[Depressive symptoms in pathomimia: comorbidity or psychiatric factitious disorder?].

BACKGROUND: The factitious disorders, more commonly known as pathomimia, are mainly expressed as organic symptoms voluntarily induced by the patient. Patients suffering from these disorders do not seek to obtain immediate secondary benefits, contrary to simulation. They send the physician a challenge, sometimes by means of self-mutilation, or exposure to a vital risk. Their objective is to raise the interest and the mobilization of the medical community. The patient will develop intense relationships with the medical staff, technically mobilized as well as emotionally, as far as the factitious

Association between air pollution exposure and mental health service use among individuals with first presentations of psychotic and mood disorders: retrospective cohort study.

BACKGROUND: Growing evidence suggests that air pollution exposure may adversely affect the brain and increase risk for psychiatric disorders such as schizophrenia and depression. However, little is known about the potential role of air pollution in severity and relapse following illness onset. AIMS: To examine the longitudinal association between residential air pollution exposure and mental health service use (an indicator of illness severity and relapse) among individuals with first presentations of psychotic and mood disorders. METHOD: We identified individuals aged ≥15 years who had firs

Severe psychiatric disturbance and abnormalities of the corpus callosum: review and case series.

The association between developmental defects of the corpus callosum and major psychiatric disturbance is discussed with a review of published cases. Seven new cases are presented, of which four had clear psychotic symptoms, two receiving a diagnosis of schizophrenia. Of the remainder, one had a developmental disorder affecting social interaction and speech which could be classed as Asperger's syndrome, one had a personality disorder with depressive and conversion symptoms, and the last was an adolescent boy with severe behavioural problems. The difficulties in determining the precise relevanc

What does history teach us about factors associated with relapse in bipolar affective disorder? Presentation at Hilton hotel, Gateshead, 2 September 2005 at Symposium entitled managing The aftermath of mania.

When investigating treatments for any chronic condition it is essential to know the usual course of the illness concerned. The natural history of bipolar affective disorder has only been established relatively recently. This review examines the factors that affect the course of bipolar disorder from an historical perspective. These include the affective nature of the episodes and the influence of psychotic symptoms, age at onset of illness, length of episodes and cycles, gender, ethnicity, concurrent drug and alcohol use, occupational status and factors leading to chronicity. The pioneering wo

Lamotrigine treatment of a patient affected by epilepsy and anxiety disorder.

Epilepsy often occurs in comorbidity with mental diseases and disorders. Early detection and/or treatment of such disorders in patients affected by epilepsy, as well as their socialisation are crucially important since epileptic patients tend to suffer more due to lack of social support than to frequent epileptic seizures. Prevalence of psychiatric disorders is higher in patients with epilepsy than in general population, the most frequent being: anxiety, depression, panic attacks, behavioural disorders as well as psychotic states with paranoid elements. The efficacy of AE treatment of patients

Validity of the family history method for identifying schizophrenia-related disorders.

We examined the family history method's validity for identifying schizophrenia related disorders (SRD) by comparing family history and family study derived diagnoses. First degree relatives (n = 284) of 48 psychiatrically disordered probands, predominantly with schizophrenia, were diagnosed using the Family History RDC (FH-RDC) which include three psychotic schizophrenia related disorders (P-SRD): schizophrenia, chronic SAD and chronic unspecified functional psychosis (CUFP). Supplementary criteria for schizophrenia related personality disorders (SRP), derived to identify schizotypal and paran

Stress incubation and the onset of affective disorders.

Incubation is the process by which life events influence the onset of psychiatric disorder after an appreciable delay. It has long been recognised clinically. In this paper we use data from the Camberwell Collaborative Depression Study to see whether incubation effects can be demonstrated in depressive illness. We used a novel adaptation of survival analysis for this purpose. The results suggest that incubation does occur in depressive disorder, that it is much less important than the effect of life events close to onset, that it is apparent in women but not in men, and that it is no more evid

Impaired olfactory function in bipolar disorder patients during acute episodes regardless of psychotic symptoms.

BACKGROUND: The aim of this study was to analyze whether the presence of psychotic symptoms affects olfactory function in patients with bipolar disorder (BD). We also compared olfactory function between the period of episode and remission in patients with BD. METHODS: BD patients in the acute phase were tracked to the remission stage. The psychiatric symptoms and social function of the enrolled subjects were assessed using the Hamilton Rating Scale for Depression (HAMD), the Young Mania Rating Scale (YMRS), the Hamilton Rating Scale for Anxiety (HAMA), the Positive and Negative Syndrome Scale

Course of illness in a sample of patients diagnosed with a schizotypal disorder and treated in a specialized early intervention setting. Findings from the 3.5year follow-up of the OPUS II study.

BACKGROUND: Previous studies report that 20% to 30% of those initially diagnosed with schizotypal disorder go on to develop a psychotic disorder (predominantly schizophrenia). Schizotypal disorder share some traits of those used to identify patients at ultra-high risk for psychosis. METHOD: As part of a randomized clinical trial testing the effect of prolonged specialized early intervention, we recruited 83 participants diagnosed with a schizotypal disorder. Participants were recruited 18 months into their two-year treatment program, and follow-up interviews were conducted three and a half ye

Bipolar disorder, miscarriage, and termination.

OBJECTIVES: To compare rates of bipolar episodes following miscarriage and termination with those occurring in the postpartum period. METHODS: Information in relation to pregnancy and childbirth was gathered retrospectively for 1,283 women with broadly defined bipolar disorder by interview and case-notes review. RESULTS: Rates of mania or affective psychosis were significantly higher after full-term delivery than after termination (p < 0.001) or miscarriage (p < 0.001). Rates of non-psychotic major depression were similar following full-term deliveries, miscarriages (p = 0.362), and terminat

Some cognitive correlates of affective disorders.

Seventy-one men completed a battery of cognitive tests which were designed to reflect verbal analytic and non-verbal holistic functioning. Interest centred around pattern of response. Thirty men were suffering from an affective disorder and forty-one were well. All the men were in prison, the majority awaiting trial. The affective disorder group was subdivided into three categories: men who had a history of manic-depressive illness; a group of unipolar, psychotically depressed men; and men who were regarded as being depressed in reaction to circumstances. All three groups showed specific diffi

Clinical characterization of depressive mixed state in bipolar-I patients: Pisa-San Diego collaboration.

BACKGROUND: Although mixed states were classically described as various concomitant admixtures of depression and mania, the official current definitions in both DSM-IV and ICD-10 tend to restrict the concept to manic patients with full syndromal depression. Recent research has actually shown that mania with few depressive symptoms constitutes the most prevalent clinical presentation of mixed or dysphoric mania. Major depressive patients with few concomitant manic symptoms are not officially recognized within the current nosology. In this paper we attempt to delineate the clinical profile of su

Traumatic experiences and substance abuse: mapping the territory.

This article examines the relationships between various types of traumatic experiences and addictive behavior, with an eye to formulating effective treatment strategies. Interventions in the posttraumatic stress disorder (PTSD) and related fields are reviewed in an effort to understand how best to integrate them into substance abuse treatment. The recovery-oriented therapy model is used as a framework to define treatment tasks at each stage of the recovery process: how one addresses painful issues depends on the objective, given the recovery stage at hand. These tasks include making a commitme

[Post emotional syndromes (author's transl)].

On the basis of ten clinical cases of post emotional shock, the following aspects are discussed: the causal emotion and its characteristics, its psychological repercussions and the mechanisms involvent, the clinical patterns observed, both somatic and psychic, and their interpretation. Attention is called to the consequences of such emotions from the forensic view-point and the need to ascertain and specify the nature and intensity of the emotional trauma and the actual state of anxiety. Emphasis is laid on the difficulty of making expert appraisals and fixing equitable compensations, as exper

Dual diagnosis and chronology of illness in treatment-seeking Italian patients dependent on heroin.

Although there are studies describing the prevalence of patients with drug addiction and dual diagnosis, there is a paucity of data regarding the progression from psychiatric disorders to drug addiction or vice versa. A total of 1,090 patients dependent on heroin were interviewed to examine the presence of dual diagnosis and the progression from psychiatric disorders to drug dependence or vice versa. A total of 574 patients met the criteria for a dual diagnosis. A total of 362 patients progressed from substance abuse disorders to psychiatric disorders (SUD-PR) and 144 patients progressed from

A rare case report of Lilliputian and Brobdingnagian hallucinations in a case of pemphigus vulgaris.

Pemphigus vulgaris is an autoimmune disorder characterized by intraepithelial, blistering lesions affecting the skin and mucous membrane. Psychiatric manifestations of pemphigus vulgaris are generally described secondary to steroid and immunosuppressant therapy though recent studies highlight independent association of pemphigus with psychotic disorders. We describe a unique development of Lilliputian hallucinations, their transformation into Brobdingnagian hallucinations on treatment with steroids and gradual resolution back to Lilliputian status on treatment with risperidone. Earlier Lillipu

Cannabidiol, neuroprotection and neuropsychiatric disorders.

Cannabidiol (CBD) is a non-psychotomimetic phytocannabinoid derived from Cannabis sativa. It has possible therapeutic effects over a broad range of neuropsychiatric disorders. CBD attenuates brain damage associated with neurodegenerative and/or ischemic conditions. It also has positive effects on attenuating psychotic-, anxiety- and depressive-like behaviors. Moreover, CBD affects synaptic plasticity and facilitates neurogenesis. The mechanisms of these effects are still not entirely clear but seem to involve multiple pharmacological targets. In the present review, we summarized the main bioch

Exploring the dimensional structure of the Thought and Language Disorder scale in acute psychiatric inpatients.

AIMS: To explore the dimensional structure of the Thought and Language Disorder scale (TALD) in a sample of acute psychiatric inpatients and to contribute to a characterization of the latent dimensions underlying formal thought disorder (FTD). METHODS: A total of 250 acute patients with psychotic and affective disorders were assessed at admission using the TALD. Exploratory factor analysis (EFA) with minimum residual extraction and promax rotation was conducted, followed by second-order EFA and Schmid-Leiman orthogonalization to evaluate the hierarchical organization of FTD. RESULTS: Hierarc
