Vertical alveolar distraction osteogenesis with complications in a reconstructed mandible.

We report here a case of vertical alveolar distraction osteogenesis with many complications that required further surgical interventions. A 54-year-old man underwent mandibular resection followed by iliac bone grafting as the result of large mandibular odontogenic keratocyst. Eleven months later, alveolar vertical distraction osteogenesis was applied to the patient for prosthetic rehabilitation. Fracture of the basal bone occurred in the consolidation period, and the fracture was fixed by the titanium miniplate system. Radiographic examination after completion of distraction osteogenesis confi

Prosthetic rehabilitation of the atrophic maxilla using pre-implant surgery and endosseous implants.

Pre-implant surgery was carried out in 20 patients with advanced atrophy of the edentulous maxilla, using autogenous bone grafts. Endosseous implants were placed after the initial healing period and these were used to retain removable overdentures. Patients were observed for up to 5 years with 15 of the 105 implants placed being lost and two remaining as sleepers. The resulting implant success rate was 84%. Three patients had to return to conventional dentures while the remaining 17 expressed a high degree of satisfaction with their implant retained prostheses. Pre-implant surgery successfully

Precision implant placement: A novel approach using dynamic navigation system-guided alveolar ridge splitting.

Conventional alveolar ridge splitting (ARS) technique involves a significant risk which may prove problematic. This case report presents the first documented application of dynamic navigation system (DNS)-guided ARS, demonstrating a precise approach to implant placement in a challenging anatomical scenario. A 37-year-old partially edentulous patient presented with missing mandibular posterior teeth. Clinical examination revealed a deficient alveolar ridge. Cone-beam computed tomography evaluation showed a Siebert's Class I resorbed ridge with 2.5 mm width and bone density ranging from 870 to 1

Severely Atrophic Mandibles Restored With Fiber-Reinforced Composite Prostheses Supported by 5.0-mm Ultra-Short Implants Present High Survival Rates Up To Eight Years.

PURPOSE: Encouraging results have been reported for ultrashort single implants; however, long-term investigations are warranted for full-arch reconstructions. This study evaluated marginal bone loss, implant, and reconstruction survival of fiber-reinforced composite full-arch prostheses supported by 4 ultrashort implants. METHODS: Patients with severely atrophic mandibles (Cawood and Howell class V and class VI) were included in this cohort study. Study predictors included time (initial and last follow-up) and vertical (epicrestally or subcrestally) and horizontal implant position (medial or

Early volumetric changes after vertical augmentation of the atrophic posterior mandible with interpositional block graft versus onlay bone graft: A retrospective radiological study.

PURPOSE: The aim of this study was to evaluate volumetric and clinical outcomes of atrophic posterior mandibles treated with inlay or onlay bone grafting techniques. MATERIALS AND METHODS: In posterior mandibles, alveolar ridges were treated either with interpositional equine cancellous bone block (inlay group) or with onlay autogenous bone block (onlay group). Bone volumes at baseline and at 4 months after surgery were measured by computed tomography. RESULTS: A total of 20 subjects were enrolled in the present study: 10 in the inlay group and 10 in the onlay group. After surgery, atrophic

[Sinus floor bone augmentation: implementation of the concept of sinus-lift].

Sinus floor elevation is a technique allowing implant placement in the posterior edentulous maxilla when residual bone height is limited. This technique has evolved a lot since it has been first introduced in the eighties. Nowadays, this approach is considered as predictable, simple and practicable under local anaesthesia. The present article aims at understanding clearly the biological phenomenon of "sinus-lift". Moreover, the surgical protocol used at the University of Liège is considered didactically and reasoned according to the scientific literature.

Ridge preservation utilizing an alloplast prior to implant placement--clinical and histological case reports.

A calcified copolymer alloplast (Bioplant HTR, Bioplant Inc., South Norwalk, CT) was utilized to fill osseous defects in five patients. Hard tissue cores were obtained from the grafted sites and prepared for biopsy. In one patient, the lining of a soft tissue flap was biopsied 8 months postgrafting. Histological evaluation of the specimens revealed that the copolymer particles placed adjacent to the bony defect walls served as an osteoconductive material in which vital remodeling bone and marrow formed and fused to the surface of the particles. This process of bone deposition and remodeling wa

Experience of maxillary sinus floor augmentation in the presence of antral pseudocysts.

Sinus floor elevation is increasingly being used with implant placement procedures. However, certain surgical difficulties can develop, such as sinus membrane perforation, bleeding, anatomic variations, or some pathologies of the maxillary sinus. Maxillary antral pseudocysts are one of the most common benign pathologies of the maxillary sinus. Although some investigators have described the presence of this pathology as a contraindication, recent reports have demonstrated that pseudocysts of the maxillary sinus are not a contraindication for sinus augmentation. In these reports, maxillary sinus

Review article: oral surgery and TMJ-related papers published in BJOMS in 2008 and 2009.

This review summarises the 81 papers relating to oral surgery and to the temporomandibular joint (TMJ) published in the British Journal of Oral and Maxillofacial Surgery (BJOMS) January 2008-December 2009. They include full length articles on distraction osteogenesis, imaging, extraction techniques, implants, and the management of facial pain and disorders of the TMJ including joint replacement. Many short communications were also published, but an editorial decision was taken in August 2009 to publish most of these in future in the online journal only. As BJOMS encompasses the whole remit of

Vertical distraction osteogenesis of fibula transplant for mandibular reconstruction: a case report.

Bone continuity defects in the mandible are caused by tumor surgery, trauma, infection, or osteoradionecrosis. Today, reconstruction of long-span mandibular defects with a free fibular flap is a routine procedure. However the bone height of the mandible after reconstruction is about half that of the dentulous mandible. Therefore, the deficiency in bone height makes implant placement impractical. In our case, because it was necessary to restore the mandibular height, a vertical distraction osteogenesis was performed on the grafted mandible of the patient who was referred to our clinic with a re

Multidisplinary management of hypodontia in adolescents: case report.

The purpose of this article is to describe a team approach to treatment of hypodontia in adolescent dentition. A case report of hypodontia with a microdontic lateral incisor in a Class I malocclusion illustrates the principles of case management. Multidisciplinary consultation during treatment planning and coordination and appropriate timing of subsequent interdisciplinary dental care enables the clinician to provide the optimum care. The scope of orthodontic and restorative management depends on the severity of the hypodontia.

Cocaine associated with onlay bone graft failure: a clinical and histologic report.

This patient report presents an unusual onlay bone graft failure following local cocaine application. Three months after the bone grafting procedure performed in the anterior maxilla for bone volume augmentation, the bone graft was totally exposed in the oral cavity as a result of the rubbing of cocaine on the gingival tissue that covered the bone graft. A histologic view of the removed bone fragment presented not only an area of necrosis but also ample spaces filled with necrosis material and resorption areas. Dental practitioners need to be aware of this phenomenon because such patients ofte
