Three-dimensional evaluation of vitreomacular traction and epiretinal membrane using spectral-domain optical coherence tomography.

PURPOSE: To delineate the 3-dimensional (3-D) relationship in vitreomacular traction (VMT) and idiopathic epiretinal membrane (ERM). DESIGN: Observational case series. METHODS: Forty-eight evaluable eyes of 35 patients with VMT or idiopathic ERM were investigated with spectral-domain (SD) optical coherence tomography (OCT). VMT was defined as focal if the diameter of the vitreous attachment was 1500 microm or less and broad if it was more than 1500 microm. The 3-D OCT representation of vitreomacular interface abnormalities was evaluated. RESULTS: Focal VMT was seen in five eyes. Broad VMT w…

Unreamed intramedullary nailing of acute femoral shaft fractures using a traction device without fracture table.

The authors present a simple distraction apparatus that makes it possible to perform nonreamed intramedullary nailing of the femur on a normal operating table. Nonreamed intramedullary (IM) nails were inserted in 24 femoral shaft fractures using this device. The setup time of this device was 60% shorter compared with a stratified group of 28 femoral nailings performed using the fracture table. The authors feel that this device is a safe and effective alternative to using a fracture table. It is especially useful in the polytrauma patient where the additional lesions can be treated without the …

Excessive papillary muscle traction and dilated mitral annulus in mitral valve prolapse without mitral regurgitation.

This study demonstrated excessive papillary muscle displacement during peak systole but normal mitral annulus function during the cardiac cycle in patients with mitral valve prolapse and no mitral regurgitation. The excessive papillary muscle displacement may play an important role in the pathogenesis of the superior displacement of mitral leaflets in patients with mitral valve prolapse.

Dynamic traction after extensor tendon repair in zones 6, 7, and 8: a retrospective study.

Review of the notes of 46 extensor tendon repairs in 21 patients treated by post-operative dynamic traction without an M.P. flexion block, no tendon ruptures or extensor lag and only one digit without full flexion after a mean follow-up of seven weeks. Re-examination of 26 treated repairs in nine patients for this study demonstrated a mean T.A.M. of 259 degrees at an average 14 months follow-up. No bow-stringing occurred because the extensor retinaculum was not excised, and no tenolyses were necessary.

Development of traction retinal detachments following intravitreal injections of retinal Muller and pigment epithelial cells.

We injected varying numbers of retinal Muller glia into the rabbit vitreous in an established model of traction retinal detachment. We used indirect ophthalmoscopy to observe the changes elicited during the following 1 month. Although the severity of the tractional changes increased with increasing numbers of the glial cells, the pathology produced stabilized within the 1st week of injury. Muller glia were less effective at eliciting retinal detachments than retinal pigment epithelial cells (RPE) or mixtures of glia and RPE. Intravitreal tissue membranes derived from the glia differed morpholo…

Traction method with pulley maneuver in minimally invasive hepatobiliary pancreatic surgery: how I do it differently?

BACKGROUND: Achieving optimal traction and exposure in minimally invasive hepatobiliary and pancreatic surgery remains a significant challenge, often necessitating additional ports or assistants. This article describes novel traction techniques utilizing the Stratafix® symmetric barbed suture, repurposed from its original wound closure design, to enhance surgical efficiency and control. METHODS: We detail the "Pulley maneuver," a versatile approach for various hepatobiliary and pancreatic procedures. Key modifications include a "Sling pulley maneuver with Endo-loop" for pancreatic elevation d…

Efficacy of an internal traction device in colorectal endoscopic submucosal dissection: a multicenter randomized controlled trial.

BACKGROUND AND AIM: Shortening the procedure time and reducing the incidence of complications are important in patients undergoing colorectal endoscopic submucosal dissection (ESD). This prospective randomized controlled trial assessed the efficacy and safety of colorectal ESD with an internal traction-assisted device (IT-ESD) by comparing its outcomes with those of conventional ESD (without internal traction-assisted device) in patients with colorectal epithelial neoplasia. METHODS: A total of 201 patients who underwent ESD between March 2020 and April 2023 were prospectively randomized to I…

TRACTION-INDUCED STRUCTURAL FAILURE STATES IN MACULAR HOLE FORMATION REVEALED BY VOLUME-RENDERED SWEPT SOURCE OCT.

PURPOSE: To analyze macular hole development using three-dimensional visualization of vitreomacular architecture volume-rendered optical coh … ture during macular hole development. The observed anatomical configurations are best understood as traction-induced structural failure states of the central macula rather than as a linear temporal staging sequence. This framework better accounts for the diversity, asymmetry, and nonuniform evolution of macular hole morphology observed in vivo.

Life-Threatening Upper Gastrointestinal Bleeding following Percutaneous Endoscopic Gastrostomy Tube Removal under Traction: A Case Report.

INTRODUCTION: Percutaneous endoscopic gastrostomy (PEG) tubes are commonly used to facilitate long-term enteral nutrition. Selected PEG tubes may be appropriately removed without endoscopic guidance via gentle traction, which is seldom associated with major bleeding sequelae. Rare reports of major upper gastrointestinal (GI) hemorrhage following PEG tube removal have been managed using Foley catheter balloon tamponade or endoscopic over-the-scope clipping. Prompt reinsertion of a PEG tube may also be considered. CASE PRESENTATION: We present the case of a multi-morbid 73-year-old female who e…

Penile Traction Therapy for Peyronie's Disease: A Contemporary Narrative Review of Clinical Evidence and Evolving Trends.

Peyronie's disease (PD) is a connective tissue disorder of the penile tunica albuginea that leads to curvature, deformity, and erectile dysfunction, resulting in significant psychosocial distress. Surgical interventions remain effective but are often reserved for severe deformities. Over the past decade, penile traction therapy (PTT) has gained recognition as a non-invasive alternative based on the principle of mechanotransduction, offering the potential to remodel fibrotic tissue, reduce curvature, and restore length. A structured narrative review was performed using PubMed, ScienceDirect, an…

High frequency traction rod model and its structure-borne sound transmission characteristic of high-speed trains.

With respect to high-speed trains, a significant proportion of interior noise is caused by high-frequency vibrations, which are generated at the wheel-rail contact and transmitted through the secondary suspension system in the bogie, to the carbody. Among suspension components, the effect of the traction rod is always underestimated. To research this structure-borne sound transmission, a structure-borne sound model of the traction rod was established, and introduced into a 3D train/track coupled dynamic model. This is suitable to research dynamic behavior at high frequencies, since it consider…
