A variable-frequency probe-tone acoustic-impedance bridge has been developed to enable an artifact-free pure-tone acoustic reflex study to be carried out on neonates. Contralateral reflex thresholds for pure tones, broadband noise, and filtered noise were measured in 28 newborns aged 4-8 days. The mean reflex threshold for the broadband noise was 73 dB SPL, 4 dB lower than the reflex threshold for the 2600-Hz low- and high-pass noise bands. Reflex activation at 500, 1000, 2000, and 4000 Hz gave responses that closely followed the normal adult pattern although reflex thresholds were approximate

Profoundly deaf subjects were given information about sound level in their environment by means of a body-worn unit coupled to a small vibrator worn on the finger. Results of trials on 19 adults are discussed. The Tactile Acoustic Monitor was found to be useful for identifying domestic sounds by means of their distinctive timing patterns. No significant overall improvement in subject's control of voice level was observed, although some subjects found that having a voice level monitor gave them greater confidence to join conversations. A 73-year-old woman with non-Hodgkin's lymphoma had two episodes of severe, bilateral, sensori-neural hearing loss after vincristine therapy. Her hearing gradually, then completely returned 2-3 months after the vincristine therapy was discontinued. Bilateral ear infections, central nervous system lymphoma and infection, or other drug-induced neuropathies were excluded as possible etiologies for the deafness. Bilateral acoustic (VIII) nerve palsy in this patient was most likely a manifestation of vincristine neurotoxicity.

This in vitro study suggests that acoustic shadows distal to calculi contain significantly fewer echoes, and are therefore more sharply defined than those distal to gas collections. Acoustic and perceptual correlates of velopharyngeal competency in two groups of retarded (one Down syndrome, one idiopathic) and one group of nonretarded subjects, equally divided between males and females, was evaluated. Blood alcohol levels between 0.09% and 0.15% were found to reduce the protective action of the acoustic reflex in five human subjects with normal hearing. In the continuing search for methods to diagnose acoustic neuromas at an early stage, we have applied the auditory brain stem response examination to 27 patients with surgically verified acoustic neuromas. It is confirmed that the main indicator of retrocochlear versus cochlear disease is the interaural latency difference of the Jewett5 wave, the IT5. Women exhibit significantly lower latency values to J5 than men, approximately 0.25 msec. Age also has some influence, so that the J5 latency tends to increase with age, but in this investigation the trend did not reach statistical significance.

Acoustic stapedial reflexes were elicited from 10 normal S's in each of three age groups. Susceptance and conductance values were measured for each subject in both the unreflexed and reflexed states and at 10-sec intervals during a continuous, reflex-sustaining stimulation with pure tones. Percentage of reflex decay over time was noted for each group and as a function of stimulus frequency and measurement mode. No significant age group differences were noted. Significant stimulus frequency and measurement mode differences were presented to represent "normal" reflex decay.

Speech-elicited acoustic reflexes and three measures of subjective loudness perceptions were administered to 10 normal ears and to 20 ears with confirmed sensorineural hearing loss. Most notable was the comparison between loudness discomfort level and the acoustic reflex, revealing a limited relationship. Within the framework of a study on the natural history of acoustic trauma, over 600 soldiers were examined. A higher rate of bilateral acoustic trauma was observed among soldiers with longer service. Comparison between unilateral and bilateral acoustic trauma showed a greater severity of damage in both ears of the latter group. Comparison of the progression of the damage in the same persons on two consecutive examinations suggested the same trend. It is suggested that bilateral acoustic trauma may be a later and more severe stage in some types of noise-induced damage.
