Hydronephrosis in pregnancy is common in the second and third trimester. Only a few cases are symptomatic, caused by a ureteric stone or by the pregnancy itself. The clinical dilemma is when to treat and when not to treat. We propose a multidisciplinary management based on renal ultrasonography to verify hydronephrosis and renography to diagnose obstructive hydronephrosis. Obstruction with a high intra-renal pressure must be treated to avoid kidney dysfunction. Patients with pyonephrosis need immediate treatment.

Four young patients, including a 7-year-old girl with Aicardi syndrome, an 11-year-old boy with Lennox-Gastaut syndrome, a 22-year-old man with epilepsy due to childhood encephalitis, and a 17-year-old girl with Rett syndrome, were scheduled to undergo extracorporeal shock wave lithotripsy (ESWL) for urolithiasis. BACKGROUND: The urinary tract calculi in children is a very unfrequent problem in the pediatric age group. Endourology plays a major role in pediatric surgery. We reported our experience in endoscopic management of pediatric calculi.
METHODS: Eighteen children (age range: 2 year-19 years) underwent endourology procedures for extraction of urinary tract lithiasis in the period from 1992 until 2006. Situs inversus is a medical condition with transposition of the organs in chest and abdomen. The condition may cause misdiagnosis due to inverted location of the origin of acute abdominal pain in cases of e.g. acute appendicitis. In this case report, the condition was first found on a CT scan for urolithiasis. It was also visible on a previously performed X-ray of the patient's chest, where both the apex of the patient's heart and the apex of a jewelry heart were located to the patient's right side. If this is not a unique incident, we propose this finding to be named "goldsmith's sign".

Herein we report the confirmation of fungus balls (circular collections of C.tropicalis) during a transurethral ureterolithotomy, which is as far as we know the first reported instance. A 250-kg heifer had signs of colic attributable to urolithiasis of the right kidney. Medical treatment did not result in resolution of clinical signs, and nephrectomy was carried out.
