Iatrogenic thyrotoxicosis. Causal circumstances, pathophysiology and principles of treatment. Review of the literature.

Thyrotoxicosis is the clinical syndrome that results when tissues are exposed to high levels of circulating thyroid hormones. In most instances, thyrotoxicosis is due to hyperthyroidism, a term reserved for disorders characterized by overproduction of thyroid hormones by the thyroid gland. Nevertheless, thyrotoxicosis may also result from a variety of conditions other than thyroid hyperfunction. The present report focuses on the etiologies, pathophysiology and treatment of iatrogenic thyrotoxicosis. Iatrogenic thyrotoxicosis may be caused by 1) subacute thyroiditis (a result of lymphocytic inf

Evaluation of the patient with a suspected thyroid disorder.

The principal types of thyroid disorders described herein can be recognized and evaluated clinically and biochemically by the obstetrician/gynecologist. Most patients with diffuse goiter should be referred for further evaluation and an RAI scan arranged while this consultation is awaited. Patients with solitary thyroid nodules should be referred for FNAB. Primary hypothyroidism without significant goiter can be treated without specialist referral, except in patients at high risk for ischemic coronary disease or arrhythmia. Primary hyperthyroidism, apart from subacute and postpartum thyroiditis

Hormonal hyperthermia: endocrinologic causes of fever.

Although hyperthermia is a component of many endocrine diseases, it is uncommon for fever to be the presenting manifestation of hormonal disorders. During a four year period we encountered six patients, hospitalized principally because of fever, who were found to have endocrine causes for the fever. In all, the admitting diagnosis was infection; three were suspected of having tuberculosis, two of gram-negative bacteremia and one of endocarditis. Except for asymptomatic bacteriuria in one patient (who remained febrile despite appropriate antibiotic therapy) infection was ruled out in all cases,

Craniopharyngioma as a Cause of Fever of Unknown Origin.

BACKGROUND: Fever of unknown origin is quite common in everyday clinical practice, and the approach is challenging. Prolonged fever as the sole manifestation of craniopharyngioma has been rarely reported in literature. OBJECTIVE: Herein, we report a case of adamantinomatous craniopharyngioma presented as fever of unknown origin in a 51-year-old woman, initially misdiagnosed as atypical subacute thyroiditis. CASE PRESENTATION: During the work up, the patient complained about bitemporal hemianopsia. Thus, she underwent a pituitary Magnetic Resonance Imaging, which revealed a mixed mass origina

99Tc(m)-MIBI scanning of the thyroid gland in patients with markedly decreased pertechnetate uptake.

Diffusely reduced 99Tc(m)-pertechnetate uptake is a relatively infrequent but annoying finding that impairs evaluation of the thyroid gland. We studied 32 female patients aged 19-85 years with markedly reduced pertechnetate uptake. The following causes of reduced pertechnetate uptake were recognized: treatment with iodinated pharmaceuticals (n = 15), suppression therapy with T4 (n = 11), subacute thyroiditis (n = 5) and massive tumour replacement (n = 1). 99Tc(m)-MIBI thyroid scintigraphy was performed within 24 h of the pertechnetate study. The results were correlated with neck ultrasound, se

Thyroid dysfunction in COVID-19.

The COVID-19 pandemic has affected over 772 million people globally. While lung damage is the major contributor to the morbidity and mortality of this disease, the involvement of multiple organs, including the endocrine glands, has been reported. This Review aims to provide an updated summary of evidence regarding COVID-19 and thyroid dysfunction, incorporating highlights of recent advances in the field, particularly in relation to long COVID and COVID-19 vaccination. Since subacute thyroiditis following COVID-19 was first reported in May 2020, thyroid dysfunction associated with COVID-19 has

Diagnostic imaging in thyrotoxicosis.

In thyrotoxicosis, imaging mainly scintigraphy, color Doppler sonography and radioiodine uptake test are used in the differential diagnosis as well as in the morphofunctional evaluation of the thyroid before and after therapy (mainly pharmacological or with radioiodine). Radioiodine uptake test differentiates high uptake thyrotoxicosis (Graves'disease, toxic nodular goiter) and low uptake thyrotoxycosis (subacute or silent thyroiditis, ectopic thyrotoxicosis, iodine-induced hyperthyroidism). In Graves'disease scintigraphy shows thyroid enlargement with intense homogeneous tracer uptake; rarely

Evaluation of a new rapid and fully automated electrochemiluminescence immunoassay for thyrotropin receptor autoantibodies.

BACKGROUND: Hyperthyroidism in Graves' disease is caused by autoantibodies to the TSH receptor (TSHR), and measurement of the TSHR autoantibody (TRAb) yields important information to diagnose and decide on the course of treatment of Graves' disease. We evaluated basic and clinical performance of a new, rapid, and fully automated electrochemiluminescence immunoassay Elecsys Anti-TSHR (Elecsys TRAb) for measuring serum TRAb. METHODS: For evaluation of basic performance of the assay, we carried out intra- and interassay precision studies using five serum pools and three serum pools, respectively

The many causes of subclinical hyperthyroidism.

Subclinical hyperthyroidism, defined as serum thyroid hormone levels in the reference range with low serum TSH concentration, is a well recognized clinical entity, but little information is available concerning the prevalence of the disorders that produce it. We conducted a 24-month retrospective survey of subclinical hyperthyroidism patients referred to a university hospital nuclear medicine service for diagnostic studies. Twenty-four consecutive patients were evaluated (22 outpatients and 2 inpatients). All patients had highly sensitive TSH determination, thyroid hormone levels, radioiodine

[Metabolic evaluation and etiology of "preclinical hypothyroidism": a preliminary report].

The TRH test, using synthetic TRH (TSH-releasing hormone) is the most sensitive test for the assessment of thyroid function. It may show elevated basal TSH and/or an exaggerated TSH response to TRH, despite normal thyroid hormone levels (T4, FT4I, T3). This condition is termed "preclinical hypothyroidism" (pc hypo). Thyroid hormone levels, the clinical index of Billewicz and metabolic impact on target tissues were studied prospectively in 38 pc hypo women and compared with 20 controls matched for age, weight and sex and 9 patients with overt hypothyroidism. For metabolic evaluation at the tiss

Rare Presentation of Thyrotoxicosis Defying Methimazole Treatment: A Case Series.

Thyrotoxicosis, also known as hyperthyroidism, is a condition characterized by the excessive production of thyroid hormones by the thyroid gland. Besides Graves' disease, other common causes of thyrotoxicosis include toxic multinodular goiter, toxic adenoma, and subacute thyroiditis. The treatment of thyrotoxicosis depends on the underlying cause and may include medications (e.g., antithyroid drugs, beta-blockers), radioactive iodine therapy, or surgical removal of the thyroid gland (thyroidectomy). In this report, we present two instances of thyrotoxicosis where conventional high doses of ant

Review of oral cholecystographic agents for the management of hyperthyroidism.

OBJECTIVE: Although the use of oral cholecystographic agents (OCAs) had declined due to limited availability, there is literature to suggest it is an effective medication for thyrotoxicosis in appropriate clinical situations. METHODS: The authors performed a PubMed search and systematically reviewed all the English written case reports, original studies and reviews from 1953 to 2012. Additional information was supplemented from available online pharmacologic databases. RESULTS: The off-label use of OCAs was reviewed for the management of neonatal and adult Graves' disease, subacute thyroidit

Thyrotoxicosis with post-treatment hypothyroidism in a patient with acute suppurative thyroiditis due to porphyromonas.

BACKGROUND: Acute suppurative thyroiditis (AST) is a rare, life-threatening thyroid infection characterized by a tender neck mass and fever. As these features are shared with self-limited subacute thyroiditis (SAT), it is important to differentiate between the two disorders. PATIENT FINDINGS: We report a case of AST in a 21-year-old woman who presented with steadily worsening throat pain for 3 weeks, a tender left neck mass, and thyrotoxicosis. She was initially given prednisone for treatment of presumed SAT but then it acutely worsened. Fine needle aspiration yielded pus on gross examination

Characterization of high titer antithyroglobulin antibodies.

Few autoantibodies directed against thyroglobulin (TgAbs) have been fully characterized in man. The present study was designed to characterize TgAbs from patients with unusually high titers (greater than 1:5(12] using tanned red cell hemagglutination technique (TRC). IgG was isolated from the sera of subjects with Hashimoto's thyroiditis (n = 4), subacute thyroiditis (n = 1) and Graves' disease (n = 1) using DEAE-Sephacel chromatography. Isolated TgAbs were substituted as first antibody in a double antibody thyroglobulin (Tg) RIA and the Ka's were determined by Scatchard analysis. Molecular ra

Qualitative and quantitative CEUS assessment for differential diagnosis in thyrotoxicosis: destruction vs. hyperfunction.

PURPOSE: Thyrotoxicosis results from either thyroid hyperfunction, as seen in Graves' disease, or tissue destruction, as in subacute thyroiditis (SAT). Differential diagnosis is essential but can be challenging, with thyroid scintigraphy traditionally used to distinguish between these types. This study investigates the potential of Contrast-Enhanced Ultrasound (CEUS) to provide a non-invasive alternative for differentiating hyperfunction and destructive thyroid disease. METHODS: In a prospective study, 42 patients with untreated thyrotoxicosis were evaluated using CEUS to analyze both qualita
