Minimally invasive anesthesia in wide awake hand surgery.

Tumescent minimally invasive local anesthesia is eliminating the need for sedation and proximal nerve blocks as well as all of their risks, costs, and inconveniences. It has facilitated advances in procedures such as hand fracture reduction, tendon repair, and tendon transfer by allowing the surgeon to see cooperative patient active movement during the surgery. It has improved the patient experience for simple hand operations such as carpal tunnel release.

Use of a Hunter Rod for Staged Reconstruction of Peroneal Tendons.

Peroneal tendon pathology is a commonly reported cause of lateral ankle pain. The causes include cavovarus foot type, overuse, chronic tendi … mplant to stimulate generation of a healthy peroneal tendon sheath to host a flexor hallucis longus tendon transfer. This has proved to be a successful treatment option for patients with severe peroneal tendon damage and scarring along the peroneal tendon sheath. We offer a sample case to illustrate a patient with such indications.

The First Experience of Triple Nerve Transfer in Proximal Radial Nerve Palsy.

BACKGROUND: Injury to distal portion of posterior cord of brachial plexus leads to palsy of radial and axillary nerves. Symptoms are usually motor deficits of the deltoid muscle; triceps brachii muscle; and extensor muscles of the wrist, thumb, and fingers. Tendon transfers, nerve grafts, and nerve transfers are options for surgical treatment of proximal radial nerve palsy to restore some motor functions. Tendon transfer is painful, requires a long immobilization, and decreases donor muscle strength; nevertheless, nerve transfer produces promising outcomes. We present a patient with proximal r…

[Secondary replacement operations for reconstruction of elbow joint function after lesion of the brachial plexus].

Elbow flexion plays a key role in the overall function of the upper extremity. In the case of unilateral complete brachial plexus lesion, re … ion appropriately, to be aware of all treatment possibilities and to recall the special problems of tendon transfer for brachial plexus patients. Our concept is based on our experience with more than 1100 patients presenting a brachial plexus lesion between 1981 and 1996 and treated in our institution. There were 528 operative revisions of the brachial plexus. So …

Achilles tendon reconstruction after sural fasciocutaneous flap using Achilles tendon allograft with attached calcaneal bone block.

Addressing segmental loss of the Achilles tendon with overlying soft tissue loss is a serious challenge. We present a case of Achilles tendon reconstruction in a patient who had significant soft tissue loss as well as segmental loss of the tendon involving the calcaneal insertion. The staged reconstruction was undertaken with a combination of a sural fasciocutaneous flap and an Achilles tendon allograft with an attached calcaneal bone block.

[The Extensor Pollicis et Indicis: An Accessory or Rudimentary Deep Extensor Tendon to the Thumb? Variants - Frequency - Clinical Relevance].

PURPOSE/BACKGROUND: Are the supernumerary deep extensor tendons to the thumb in man of accessory or rudimentary origin? METHOD AND CLINICAL MATERIAL: Two findings of extensor pollicis et indicis (EPI) were classified referring to the literature of comparative anatomy. RESULTS: Extensor pollicis longus (EPL) and extensor indicis (EI) developed phylogenetically from EPI. A common origin and proximal muscle belly of EPL and EI is a frequent manifestation of incomplete rudimentary EPI. A complete rudimentary EPI with absent EPL and EI has not yet been found in man. The accessory EPI coexisting w

Bilateral spontaneous rupture of flexor digitorum profundus tendons.

Spontaneous tendon rupture is an unusual condition usually associated with underlying disease processes such as rheumatoid arthritis, chronic renal failure or bony abnormalities of the hand. We report a case of spontaneous, non-concurrent bilateral rupture of flexor profundus tendons in an otherwise healthy individual. Treatment was successful and consisted of a two-stage reconstruction of the ruptured tendon.

The treatment of superior oblique myokymia utilizing the Harada-Ito procedure.

A woman with superior oblique myokymia (SOM) was cured of her condition by performing a Harada-Ito procedure. This procedure involves transposing the anterior portion of the superior oblique tendon, which is responsible for cyclorotation, nasally to create an effective weakening of the anterior portion of the tendon instead of temporal displacement utilized for superior oblique paresis. We measured the patient's eye movements before and after surgery, using the magnetic search coil technique, and confirmed that (1) the SOM was abolished and (2) vertical eye movements, including saccades, were

The Bridle procedure: a new treatment for equinus and equinovarus deformities in children.

The Bridle procedure is a tritendon anastomosis between the posterior tibialis, anterior tibialis, and peroneus longus, combined with an Achilles tendon lengthening for treating equinus and equinovarus deformities. The technique avoids problems of tendon attachment to bone and tendon placement for balance. One hundred seven procedures were performed on patients with cerebral palsy with 74% excellent and good results overall. The average follow-up was 5 years 9 months. The procedure was also performed with mixed results, on smaller groups of patients with other neuromuscular diseases.

Repair of rupture of flexor pollicis longus by "Z" lengthening at the wrist.

Rupture of the flexor pollicis longus in the digital canal can be repaired by one of the following methods: a) end-to-end suture, b) tendon grafting, c) transplantation of the flexor sublimis tendon of the ring finger, d) fixation of the proximal stump to the base of the second phalanx following elongation at the wrist. The efficacy of the first two methods has been confirmed by many authors on the basis of extensive clinical experience, but very few reports have been made on the other two techniques. The authors therefore report eleven cases of repair of the flexor pollicis longus by the tech

Rebalancing the Spastic Wrist by Transposition of Antagonistic Muscle-Tendon Complex.

Upper limb spasticity and spasticity-induced deformities after upper motor neuron lesions because of traumatic brain injury, encephalitis or cerebral palsy inhibit activities of daily living, result in impaired self-care and often dependence on assistance of carers. A key element of the dysfunction is wrist hyper-flexion and ulnar deviation deformity. Traditionally, this deformity has been corrected by transfer of the spastic flexor carpi ulnaris to the extensor carpi radialis brevis. Instead, this study emphasizes the causative role of the palmar subluxation of extensor carpi ulnaris and desc

Limited plantar incision for flexor digitorum longus tendon harvest in surgical treatment of tibialis posterior tendon insufficiency.

BACKGROUND: Surgical correction of stage II tibialis posterior tendon dysfunction (TPTD) commonly utilises the Flexor Digitorum Longus (FDL) tendon to augment the tibialis posterior tendon. The aim of this study is to present our experience and clinical outcomes harvesting the FDL via a limited plantar incision technique. METHODS: 25 flat foot operations for stage II TPTD were performed harvesting the FDL via a limited plantar incision centred half-way between the base of the heel to the base of the 2nd toe and two-thirds from the lateral border of the foot. RESULTS: The FDL was isolated wit

Two-stage extensor tendon graft using the Paneva-Holevitch procedure: A new technique.

Reconstruction of the extensor tendons remains a therapeutic challenge. Tendon transfers and grafts are a potential source of morbidity at the donor site, and the graft stock is limited. In the index finger, the tendon of the extensor indicis proprius can be anastomosed to the tendon of the extensor digitorum, and then the extensor digitorum tendon turned over after being cut at the forearm. We assessed the feasibility of this reconstruction on 12 upper limbs from 6 cadavers and we report the case of a 24-year-old patient who suffered destruction of the extensor apparatus in the index and midd…

The Lisfranc Amputation: A More Reliable Level of Amputation With Proper Intraoperative Tendon Balancing.

Traditional transmetatarsal amputations are a reliable level of amputation. However, amputations at the Lisfranc level have met with limited success owing to improper biomechanics resulting from tendon imbalance, ultimately leading to foot deformity positions and an unstable soft tissue envelope with ensuing skin breakdown, infection, and below-the-knee amputation. We describe proper tendon rebalancing that results in improved biomechanics and a more reliable and stable amputation at the more proximal Lisfranc level.
