Gluteal ischemic gangrene due to chronic aortoiliac occlusive disease.

Gluteal gangrene rarely complicates interventional procedures such as angiographic embolization in the pelvis or may develop postoperatively after open ligation or endovascular covering or embolization of one or mainly both internal iliac arteries in abdominal aortic surgery if collateral circulation is compromised. On the other hand, gluteal gangrene as a primary manifestation of chronic aortoiliac occlusive disease is very exceptional in literature. We present a patient with atherosclerotic aortoiliac obstruction and a necrotic eschar on her left buttock treated with aortobiiliac bypass afte

Endovascular Embolization in a Rare Case of Left Basal Ganglia Large Arteriovenous Malformation with Hydrocephalus: A Case Report.

Brain arteriovenous malformation (AVM) is a rare congenital disorder affecting young adults with an incidence of 0.94 per 100,000 population. Intracranial digital subtraction angiography has to be done in all patients and grading of AVM is done as per Spetzler-Martin grading. We report a rare case of left basal ganglia large AVM treated by endovascular embolization. Our experience with endovascular embolization using Onyx is successful in the treatment of large brain AVM. Endovascular embolization with Onyx is safe and feasible in deeply located large AVMs of the brain. Our patient has postope

Carotid-Cavernous Sinus Fistula caused by Persistent Primitive Trigeminal Artery Aneurysm rupture: A case report.

Carotid-Cavernous sinus fistula (CCF) caused by a ruptured aneurysm of persistent primitive trigeminal artery (PPTA) is exceptionally rare. Herein, we reported the case of a 64-year-old female who presented with headache, pulsatile tinnitus, and diplopia. Angiography showed a PPTA and CCF. Three-dimensional digital subtraction angiograms revealed a direct shunt between the PPTA aneurysm and the left cavernous sinus. The trigeminal carotid fistula was successfully treated with detachable coils and Onyx, and the PPTA was preserved. Hence, coil and Onyx occlusion of the cavernous sinus is a safe

[A3-A3 side-to-side anastomosis combined with endovascular intervention in recurrent complex anterior artery aneurysm: a case report and literature review].

A 28-year-old female patient was admitted to the Second Affiliated Hospital, Zhejiang University School of Medicine, with sudden headache and vomiting for 1 day. CT scan conducted at emergency revealed subarachnoid hemorrhage, whereas digital subtraction angiography demonstrated a wide-neck aneurysm located at A1 segment of the left anterior cerebral artery. The aneurysm was totally coiled using stent assistance, which, however, was recanalized at 3 month follow-up. This patient was then subjected to aneurysm and parent artery occlusion after bypass of the bilateral A3 segments, who recovered

Jugular vein invasion rate in surgically operated paragangliomas: a multimodality retrospective study.

PURPOSE: To assess the rate of jugular vein (JV) invasion by paragangliomas and imaging's ability to predict invasion. METHODS: Imaging studies of patients with paragangliomas were evaluated for JV invasion. Rates of invasion by different paragangliomas and each modality's accuracy were determined. RESULTS: JV invasion occurred in 11/11 glomus jugulares (GJs), 3/5 glomus vagales, and 2/8 carotid body tumors. Accuracy for magnetic resonance imaging (92.3%), computed tomography (92.3%), and digital subtraction angiography (94.1%) was comparable. CONCLUSIONS: JV invasion is characteristic of b

Endovascular management of a young patient of stroke with absent right upper limb pulse.

A 39 years old, right handed, hypertensive non-diabetic smoker presented with ischemic stroke with left sided hemiplegia and absent right upper limb peripheral pulses. Possible causes of young stroke were searched. Cerebral digital subtraction angiography (DSA) revealed 95% stenosis at origin of brachiocephalic trunk with absent flow in right sub-clavian artery and 75% stenosis at osteum of left vertebral artery with flow reversal through right vertebral artery. Subsequently, stents were placed within the stenosed portion Right brachiocephalic trunk and left vertebral artery. The intervention

A rare case of combined soft-tissue and intraosseous arteriovenous malformation of the hand with diffuse periosteal elevation: imaging appearances.

Arteriovenous malformations are uncommon congenital lesions of the hand. Although present at birth, they usually manifest later in life. Ultrasonography, MRI and digital subtraction angiography play an important role in the diagnosis of these lesions. We report the imaging appearances of an unusual case of combined soft-tissue and intraosseous arteriovenous malformation (AVM) of the hand with marked periosteal elevation in a 12-year-old child. Although associated skeletal changes are common, diffuse periosteal elevation owing to direct communication of the AVM with the subperiosteal space has

Gadopentetate dimeglumine as a potential alternative contrast medium during percutaneous coronary intervention: a case report.

BACKGROUND: There have not been previous reports of patients undergoing percutaneous coronary intervention (PCI) using a gadolinium chelate. METHODS AND RESULTS: A 74-year-old woman, who had a history of anaphylactic shock 4 times in response to iodinated contrast media despite preprocedural intravenous administration of hydrocortisone, was hospitalized because of unstable angina refractory to intensive medical treatment. Fully considering the risks of iodinated agents, digital subtraction coronary angiography and PCI were performed using gadopentetate dimeglumine without any side effects or

Bilateral anomalous posterior inferior cerebellar artery-anterior inferior cerebellar artery anastomotic arteries associated with a ruptured cerebral aneurysm--case report.

A 50-year-old man presented with subarachnoid hemorrhage from a ruptured cerebral aneurysm arising from a left posterior inferior cerebellar artery (PICA)-anterior inferior cerebellar artery anastomotic artery manifesting as severe headache, obtundation, and quadriplegia. Conventional and three-dimensional digital subtraction angiography showed that the anastomotic complex was present bilaterally and both vertebral arteries terminated at the origin of the PICA. The identification of this anomalous vascular network allowed coil embolization of the broad-based aneurysm with occlusion of the pare

Spontaneous thrombosis of a giant intracavernous internal carotid artery aneurysm and ipsilateral internal carotid artery occlusion.

A 75-year-old man who had suffered from right visual disturbance for 10 years suddenly experienced right cavernous sinus syndrome. Magnetic resonance imaging revealed a giant thrombosed aneurysm in the right cavernous sinus extending to the right middle cranial fossa. Digital subtraction angiography disclosed occlusion of the right internal carotid artery at the petrous portion and good cross filling in the right-sided circulation through the anterior communicating artery. There was no filling of the aneurysm. In this case, the mechanism of parent artery occlusion is unclear, but direct compre

The formation and regression of a flow-related cerebral artery aneurysm.

The authors report a patient with an aneurysm of the right cerebral posterior communicating artery (PCoA) developing after thrombotic pseudo-occlusion of the right internal carotid artery (ICA). The aneurysm regressed spontaneously subsequent to ipsilateral ICA endarterectomy and reversal of blood flow in the PCoA. The formation and regression of the aneurysm was well documented by repeat cerebral digital subtraction angiography studies, computed tomography and magnetic resonance imaging. The authors conclude that the formation and regression of this 'flow-related' aneurysm was associated with

Angiographic arterial occlusion and extra-anatomical vascular bypass for the management of a ureteral-iliac fistula: case report and review of the literature.

Ureteroarterial fistulas related to nonvascular etiology are rare clinical entities that are difficult to diagnose and manage. Diagnosis is best made by a combination of studies, including digital subtraction angiography. Low morbidity and mortality rates have been described with either percutaneous thrombosis of the iliac artery and extra-anatomic bypass, or by placement of a stented graft in the involved artery. We report on a well-documented case of a ureteroarterial fistula managed successfully with interventional radiologic embolization of the common iliac artery and immediate femoral-fem

[Surgical repair of vascular ring with aberrant brachiocephalic artery].

The vascular ring causes tracheal and esophageal compression and some cases need the surgical intervention. We report a rare case of vascular ring with aberrant brachiocephalic artery. A nine-month-old boy had suffered from progressing stridor and was admitted to our hospital due to the stridor. The digital subtraction angiography revealed the aberrant brachiocephalic artery which compressed the trachea from anterior aspect. The compression of the trachea was released by tacking forward the aberrant brachiocephalic artery to the sternum with the aid of two stitches. The postoperative course wa

Angiographically occult in-stent protrusion detected by intravascular ultrasound during carotid artery stenting: a case report.

BACKGROUND: Carotid artery stenting (CAS) is an established revascularization strategy for carotid stenosis, but periprocedural ischemic events remain a concern. In-stent protrusion, representing plaque protrusion and/or adherent thrombus, may be clinically relevant and can be underestimated by angiography alone. Intravascular ultrasound (IVUS) provides cross-sectional visualization of the stent lumen and may detect lesions not apparent on digital subtraction angiography. CASE PRESENTATION: We report the case of a 92-year-old man of Japanese ethnicity with symptomatic right internal carotid a

Multiple Cervical Arteries Dissection Associated With Fibromuscular Dysplasia: A Case Report.

Spontaneous cervical artery dissection is a condition often associated with connective tissue diseases, such as fibromuscular dysplasia (FMD), a complex angiopathy primarily affecting medium-sized arteries. While single-vessel dissection is more common, simultaneous involvement of three cervical arteries is rare. We present the case of a 30-year-old female patient who suffered from bilateral cerebellar infarction, subarachnoid hemorrhage, and hydrocephalus, necessitating external ventricular drainage and posterior fossa decompressive craniectomy. Multimodal imaging, including computed tomograp

Hypoglossal Nerve Palsy due to Compression by a Persistent Primitive Hypoglossal Artery: Case Report.

A 51-year-old Japanese woman was admitted to our hospital because of speech difficulty following severe headache. Neurological examination showed dysarthria and tongue weakness on the right side, indicating right hypoglossal nerve palsy. Needle electromyography of the right side of the tongue showed fibrillation potentials. Magnetic resonance angiography and computed tomography angiography revealed a right, persistent, primitive hypoglossal artery (PPHA) that met Lie's diagnostic criteria. Digital subtraction angiography showed an extended PPHA with irregular caliber in the portion running thr

Advances in the Pathogenesis, Diagnosis and Treatment of Bow Hunter's Syndrome: A Comprehensive Review of the Literature.

Bow hunter's syndrome (BHS), also known as rotational vertebral artery (VA) occlusion syndrome, is a rare yet treatable type of symptomatic vertebrobasilar insufficiency resulting from mechanical occlusion or stenosis of the VA during head and neck rotation or extension. The symptoms of BHS range from transient vertigo to posterior circulation stroke. The underlying pathology is dynamic stenosis or compression of the VA by abnormal bony structures with neck rotation or extension in many cases, such as osteophyte, disc herniation, cervical spondylosis, tendinous bands or tumors. Imaging approac

Imaging difficulties in Takayasu arteritis - case report and review of the literature.

BACKGROUND: Takayasu arteritis is an inflammatory disease of large-diameter arteries. Aorta and its branches are most frequently affected. Takayasu arteritis occurs mainly in young women and, if left untreated, leads to fatal complications. Digital subtraction angiography (DSA) is considered the gold standard in imaging of Takayasu arteritis. CASE REPORT: A thirty-five-year-old woman was admitted to the hospital with transient loss of consciousness, effort-associated vertigo, upper limb weakness and temporary vision problems. On admission, there was no pulse on the left radial artery while th

Pseudotumoral presentation of a cervical extracranial vertebral artery aneurysm in neurofibromatosis type 1: case report.

OBJECTIVE: Neurofibromatosis Type 1 (NF1) is known to be associated with vascular lesions. Association with an extracranial vertebral artery aneurysm is very rare. We report the case of such an aneurysm mimicking a cervical neurofibroma in NF1. CLINICAL PRESENTATION: An 18-year-old woman with previously diagnosed NF1 presented with a C6 radiculopathy. There were no clinical features suggesting a vascular origin for the lesion. The computed tomographic and standard magnetic resonance imaging scans showed a C5-C6 contrast-enhancing lesion responsible for bony erosion. Subsequent, magnetic reson
