Does septal thickness influence outcome of myectomy for hypertrophic obstructive cardiomyopathy?

OBJECTIVES: Patients with hypertrophic obstructive cardiomyopathy and basal septal thickness <18 mm are often considered unsuitable candidates for myectomy. Mitral valve (MV) replacement is frequently performed instead. We aimed to determine whether septal thickness affects outcomes and adequacy of myectomy. METHODS: Clinical and echocardiographic data were reviewed for 1486 consecutive adult patients with hypertrophic obstructive cardiomyopathy who underwent transaortic septal myectomy from January 2005 through December 2014. Comparisons between patients, grouped by septal thickness (<18 mm,

[Diagnosis and treatment of hypertrophic cardiomyopathies].

Hypertrophic cardiomyopathy is the most common inherited disease of the heart with a prevalence of 0.2 %. One third of patients show no obstruction of the left ventricular outflow tract, whereas two third develop a significant gradient under resting conditions and/or on exertion. Angina pectoris, dyspnea, syncope, heart failure und atrial fibrillation are typical clinical features. Apart from ECG and echocardiography, holter ECG and exercise testing via ergospirometry should be routinely used to identify an adverse clinical course and potential risk factors. Cardiac MRI is a complementary and

Myocardial Infarction Secondary to Inflammatory Myofibroblastic Tumor Obstruction of the Left Main: Treated With Primary PCI.

INTRODUCTION: Cardiac inflammatory myofibroblastic tumor is a rare tumor that can cause potentially fatal outcomes. CASE PRESENTATION: We describe a case where the tumor originated on the mitral valve and extended through the left ventricular outflow tract and aortic valve and into the left main artery obstructing it. Due to the hemodynamic instability of the patient, we proceeded for cardiac catheterization with the intention to aspirate the mass but were forced to do percutaneous intervention to stabilize the patient and bridge him to surgery. CONCLUSIONS: The patient underwent surgery sev

Myocardial perfusion scanning in patients considered for late arterial switch.

BACKGROUND: Our aims were to evaluate left ventricular uptake of radionuclide in patients with Mustard's or Senning's procedure, comparing them with patients who had undergone banding of the pulmonary trunk and conversion to the arterial switch. METHODS: Technetium perfusion scans were performed on 27 patients (25 male), aged from 10 to 28 years with a mean of 17.8 years and a standard deviation of 5.8 years, who had undergone Mustard's or Senning's procedure for correction of complete transposition. Of the 27 patients, six had been accepted for staged conversion to an arterial switch. At the

Pharmacologic resolution of functional outflow tract obstruction after mitral valve repair.

A 74-year-old woman with mitral regurgitation secondary to ruptured chordae tendineae, complicated by a cleft in the posterior mitral leaflet and a severely calcified mitral annulus, underwent mitral valve repair by implantation of polytetrafluoroethylene chords and closure of the cleft, without the use of an annuloplasty ring. Immediately after the repair severe left ventricular outflow tract obstruction developed secondary to the systolic anterior motion of the mitral valve. Echocardiography identified the cause as functional, in the presence of a hypertrophic left ventricle with no signific

[Mono and bidimensional echocardiographic control in patients surgically treated by abdominal apical conduit (author's transl)].

Creation of a double outlet left ventricle by means of an apical conduit may solve the problems related to left ventricular outflow tract obstructions which are not amenaable to conventional surgical management. Searching a diagnostic method easily applicable, repeatable and reliable, the results obtained with serial echocardiographic controls have been evaluated. This technique has shown to be of great value in answering several hemodynamic questions. An echocardiographic protocol has been developed for the follow-up of these patients.

[Update on hypertrophic cardiomyopathy].

In Europe, the term 'hypertrophic cardiomyopathy' (HCM) encompasses both primary, genetic forms and secondary, acquired forms. If echocardiography confirms wall thickening of the left ventricle without a clear connection to pathological stress conditions, a targeted differential diagnosis should be performed. Cardiac magnetic resonance imaging plays a key role in definitive diagnosis. Genetic testing should also be considered for many forms of HCM, especially for the common primary HCM. Stratification with regard to arrhythmia risk is mandatory for all patients with primary HCM. Symptomatic tr

Transapical mitral valve-in-ring procedure with a novel self-expandable transcatheter heart valve: first- and last-in-man report.

Interventional therapy of failing surgical mitral valve replacement and repair is an appealing option in patients who are not eligible for redo surgery. We herein report on the first-in-human use of a repositionable, self-expandable transcatheter heart valve with motorized delivery for a transapical mitral valve-in-ring procedure. After implantation, fluoroscopy showed no residual regurgitation and pressure measurements did not reveal any signs of left ventricular outflow tract obstruction.

Effect of septal ablation on myocardial relaxation and left atrial pressure in hypertrophic cardiomyopathy an invasive hemodynamic study.

OBJECTIVES: The objective of this study was to examine the effects of septal ablation on diastolic function with the use of invasive hemodynamics. BACKGROUND: Septal ablation is an alternative therapy for patients with obstructive hypertrophic cardiomyopathy (HCM). However, its beneficial effect on diastolic function, by relieving the systolic contraction load, may be countered by adverse effects from the infarction on left ventricular mechanics. METHODS: Using high-fidelity, micromanometer-tipped catheters, we examined 40 HCM patients by taking direct measurements of the left ventricular ou

Hypertrophic obstructive cardiomyopathy: mechanism of obstruction and response to therapy.

Hypertrophic cardiomyopathy is a primary, usually familial, disorder of heart muscle whose primary feature is muscular hypertrophy without recognized cause that encroaches on the ventricular chamber, reducing chamber area and volume. In roughly 25% of cases, there is associated obstruction to left ventricular outflow (hypertrophic obstructive cardiomyopathy [HOCM]). This article details the mechanism of obstruction in HOCM, focusing on obstruction at the mitral valve level, and reviews the pharmacologic and surgical therapies currently available. Mainstays of pharmacologic therapy include b-bl

Diagnostic pitfalls in constrictive pericarditis coexisting with right ventricular outflow tract obstruction and coronary artery anomaly.

Constrictive pericarditis associated with an anomalous left anterior descending artery is a very rare and complex condition. The aim of this report is to highlight the diagnostic challenges and surgical management in this disease. A 40-year-old man with right heart failure, dyspnea, and ascites, underwent comprehensive cardiac evaluation and surgery. The pulmonary valve was dysplastic, causing severe right ventricular outflow tract (RVOT) obstruction. During resection, the left anterior descending artery was inadvertently transected and bypassed with a saphenous vein graft. RVOT reconstruction

Apical ventricular septal defect with reverse bidirectional transseptal flow: a sign of restriction within the right ventricle.

Apical ventricular septal defects (VSD) may spontaneously become restricted or may even close following tissue outgrowth within the right ventricle in the region of the apical muscle bundles. We report a case of spontaneous restriction of an apical VSD localized within the right ventricle in the region of prominent apical trabeculae, in which pulsed Doppler interrogation showed an unusual pattern of bidirectional blood flow across the VSD, with right to left flow in systole and left to right in diastole. Angiography confirmed these unusual aspects. This reverse bidirectional transseptal flow i

Regional myocardial blood flow in left ventricular hypertrophy. An experimental investigation in Newfoundland dogs with congenital subaortic stenosis.

To test the hypothesis that left ventricular hypertrophy (LVH) may predispose the subendocardium to ischemia, we studied regional myocardial blood flow in dogs with the fibrous ring form of subvalvular aortic stenosis and concentric LVH. Radioactive microspheres, 9 +/- 1 mu in diameter, were used. Eleven dogs with LVH (left ventricular body weight ratio of 6.35 +/- 0.46 gm/kg [mean +/- SEM] and peak left ventricular outflow gradient of 51 +/- 7 mm Hg) were compared to 12 normal dogs (left ventricular/body weight ratio of 3.41 +/- 0.12 gm/kg and peak left ventricular outflow gradient of 6 +/- 3

Surgical management of double outlet right ventricle with outflow obstruction: a systematic review of biventricular strategies.

BACKGROUND: Double outlet right ventricle is a heterogeneous congenital defect in which both great arteries arise predominantly from the right ventricle. Several operative strategies exist, but contemporary paediatric outcome data have not been systematically synthesised. MATERIAL AND METHODS: A systematic search of PubMed, Embase, Scopus, the Cochrane Library, and medRxiv (2000-2025) identified cohort studies and case series including ≥10 children undergoing biventricular repair strategies reported in double outlet right ventricle cohorts, including intraventricular rerouting procedures, roo

Phenotypic overlap in hypertrophic cardiomyopathy: apical hypertrophy, midventricular obstruction, and apical aneurysm.

BACKGROUND: Within the diverse phenotypic spectrum of hypertrophic cardiomyopathy (HCM), subgroups of patients with apical hypertrophy (APH), midventricular obstruction (MVO), and apical aneurysm (APA) have emerged. While previous studies have suggested the existence of considerable overlap between APH, MVO, and APA, there are still many unanswered questions. Therefore, we attempted to clarify the relationship of the above three phenotypes of HCM with respect to prevalence, overlap, and outcomes. METHODS: Among the 544 study HCM patients (mean follow-up period: 11.6±7.4 years), 170 with APH (

The pathological spectrum of left-ventricular hypoplasia.

Left-ventricular (LV) hypoplasia encompasses a range of LV sizes, varying from a mildly underdeveloped, but functionally adequate, chamber to the miniscule, barely perceptible LV cavity seen in hypoplastic left-heart syndrome. Associated malformations include obstructive lesions of LV inflow, outflow, and the aortic arch, often in combination. Repair of complex combinations and/or severe LV hypoplasia usually results in a single-ventricle anatomy with the right ventricle serving as the systemic ventricle. New therapeutic interventions, including fetal procedures, are expanding the spectrum of

Surgical repair of double-chambered right ventricle with or without ventriculotomy.

Double-chambered right ventricle is an uncommon congenital anomaly consisting of obstruction of the trabecular zone of the ventricle by an elevated, hypertrophied moderator band, with or without additional muscle bundles(s). During an 18 month period (August 1984 to February 1986) 12 patients underwent repair of double-chambered right ventricle. Their ages ranged from 13 months to 12 years (mean 4.4 years). Associated lesions were ventricular septal defect in 11 patients (three closed spontaneously), atrial septal defect in two, left superior vena cava in three, aortic valve prolapse in one, a
