Highest brain bismuth levels and neuropathology are adjacent to fenestrated blood vessels in mouse brain after intraperitoneal dosing of bismuth subnitrate.

A small fraction of humans ingesting bismuth (Bi)-containing medications develops neurotoxicity in which neuropsychiatric signs precede motor dysfunction. Large ip doses of Bi subnitrate (BSN) produce similar signs in mice, but little is known about the pathogenesis of neurotoxicity in either species. Adult female Swiss-Webster mice received a neurotoxic dose (2500 mg/kg ip) of BSN. Bi distribution and neuropathology were determined as follows: (1) Regions of central and peripheral nervous system were assayed for Bi by atomic absorption spectrometry (AAS) 28 days after dosing, (2) regional bra

Pathogenicity of Campylobacter pylori--a causative factor in gastritis?

There is now much worldwide evidence that Campylobacter pylori plays a pathogenic role in the aetiology of gastritis rather than colonizing an already inflamed gastric mucosa. 1) There is a very close association between the presence of C. pylori on the gastric mucosa and histologically confirmed Type B chronic active gastritis. Ninety percent of patients with C. pylori infection have gastritis whereas less than 5% of patients with normal mucosa are colonized. 2) C. pylori only colonizes gastric type mucosa; it is not found colonizing intestinal type mucosa in the stomach. 3) Two volunteer stu

Sandifer syndrome reconsidered.

Three children with Sandifer syndrome are described. One patient was at first erroneously diagnosed as having neurological disease; the two others had true neurological damage, which led initially to misinterpretation of their bizarre dystonic features. Awareness of this entity will spare such children needless investigations and suffering, while giving them the benefit of proper treatment.

Regulation of 1,25 (OH)2D synthesis in hypoparathyroidism and pseudohypoparathyroidism.

We examined the regulation of 1,25-dihydroxyvitamin D [1,25(OH)2D] synthesis in patients with hypoparathyroidism (n = 5) and pseudohypoparathyroidism (n = 5) by administration of parathyroid extract (PTE) and N6,O2-dibutyryladenosine 3',5'-cyclic monophosphate (dbcAMP) and by phosphorus deprivation with antacids. In response to PTE, patients with hypoparathyroidism increased serum 1,25(OH)2D from 17 +/- 5 to 30 +/- 5 (SD) pg/ml (P less than 0.01). An approximate doubling of the 1,25(OH)2D concentration also occurred following dbcAMP infusion or phosphorus deprivation (serum phosphorus 4.4 +/- …

[Medical management of reflux esophagitis].

When the cause of oesophagitis has been discovered, patients for whom surgical management is impossible may be successfully treated by improving oesophageal peristalsis, increasing the tone of the LES, and speeding up the emptying of the stomach. The best results are obtained in peptic oesophagitis, since the latest secretion-inhibiting drugs ensure constant reduction of the acidity of the gastric juice, as has been demonstrated by continuous pH monitoring.

Urinary alkalisation for symptomatic uncomplicated urinary tract infection in women.

BACKGROUND: Uncomplicated urinary tract infection (UTI) is the most common bacterial infection in women, characterised by dysuria and urinary frequency. Urinary alkalisers are widely used in some countries for the symptomatic treatment of uncomplicated UTI, and they are recommended in some national formularies. However, there is a lack of empirical evidence to support their use for UTI and some healthcare guidelines advise against their use. OBJECTIVES: We aimed to look at the benefits and harms of the use of urinary alkalisers for the treatment of uncomplicated UTIs in adult women. SEARCH M

Efficacy of acid suppression therapy in gastroesophageal reflux disease-related chronic laryngitis.

BACKGROUND: This research aims to assess the response to acid suppression therapy in gastroesophageal reflux disease (GERD)-related chronic laryngitis (CL). METHODS: Data were extracted from Web of Knowledge, Embase, and PubMed for English language article published up to March 2016. Pooled overall response rate (ORR) rates were evaluated to determine acid suppression treatment efficacy. Random effects model was used with standard approaches to sensitivity analysis, quality assessment, heterogeneity, and exploration of publication bias. RESULTS: Pooled data from 21 reports (N = 2864, antiref

A review of drug therapy for functional dyspepsia.

The management of functional dyspepsia (FD) is a challenge for gastroenterologists in clinical practice. The eradication of Helicobacter pylori (H. pylori) and the utility of antacids, prokinetics and antidepressants are recommended as treatment choices for FD by consensus. Unlike in Europe and the USA, H. pylori eradication in Asia can lead to a higher proportion of FD patients with symptom relief and a higher symptom response rate to prokinetics. Moreover, response rates to anti-secretory drugs such as proton pump inhibitors deviate considerably from those in the Western world. Digestive enz…

Chronic duodenitis in Zaria, northern Nigeria.

Thirty-four consecutive patients presenting to the Endoscopy clinic of the Ahmadu Bello University Teaching Hospital, Zaria with symptoms of upper abdominal pain were investigated for chronic duodenitis by endoscopy and histology and for associated diseases. Twenty-two patients had histological evidence of chronic duodenitis. No clinical features separated these from those who had no duodenitis. Endoscopy was a good enough tool for diagnosis. Stool parasites were significantly commoner in those with histological duodenitis. Gastritis was also commoner in those with duodenitis. Ranitidine appea

Gastroesophageal reflux: clinical diagnosis, current therapy, future trends.

In the usual clinical setting, symptomatic gastroesophageal reflux can be equated with heartburn; however, the diagnosis of gastroesophageal reflux disease (GERD) can be obscure. Recent improvements in the quality of fiberoptic endoscopy along with other imaging and diagnostic techniques have permitted a more complete understanding of the pathophysiology of gastroesophageal reflux. The continued development of antisecretory, prokinetic, and mucosal protective agents allows the gastroenterologist a choice of effective therapeutic approaches to deal with contributing factors such as gastric acid

The effect of an oral morning dose of nizatidine and ranitidine on gastric acid secretion in duodenal ulcer patients.

The aim of this study was to compare the inhibitory effect and duration of action of a new H2-antagonist, nizatidine (150 mg per os), with ranitidine (150 mg per os) in a group of 10 patients with healed duodenal ulcer. The total inhibitory effect on acid output observed under basal conditions (30 min), during pentagastrin stimulation (120 min) and after stimulation (60 min), is overlapping. However, nizatidine showed a more rapid inhibitory activity -80%, and 31% higher than ranitidine with regard to basal acid output and maximum acid output, during the first hour of stimulation. On the other
