The pattern of infection of the nervous system in Riyadh: a review of 121 cases.

Analysis of 121 consecutive cases with infection of the nervous system showed that the majority were the granulomatous infections, tuberculosis and brucellosis (53 cases (44 per cent)). Thirty-nine patients had tuberculosis and 14 had brucellosis. The clinical and microbiological pattern of infection differs from that frequently reported from Western countries. Tuberculosis lesions presented with features of intracranial space occupying lesions (14), spinal cord compression (13) and lumbosacral root compression (1 child). Ten adults and one child had tuberculous meningitis. Pyogenic meningitis

Tuberculosis of the spine.

Pott's spine, commonly known as spinal tuberculosis (TB), is an extrapulmonary form of TB caused by Mycobacterium TB. Pott's paraplegia occurs when the spine is involved. Spinal TB is usually caused by the hematogenous spread of infection from a central focus, which can be in the lungs or another location. Spinal TB is distinguished by intervertebral disc involvement caused by the same segmental arterial supply, which can result in severe morbidity even after years of approved therapy. Neurological impairments and spine deformities are caused by progressive damage to the anterior vertebral bod

Andersson Lesion Occurring in the Lumbosacral Segment of a Young Man: A Case Report and Literature Review.

BACKGROUND: Andersson lesion (AL) is not a rare complication of ankylosing spondylitis (AS), but it seldom occurs in single lumbosacral segments or young people. We present a case of a young patient with AS and AL between L5 and S1. CASE DESCRIPTION: A 30-year-old man with a history of AS presented with AL between L5 and S1. Surgery performed under O-arm navigation was successful. At 6-month follow-up, the patient's pain was reduced, and his function had improved. CONCLUSIONS: AL rarely occurs in the lumbosacral region or in young people and can be misdiagnosed as spinal tuberculosis. Surgic

A review of the neurological and neurosurgical implications of tuberculosis in children.

Tuberculous involvement of the central nervous system (CNS) and vertebral column is the most lethal and disabling form of tuberculosis (TB). Several factors contribute to poor outcome, including cerebrovascular involvement with ischemia, hydrocephalus, direct parenchymal injury and formation of abscess and inflammation in the brain and spinal cord, hyponatremia, seizures, and delayed diagnosis. Spinal spondylitis from TB and associated spinal deformity is the leading cause of paraplegia in developing countries. The evidence for supportive treatment of TB infection of the CNS is limited, leadin

Intramedullary spinal tuberculoma--case report.

A healthy 33-year-old man presented with an intramedullary tuberculoma of the thoracic spinal cord manifesting as a 2-month history of progressive paraparesis and sphincter dysfunction. Magnetic resonance imaging showed ring enhancement of the intramedullary thoracic lesion with perifocal edema. General physical examination was unremarkable with no signs of inflammation except for a positive finding by the tuberculin skin test. Total resection of the intramedullary mass was performed through a posterior myelotomy following T11-12 laminectomy. Histological examination revealed a granulomatous l

Near-miss Thoracic Spine Solitary Plasmacytoma with Neurological Deficit during Pregnancy: A Case Report.

Solitary plasmacytoma (SPC) account for only 5% of plasma cell neoplasms, and the literature hardly reports spinal SPC with a neurological deficit. Furthermore, spinal surgical intervention during pregnancy is rarely encountered and often requires multidisciplinary collaboration and management. The objective of this case report is to highlight this near-miss diagnosis and spinal surgical intervention during pregnancy. A 31-year-old woman with 24 weeks gestation presented with sudden paralysis and incontinence, with an underlying history of chronic backpain over a two-month period. Initially, s

Tuberculosis menace: a case report of disseminated tuberculosis presenting as tubercular meningitis and long-segment cervical tuberculous myelitis in a 32-year-old man from rural India.

This case report discusses an uncommon presentation of miliary tuberculosis as tubercular meningitis (TBM) and long-segment cervical tuberculous myelitis in a 32-year-old man from rural India. The patient presented with symptoms of fever, headache, neck stiffness, and gradual weakness in all four limbs. Hydrocephalic changes secondary to meningitis and involvement of the spinal cord were observed on neuroimaging and were correlated with clinical findings of cervical myelitis, confirming the diagnosis of TBM with cervical myelitis. TBM, together with cervical myelitis in patients with miliary t

Case report 654: Disseminated tuberculosis.

A case of disseminated tuberculosis with a psoas abscess and associated destruction of bone in a 28-year-old man has been presented. Multiple skeletal lesions and hilar adenopathy were observed. The initial diagnosis from pathological specimens was equivocal, since acid-fast bacilli were not demonstrated. However, special stains of biopsy specimens subsequently confirmed the diagnosis. The initial radiographs suggested disseminated malignancy. Good therapeutic results were obtained, utilizing surgical and chemotherapeutic methods. This case demonstrates the ability of disseminated tuberculosis

Experimental allergic encephalomyelitis raises betaine levels in the spinal cord of strain 13 guinea-pigs.

Chronic relapsing experimental allergic encephalomyelitis, an animal model of multiple sclerosis, was induced in Strain 13 guinea-pigs by subcutaneous injection of spinal cord homogenate and Freund's incomplete adjuvant supplemented with Mycobacterium tuberculosis. High resolution 1H NMR spectra of CNS tissue extracts indicated that the levels of choline metabolites, particularly betaine, were elevated in the spinal cord tissue, the principal site of lesion formation in this guinea-pig strain. The spectra also show that N-acetylated compounds are slightly depleted in the disease. The results a

Tuberculosis of the spine: a review.

Spinal tuberculosis remains common in the "Third World" and is commoner in Britain and "developed" countries than many other disorders that receive much more attention. Though little new has been added to knowledge of its morbid anatomy, the observation that it is associated with a low bacterial population is emphasised. Controlled clinical trials in Korea, Hong Kong and Bulawayo have demonstrated the excellent results of outpatient ambulant chemotherapy, which are unimproved by other additional measures with the one exception of the skilled performance of radical resection of the focus with a

Senile thoracolumbar tuberculosis with kyphosis and severe osteoporosis treated with polymethylmethacrylate-augmented screw fixation.

BACKGROUND: Elderly osteoporotic patients with kyphosis secondary to spinal tuberculosis (TB) are at high risk for implant failure. For these patients, polymethylmethacrylate (PMMA)-augmented pedicle screw fixation provides immediate stability but increases the risk of infection. Here, we evaluated the outcome for a 78-year-old osteoporotic female with TB-related kyphosis successfully managed with PMMA-augmented pedicle screw fusion. CASE DESCRIPTION: A 78-year-old female presented with mid-back pain, progressive kyphotic deformity, and increasing paraparesis. The magnetic resonance imaging r

Spondylodiscitis Presenting to a Chiropractor: A Case Report and Literature Review.

An 80-year-old man under combination therapy for pulmonary tuberculosis presented to a chiropractor with a one-month history of worsening chronic low back pain, yet denied having any respiratory symptoms, weight loss, or night sweats. Two weeks prior, he saw an orthopedist who ordered lumbar radiographs and magnetic resonance imaging (MRI), showing degenerative changes and subtle findings of spondylodiscitis, but was treated conservatively with a nonsteroidal anti-inflammatory drug. The patient was afebrile, yet considering his older age and worsening symptoms, the chiropractor ordered a repea

Anterior lumbar fusion in Pott's disease.

Although most lumbar lesions associated with spinal tuberculosis can be satisfactorily resolved with nonoperative treatment, there are cases in which surgery is indicated. Between July 1982 and December 2003 we retrospectively reviewed 51 patients who were operated by anterior débridement and fusion. Twelve patients were lost to followup and one died due to inferior vena cava tear. Followup of 38 patients ranged from 2 to 10 years (average 4 years). There were 22 male and 16 female patients, average age 35 years (range, 8-65 years); upper lumbar area (L1-L3) was involved in 17 cases, lower lum

Unmasking the mimic: vertebral alveolar echinococcosis diagnosed by metagenomic next-generation sequencing.

A Siberian woman in her forties presented to a public hospital in northeastern Germany with chronic back pain and a paravertebral mass, initially misdiagnosed as spinal tuberculosis. Repeated biopsies and metagenomic next-generation sequencing (mNGS) ultimately confirmed vertebral alveolar echinococcosis. Haplotype analysis revealed a novel Asian-cluster variant, supporting the presumed origin of infection.

Spinal tuberculoma in a patient with spinal myxopapillary ependymoma.

Intramedullary spinal tuberculosis is a clinical curiosity. A 19-year-old female was diagnosed and treated for lumbosacral myxopapllary ependy moma (MPE). Three years later, she presented with back pain and hypoesthesia of the left upper limb. Besides revealing local recurrence, the MRI demonstrated a fresh lesion in the cervicomedullary area. The latter was operated and the histopathology revealed a tuberculoma.

Fatty infiltration of osseous structures: a long-term complication of oleothorax--case report.

Thoracic imaging of a patient treated for pulmonary tuberculosis with oleothorax therapy before the antibiotic era demonstrated a rare complication. Gross invasion by lipid with subsequent pathologic fracture of the adjacent thoracic vertebra may give rise to symptomatic spinal cord compression. Magnetic resonance imaging is a useful modality for help in diagnosing treatment complications of oleothorax.
