Examining a bone marrow (BM) karyotype of a patient with a refractory anemia with excess of blasts (RAEB), we detected a clone with an unbalanced translocation(1;17), resulting in monosomy of 17p and trisomy of 1q. Using fluorescence in situ hybridization (FISH) technique with alpha-satellite DNA probes specific for chromosomes 1 and 17, we observed that the chromosome derived from the translocation shows hybridization signals for both the centromeres of chromosomes 1 and 17. This finding suggests that the breakpoints of the two autosomes involved in the rearrangement occurred in the primary c

This report deals with a case of RAEB in transformation (RAEB-T) who received peripheral blood stem cell transfusion (PBSCT) and has remained in complete remission for 15 months. A 51-year-old man was admitted to our hospital with abnormal hematological examinations in August 1991. Peripheral blood showed a WBC of 3,580/microliters with 19% myeloblasts. Bone marrow aspirate revealed slight hypocellularity with 21.2% myeloblasts. Many of these myeloblasts contained Auer rods. He was diagnosed as having RAEB-T. He was treated with low dose Ara-C, followed by the BHAC-VEP regimen and complete rem

There is experimental evidence that a portion of follicle-stimulating hormone (FSH) secretion is independent of hypothalamic influences. A 29 year old woman with familial pure gonadal dysgenesis developed myelodysplastic syndrome. Endocrine investigations showed discrepancy between serum FSH and luteinizing hormone (LH) levels. FSH levels remained elevated while LH levels decreased. The FSH to LH ratio was 10 (normal 2-2.5). The fall in LH is likely to be due to factor(s) involved directly and specifically in LH synthesis and release. Exogenous LH releasing hormone administration as well as ho

We herein describe a 49-year-old man with severe heart failure due to fulminant myocarditis who underwent left ventricular assist device implantation and received clopidogrel and warfarin as antithrombotic agents. The patient developed anemia secondary to chronic bleeding gastric hyperplastic polyps, necessitating endoscopic mucosal resection. Despite attempts to manage post-endoscopic mucosal resection bleeding from a gastric ulcer by endoscopic hemostasis using hemostatic forceps, local hemostatic agents, and polyglycolic acid sheets, the bleeding persisted. Hemostasis of the refractory blee

Idiopathic Pulmonary Hemosiderosis (IPH) is characterized by the triad of iron deficiency anemia, pulmonary infiltrates and haemoptysis with no recognizable cause. Since the first description of its association with Celiac Disease (CD) by Lane and Hamilton in 1971, only a few isolated cases have been reported in literature. Although it has been considered an uncommon association of two disease entities, recent reports indicate that prevalence of celiac disease is as high as one percent. Further, individually both celiac disease and IPH are known to present as refractory anemia only. We are rep

Nijmegen breakage syndrome (NBS) is characterized by growth retardation, microcephaly, mental retardation, immunodeficiency, and predisposition to malignancies, especially B-cell lymphomas. In contrast, leukemia is rare. A 23-year-old NBS patient presented with anemia, thrombocytopenia, and hyperlymphocytosis. The diagnosis of T-cell prolymphocytic leukemia (T-PLL) was confirmed by cytological and immunological assays (TdT(-), CD2(+), CD5(+), CD3m, and CD7(+)). Biological assays also showed a hemolytic anemia and a clotting factor V decrease. The patient was first treated by methylprednisone f

Cold agglutinin mediated immune hemolytic anemia secondary to lymphoproliferative disease (LPD), is primarily treated with measures directed to eliminate the malignant clone and as such, chemotherapy is usually given. The recent availability of monoclonal antibodies, has made it feasible to obtain both a clinical and molecular remission, as well as a remission on the functional level, such as elimination of secondary autoimmune phenomena. Recently we have administered a course of monotherapy with rituximab (4 weekly injections, x 375 mg/m2) to a patient with refractory and transfusion dependen

BACKGROUND: Neurological compromise due to extramedullary hematopoiesis is a rare occurrence, typically associated with myeloproliferative disorders such as thalassemia or hereditary spherocytosis. Few cases have been reported in the setting of sickle cell anemia, and there is no established consensus on optimal treatment. Management strategies range from exchange transfusion to radiotherapy and surgical decompression. This case report presents a rare case of stenosis compression caused by extramedullary hematopoiesis in a patient with sickle cell anemia and highlights the therapeutic efficacy

To avoid iron deficiency and overload, iron availability is tightly regulated at both the cellular and systemic levels. The liver peptide hepcidin controls iron flux to plasma from enterocytes and macrophages through degradation of the cellular iron exporter ferroportin. The hepcidin-ferroportin axis is essential to maintaining iron homeostasis. Genetic inactivation of proteins of the hepcidin-activating pathway causes iron overload of varying severity in human and mice. Hepcidin insufficiency and increased iron absorption are also characteristic of anemia due to ineffective erythropoiesis in

A new immunoglobulin (Ig) for intravenous use was given in high doses to 4 children with refractory idiopathic thrombocytopenic purpura (ITP) and 2 children with idiopathic aplastic anemia (IAA). Within 5-10 days after initiation of Ig therapy the platelets of the children with ITP rose to 300,000-650,000/mm3 and could be maintained at normal levels with one Ig infusion every 1-3 weeks. No reaction was observed in the 2 patients with IAA.

Kidney transplant recipients are at increased risk of developing clinical disease due to uncommon opportunistic viral pathogens. Refractory anemia is classically associated with parvovirus B19 infection. West Nile virus has the propensity to cause fever and neurologic symptoms, while spastic paresis and lymphoma can be triggered by human T cell lymphotrophic virus. In this review article, the epidemiology, clinical manifestations, diagnosis and treatment of less common viruses are discussed in the setting of kidney transplantation.

Hemoglobin D variations are a group of hemoglobinopathies caused by mutations in the genes that control the synthesis of new globin chains. Hemoglobin D-Punjab is the most prevalent but frequently asymptomatic, it can occasionally cause mild to moderate hemolytic anemia, making diagnosis difficult and raising the risk of misdiagnosis. This article discusses a rare instance of a seventeen-year-old male in Sindh, Pakistan with iron deficiency anemia who was later found to have the Punjab variation of the hemoglobin D. The patient had signs of weakness, exhaustion, and shortness of breath, which

Iron is an essential nutrient to life and is required for erythropoiesis, oxidative, metabolism, and enzymatic activities. It is a cofactor for mitochondrial respiratory chain enzymes, the citric acid cycle, and DNA synthesis, and it promotes the growth of immune system cells. Thus, iron deficiency (ID) leads to deleterious effects on the overall health of individuals, causing significant morbidity. Iron deficiency anemia (IDA) is the most recognized type of anemia in patients with celiac disease (CD) and may be present in over half of patients at the time of diagnosis. Folate and vitamin B12

BACKGROUND: Sulofenur is a diarylsulfonylurea with demonstrated antitumor activity in patients with advanced epithelial ovarian cancer refractory to standard chemotherapy. The dose-limiting toxic effects observed in phase I clinical trials have been anemia and methemoglobinemia, resulting in cyanosis.
PURPOSE: The purposes of this study were to further define the response rate, toxic effects, and pharmacokinetics and pharmacodynamics of sulofenur in patients with advanced ovarian cancer.
METHODS: We conducted a phase II trial of sulofenur at a dose of 800 mg/m2 per day in 35 patients with st

Autoimmune hemolytic anemia (AIHA) is a potentially life-threatening complication of lymphoid malignancies, and transfusion management can be particularly challenging in refractory cases. We report a case of angioimmunoblastic T-cell lymphoma complicated by severe, transfusion-refractory warm AIHA, in which red blood cell transfusion was paradoxically associated with a hyperhemolysis-like process and overt disseminated intravascular coagulation. Despite corticosteroid therapy and rituximab, repeated transfusions failed to produce sustained hemoglobin increments and were followed by worsening h

Acquired thrombotic thrombocytopenic purpura (aTTP) is a life-threatening systemic thrombotic microangiopathy characterized by the presence of anti-ADAMTS13 antibodies (inhibitor). Here we report the case of a patient with refractory aTTP successfully treated with cyclosporine. A 69-year-old man presenting with hematuria and petechiae was referred to our hospital; he was disoriented and febrile. Laboratory results revealed Coombs-negative hemolytic anemia, thrombocytopenia, and renal failure. Undetectable ADAMTS13 activity and presence of anti-ADAMTS13 antibodies (inhibitor) confirmed the diag

Transcatheter Edge-to-Edge Repair (TEER) is an established therapy for high-surgical-risk patients with severe mitral regurgitation (MR). Iatrogenic atrial septal defects (iASD) are a routine consequence of transseptal puncture, and significant right-to-left shunting (RLS) causing refractory hypoxemia is rare. This report presents a man in his 80s with multiple comorbidities who underwent successful surgery. Despite procedural success confirmed by transesophageal echocardiography (TEE), the patient developed refractory hypoxemia during the early recovery phase while still undergoing mechanical
