Chylous pericardial effusion is a rarely reported complication of lung cancer surgery. Here, we report a case of an elderly man who suffered chylous pericardial effusion after radical right upper lung resection for cancer. The massive chylous effusion first occurred in the pericardium, drained to the right chest after the drainage of the hydropericardium and subsequently moved back to the pericardium again. Malignant tumors of the anterior mediastinum frequently appear locally advanced at the time of diagnosis, with invasion of the surrounding organs including major blood vessels, lungs, and pericardium. Surgery can be indicated either with diagnostic intent or for radical resection, usually as a part of a multimodality treatment. Extended operations with complex vascular reconstructions may be required for the complete removal of the mediastinal mass. In this article, surgical approaches and techniques of diagnostic and therapeutic procedures are reported and discussed.

Ectopic parathyroid glands are frequently located in the thymus and may become hyperplasic in patients with secondary hyperparathyroidism. When medical management fails, surgical removal is required, using either a traditional open sternotomy approach or video-assisted thoracic surgery (VATS). We were able to excise large, bilateral, mediastinal parathyroid glands using only left VATS. To the authors' knowledge, this is the first reported case of the use of unilateral thoracoscopic subtotal thymectomy for the excision of bilateral ectopic mediastinal parathyroid glands.

We report a case of multiple pulmonary hamartomas with a very unusual growth pattern. A 63-year-old woman underwent video-assisted thoracic surgery to investigate a round shadow seen on a routine chest radiograph. We found two tumors in the right S4, penetrating the visceral pleura and adhered to the right upper and lower lobes, respectively. Both tumors were removed and diagnosed histologically as pulmonary hamartoma. We hypothesized that these subpleural hamartomas might have grown invasively to penetrate the visceral pleura.

The authors report a case of american pulmonary histoplasmosis discovered by chance on a chest radiograph in a non immunocompromised patient, back from a stay in French Guyana. Confronted with the negativity of usual mycotic research, diagnosis has been made thanks to wedge excision by video-assisted thoracic surgery.
