyrexia have occasionally been reported, mostly in neuroleptic malignant syndromes, but neuropathologic studies are extremely rare. In recent years there has been increasing interest in the relationship between obstructive sleep apnoea and stroke. It is clear that many patients who have had a stroke have marked obstructive sleep apnoea. This is seen during recovery but also during the acute phase when transient hypoxaemia and the blood pressure swings associated with upper airway obstruction, may worsen the ischaemic penumbra of the area of the brain which is compromised, leading to a worse outcome. There is some evidence to support this hypothesis. This article explores these issues.
