Upper urinary tract transitional cell carcinoma in patients with bladder carcinoma.

An upper urinary tract tumor following the initial diagnosis of bladder carcinoma is rare. We report 3 patients with bladder carcinoma who later developed upper urinary tract tumors. Ureteral tumor was diagnosed by ureterorenoscope in the first 2 patients, while in the third case, urothelial tumor involving the entire left upper tract was found at the time of planned operation for cystectomy. The etiology, risk factors, and investigation plans were reviewed, in order to make an early detection of tumors.

Prostatic-type epithelium in urinary bladder. Clinical, histologic, and immunohistochemical study.

Six cases of urinary bladder mucosa with prostate-type epithelium were studied clinically, morphologically, and immunohistochemically. All patients were male with an average age of fifty-three years; most presented with painless hematuria. Histologically, two types of lesions were observed, the polypoid located in various sites of the bladder wall and the flat lesion found in the bladder neck. Both lesions shared in common a prostatic-type and transitional surface epithelium while prostatic-type glands were prominent in the polypoid lesion. The prostatic-type epithelium was confirmed immunohis…

Bladder neck electrical conductivity in female urinary urgency and urge incontinence.

Thirty-nine consecutive women, referred because of lower urinary tract symptoms of stress incontinence, urgency and urge incontinence, were interviewed and their symptoms were graded using a standardized questionnaire. They were investigated with conventional, fast-fill cystometry and with a new bladder neck electrical conductivity (BNEC) test which measures bladder neck activity. A highly significant correlation between the symptoms of urgency and urge incontinence and the maximum amplitude of bladder neck activity was found with the new test. The correlation of the maximum deflection at rest

Suprapubic versus transurethral bladder drainage after surgery for stress urinary incontinence.

Fifty-one patients with clinical and urodynamic diagnoses of stress urinary incontinence were randomly allocated to either suprapubic (N = 24) or transurethral (N = 27) bladder drainage after vaginal surgery for stress incontinence (revised Pereyra procedure). Postoperative use of suprapubic bladder drainage significantly reduced febrile morbidity (calculated as fever index; P less than .01) and length of hospitalization (P less than .05). Postoperative normal bladder functions resumed more quickly when suprapubic drainage was used (P less than .05), so that most patients did not need bladder

Upper thoracic sympathetic neuron responses to input from urinary bladder afferents.

The aim of the present study was to evaluate the organization of neural circuitry responsible for the intersegmental transmission of input from urinary bladder afferents to sympathetic preganglionic neurons (SPNs). The electrical activity of SPNs was recorded from axons of the cervical sympathetic trunk in anesthetized central nervous system (CNS)-intact and in unanesthetized midcollicular-decerebrate or acute C1 spinal cats. In all three preparations, tonically active SPNs were excited or inhibited by 1) electrical stimulation of myelinated afferents of the pelvic or hypogastric nerve, both o…

Urinary bladder necrosis following selective embolization of the internal iliac artery.

A case of massive hemorrhage from pelvic fractures was controlled by embolization of the internal iliac artery with autologous blood clots. Later an infarct of the bladder wall developed, probably because of embolic material in the capillary system preventing adequate collateral supply. It is concluded that collateral flow should be considered before embolization and that the technique of embolization must be adapted to the individual case.

Bladder recurrence of primary upper tract urinary carcinoma following nephroureterectomy, and risk of upper urinary tract recurrence after ureteral stent positioning in patients with primary bladder cancer.

Upper tract urinary carcinoma (UTUC) and bladder cancer (BCa) share similar biological, histological and pathological characteristics. These resemblances could explain the high rate of synchronous and metachronous tumors affecting both upper urinary tract and bladder. In patients affected by primary UTUC bladder recurrence is quite common and it represents one of the major concerns during the follow-up of patients who underwent radical nephroureterectomy. Conversely, UTUC recurrence after primary non-muscular invasive BCa is a relatively rare event. Moreover, there is no clear evidence on whet

Bladder neck position differences on CT in patients with incidental asymptomatic urinary bladder gas: A comparative retrospective study.

OBJECTIVE: To investigate the potential link between differences in bladder neck position and the occurrence of incidental asymptomatic urinary bladder gas on computed tomography (CT) scans in female patients. METHODS: CT scans reports from January 2012 to December 2023 were analyzed in this retrospective study to identify adults presenting with bladder gas. Exclusion was based on factors such as prior urological procedures, urinary tract infections (UTIs), and other potential contributors to bladder gas. The distance between the bladder neck and the pubococcygeal line (B-PCL) was measured to…

A comparison of bladder neck preservation and bladder neck reconstruction for urinary incontinence after radical retro pubic prostatectomy.

BACKGROUND: Prostate cancer is the sixth most common cancer worldwide and will include about 30% of all malignancies in men. Since the initial report of the anatomic radical prostatectomy, refinements in the surgical technique have been made. Several studies show that bladder neck preservation (BNP) during radical prostatectomy makes improve early return of urinary continence, erectile function or both. However, some clinical trials have suggested little difference between the return of continence while using modifications. In this study, we compared outcomes of BNP and bladder neck reconstruc

Genome-wide expression profiling of urinary bladder implicates desmosomal and cytoskeletal dysregulation in the bladder exstrophy-epispadias complex.

The bladder exstrophy-epispadias complex (BEEC) represents a spectrum of urological abnormalities where part, or all, of the distal urinary tract fails to close during development, becoming exposed on the outer abdominal wall. While the etiology of BEEC remains unknown, strong evidence exists that genetic factors are implicated. To understand the pathways regulating embryonic bladder development and to identify high-probability BEEC candidate genes, we conducted a genome-wide expression profiling (GWEP) study using normal and exstrophic human urinary bladders and human and mouse embryologic bl…

Chloride-dependent mechanisms contribute to urinary bladder smooth muscle contractility: Pharmacological and molecular evidence from rat bladder tissue.

OBJECTIVES: To investigate the functional role of chloride-dependent signaling pathways in urinary bladder smooth muscle (SM) contractility and to evaluate the expression of ClC-3 and CLCA4 in rat bladder tissue. MATERIALS AND METHODS: Expression of ClC-3 and CLCA4 in rat bladder tissue was examined using Western blotting and immunohistochemistry. Functional studies were conducted using isolated bladder SM strips (2 mm × 2 mm × 10 mm) from adult male Wistar rats mounted in organ bath chambers for isometric tension recording. Potassium chloride (KCl)-induced contractions were assessed under va…

A constrained mixture-micturition-growth (CMMG) model of the urinary bladder: Application to partial bladder outlet obstruction (BOO).

We present a constrained mixture-micturition-growth (CMMG) model for the bladder. It simulates bladder mechanics, voiding function (micturition) and tissue adaptations in response to altered biomechanical conditions. The CMMG model is calibrated with both in vivo and in vitro data from healthy male rat urinary bladders (cystometry, bioimaging of wall structure, mechanical testing) and applied to simulate the growth and remodeling (G&R) response to partial bladder outlet obstruction (BOO). The bladder wall is represented as a multi-layered, anisotropic, nonlinear constrained mixture. A short ti…

Intra-bladder wall transplantation of bone marrow mesenchymal stem cells improved urinary bladder dysfunction following spinal cord injury.

BACKGROUND: In the most of previous experiments, intrathecal administration of stem cells (SCs) was seen in the management of neurogenic bladder (NGB) following contusion or complete transaction in the rodent model of spinal cord injury (SCI). Here, we aimed to investigate whether intra bladder wall autologous bone marrow mesenchymal SC (BM-MSCs) transplantation, as a minimally invasive method, could improve bladder dysfunctions after a chronic phase of hemi- and complete-transection SCI in a female rat model. MATERIAL AND METHODS: A total of forty-two female Wistar rats were randomly divided

The impact of bladder neck mucosal eversion during open radical prostatectomy on bladder neck stricture and urinary extravasation.

PURPOSE: To determine whether the bladder neck mucosal eversion (BNM-eversion) during radical retropubic prostatectomy (RRP) reduces the risk of bladder neck stricture (BNS) and of peri-anastomotic extravasation (PAE) in postoperative cystography. METHODS: Two hundred and eleven patients with clinically localized prostate cancer underwent RRP and were prospectively randomized into patients with BNM-eversion (group I) and without BNM-eversion (group II). All patients underwent an evaluation of PAE by retrograde cystography on postoperative day 8. We assessed BNS after 6 months. RESULTS: Ninet
