Trends in hospital admissions during transition from paediatric to adult services for young people with learning disabilities or autism: Population-based cohort study.

BACKGROUND: Transition from paediatric to adult health care may disrupt continuity of care, and result in unmet health needs. We describe changes in planned and unplanned hospital admission rates before, during and after transition for young people with learning disability (LD), or autism spectrum disorders (ASD) indicated in hospital records, who are likely to have more complex health needs. METHODS: We developed two mutually exclusive cohorts of young people with LD, and with ASD without LD, born between 1990 and 2001 in England using national hospital admission data. We determined the annu

Hemodynamic consequences of respiratory interventions in preterm infants.

Advances in perinatal management have led to improvements in survival rates for premature infants. It is known that the transitional period soon after birth, and the subsequent weeks, remain periods of rapid circulatory changes. Preterm infants, especially those born at the limits of viability, are susceptible to hemodynamic effects of routine respiratory care practices. In particular, the immature myocardium and cardiovascular system is developmentally vulnerable. Standard of care (but essential) respiratory interventions, administered as part of neonatal care, may negatively impact heart fun

Development of a Locator Tool and Protocol to Support People Living With HIV in Baltimore: A Quality Improvement Project.

Retaining people with HIV (PWH) in medical care is crucial to ending HIV as an epidemic. Many PWH in Baltimore also experience high rates of substance use, mental health disorders, and homelessness, making it difficult to locate and engage them in care. In our hospital-to-community transitional care program for PWH, 50% of clients were lost to follow-up. To increase the percentage of engaged patients, we developed a locator tool and protocol detailing patient's social and personal information, to be used when clients could not otherwise be located. We obtained completed locator tools on 85% of…

Transition Interventions for Adolescents on Antiretroviral Therapy on Transfer from Pediatric to Adult Healthcare: A Systematic Review.

Globally, adolescents living with HIV (ALHIV) experience poor health outcomes such as low retention in care, ART non-adherence and viral non-suppression. These outcomes coincide with the period during and after their transition from pediatric to adult healthcare. This study aimed to systematically describe the compendium of transition interventions and synthesize the effects of such transition interventions on adherence to ART, retention in care and viral load suppression. Seven databases and Google Scholar were searched and the review findings were reported according to the Preferred Reportin

Transitions for youth and young adults with eating disorders and/or other mental health conditions: a Canadian guideline.

BACKGROUND: Eating disorders (EDs) are severe mental illnesses with high rates of mortality, morbidity, and reduced quality of life. Their onset occurs during adolescence and early adulthood, coinciding with the critical transition from pediatric to adult care. To address the lack of guidelines to support ED transitions in Canada, this study developed evidence-based guideline recommendations. METHODS: Scoping review methodology was employed using comprehensive searches across seven databases, supplemented by forward and backward citation chaining to identify records on youth and young adults

Care of adults with congenital heart disease--a challenge for the new millennium.

The care of complex congenital heart defect (CHD) patients should be continuous through life. Medium- and high-risk patients should be seen in special facilities for grown-up congenital heart (GUCH) patients, and followed for life. CHD in the adult is different than CHD in the child. Transitional programs should be available to prepare the adolescent patient to take charge of his/her own health. The patient should be transferred smoothly from pediatric to adult care. GUCH care should be regionally anchored in special facilities (groups or sometimes individuals) willing to make a commitment to

Predictors of linkage to HIV care and viral suppression after release from jails and prisons: a retrospective cohort study.

BACKGROUND: Incarceration provides an opportunity for engagement in HIV care but is associated with poor HIV treatment outcomes after release. We aimed to assess post-release linkage to HIV care (LTC) and the effect of transitional case management services. METHODS: To create a retrospective cohort of all adults with HIV released from jails and prisons in Connecticut, USA (2007-14), we linked administrative custody and pharmacy databases with mandatory HIV/AIDS surveillance monitoring and case management data. We examined time to LTC (defined as first viral load measurement after release) and

Development and Temporal Validation of a Discharge-Based Administrative Data Risk Model for 30-Day Unplanned Readmission After Endometrial Cancer Staging Surgery.

Background: Postoperative complications after staging surgery for endometrial cancer often occur after discharge, and 30-day unplanned readmission is an important post-discharge outcome. Existing models lack same-day usability at discharge and sufficient validation, limiting discharge-time risk stratification and transitional care planning. This study aimed to develop and validate a discharge-time risk prediction model based on administrative data for 30-day unplanned hospital readmission among patients undergoing staging surgery for endometrial cancer. Methods: This retrospective cohort study…

Operating a Street Medicine Program within New York City's Public Health Care System.

New York City has the largest homeless population of any city in the United States, with thousands of people experiencing unsheltered homelessness on its streets. These individuals have poor health outcomes and limited connection to care. To better meet their health and social needs, New York City Health+Hospitals, the largest public hospital system in the United States, has implemented the Street Health Outreach and Wellness (SHOW) initiative to bring unsheltered New Yorkers health care, material goods, and connections to transitional housing. This article documents the evolution of the progr

The COCCOS study: A protocol for a multicentric quasi-experimental study of a healthcare transition program for young people with chronic conditions.

INTRODUCTION: The growing number of adolescents and young adults (AYAs) living with chronic conditions highlights the need for effective hea … linical practice. By integrating clinical, economic, and process outcomes, it addresses key gaps in transitional care research. Findings will inform refinement of the programme and the design of a future large-scale trial, contributing to the development of sustainable, patient-centred transition care for AYAs with chronic conditions.

Transitioning Young Adult Patients from Pediatric to Adult Rheumatology Practices in Rhode Island.

BACKGROUND: Young adults with rheumatic conditions encounter challenges when transitioning to an adult rheumatologist. At the Hasbro Pediatric Rheumatology Clinic, patients aged 18 and older were historically provided a list of adult rheumatologists; however, no formal transition process existed. We assessed the feasibility and acceptability of a pilot model to systematically transition pediatric patients to an adult rheumatologist using aspects of the Six Core Elements of Health Care Transition (HCT) from Got Transition®. METHODS: A multidisciplinary team facilitated the program. Eligibility

Healthcare transition from childhood to adulthood in Tuberous Sclerosis Complex.

Healthcare transition from childhood to adulthood is required to ensure continuity of care of an increasing number of individuals with chronic conditions surviving into adulthood. The transition for patients with tuberous sclerosis complex (TSC) is complicated by the multisystemic nature of this condition, age-dependent manifestations, and high clinical variability and by the presence of intellectual disability in at least half of the individuals. In this article, we address the medical needs regarding each TSC-related manifestation in adulthood, and the services and support required. We revie

The Importance of Implementing a Transition Strategy for Patients with Muscular Dystrophy: From Child to Adult-Insights from a Tertiary Centre for Rare Neurological Diseases.

Progress in the field of muscular dystrophy (MD) using a multidisciplinary approach based on international standards of care has led to a significant increase in the life expectancy of patients. The challenge of transitioning from pediatric to adult healthcare has been acknowledged for over a decade, yet it continues to be a last-minute concern. Currently, there is no established consensus on how to evaluate the effectiveness of the transition process. Our study aimed to identify how well patients are prepared for the transition and to determine their needs. We conducted a descriptive, cross-s
