Adherence to community versus facility-based delivery of monthly malaria chemoprevention with dihydroartemisinin-piperaquine for the post-discharge management of severe anemia in Malawian children: A cluster randomized trial.

BACKGROUND: The provision of post-discharge malaria chemoprevention (PMC) in children recently admitted with severe anemia reduces the risk of death and re-admissions in malaria endemic countries. The main objective of this trial was to identify the most effective method of delivering dihydroartemesinin-piperaquine to children recovering from severe anemia. METHODS: This was a 5-arm, cluster-randomized trial among under-5 children hospitalized with severe anemia at Zomba Central Hospital in Southern Malawi. Children were randomized to receive three day treatment doses of dihydroartemesinin-pi

Interventions for quitting vaping.

RATIONALE: There is limited guidance on how to stop using nicotine-containing vapes (otherwise known as e-cigarettes) and ensure long-term abstinence, whilst minimising the risk of tobacco smoking and other unintended consequences. Treatments could include pharmacological interventions, behavioural interventions, or both. OBJECTIVES: To conduct a living systematic review assessing the benefits and harms of interventions to help people stop vaping compared to each other, placebo or no intervention. To assess how these interventions affect the use of combustible tobacco, and whether effects var

Two-Way Clinical Messaging in a CML Specialty Pharmacy Service Model.

BACKGROUND: Adherence to treatment is correlated with treatment success in chronic myeloid leukemia (CML). CVS Specialty explored novel methods to improve adherence in this population to ensure optimal adherence and lower the risk of unsuccessful treatment. One novel program explored involved an interactive 2-way clinical messaging technology that coaches patients with adherence and knowledge about taking their treatment and managing their conditions. Clinical secure messaging is a 2-way messaging program distinct from the 1-way refill reminders and order status messages that patients were rec

Nurses' Roles in Supporting Digital Engagement and Self-Management in Adults with Type 2 Diabetes: A Scoping Review.

Background: Adults with type 2 diabetes increasingly use patient portals, telemonitoring systems, mobile applications, text messaging programs, and other digital services to support self-management. In practice, however, these approaches often still depend on nursing support to help patients understand, use, and sustain digital care in everyday settings. This scoping review mapped how nurses are involved in supporting adults with type 2 diabetes to use digital tools, information, and services for self-management across care settings. Methods: This scoping review followed Joanna Briggs Institut…

Using Digital Outreach to Identify Individuals Eligible for Lung Cancer Screening: A Secondary Analysis of a Randomized Trial.

BACKGROUND: Lung cancer is the third most common cancer in the United States, but screening rates among high-risk adults remain low. In a multisite randomized trial conducted in two large academic health systems in North Carolina, we found that a digital health intervention delivered to patients at home increased lung screening (LCS) rates. While LCS reduces mortality, identifying and reaching eligible patients is challenging and limits screening uptake. OBJECTIVE: This study examines the reach of a digital health outreach strategy for determining eligibility for LCS. DESIGN: A secondary ana

Implementation of Quitline Financial Incentives to Increase Counseling Sessions Among Adults Who Use Menthol Tobacco Products.

Since 2017, the Vermont Tobacco Control Program (VTCP) has worked to reduce the impact of flavored tobacco products on Vermonters. With the proposed U.S. Food and Drug Administration (FDA) rules banning menthol cigarettes and flavored cigars and proposed legislation banning sales of all menthol and flavored tobacco products in Vermont, VTCP prioritized resources to support cessation among Vermonters who use menthol tobacco products. In March 2021, VTCP began offering a tailored quitline protocol for adults who use menthol tobacco, including financial incentives, for completed coaching sessions

Long-term independent use of an intracortical brain-computer interface for speech and cursor control.

Brain-computer interfaces (BCIs) can provide naturalistic communication and digital access to people with severe paralysis by decoding neural activity associated with attempted speech and movement. Recent work has demonstrated highly accurate intracortical BCIs for speech and cursor control, but two critical capabilities needed for practical viability were unmet: independent at-home operation without researcher assistance and reliable long-term performance supporting accurate speech and cursor decoding. Here we demonstrate the independent and near-daily use of a multimodal BCI with novel brain

Remote monitoring to improve engagement for patients with inflammatory bowel disease: a randomized clinical trial.

BACKGROUND: Biologic therapy has improved care for patients with inflammatory bowel disease (IBD), but adherence is often suboptimal, and a gap in care is lack of engagement between visits. We evaluated the effect of a behaviorally-informed remote text monitoring program on outcomes and adherence among patients with IBD. METHODS: Adults with IBD and receiving biologics through home health or self-injection were individually randomized 1:1 to control (usual care) or intervention (remote monitoring), stratified by medication type (home infusion vs. injection). The 4-month intervention included
