Pre-admission antibiotics for suspected cases of meningococcal disease.

BACKGROUND: Meningococcal disease begins suddenly and death can follow within hours. Pre-admission antibiotic therapy aims to prevent delay in starting therapy that occurs if bacterial confirmation is sought before instituting therapy. OBJECTIVES: To study the effectiveness and safety of pre-admission antibiotics versus no pre-admission antibiotics or placebo and of different pre-admission antibiotic regimens in decreasing mortality and morbidity in people suspected of meningococcal disease. SEARCH STRATEGY: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochra

Etiology of bacteraemia in patients with various malignancies: is there association between certain antineoplastic drugs and microorganism?

The authors studied a relationship between particular bacterial or fungal organisms isolated from blood cultures and type of malignancy and antineoplastic drugs in 237 cancer patients. Sixty four had acute myelogenous leukemia (AML), 43 non-Hodgkin's lymphoma (NHL) and 140 solid tumors (ST). All patients had at least one positive blood cultures for one or more microorganism drawn during 1-10 days after cytotoxic chemotherapy, viridans streptococcal bacteremia was more frequently observed in patients with AML (12.5%) and NHL (27.9%) than ST (43%, p < 0.01 and 0.03). The incidence of anaerobic b

Significant antibiotic use variations in testicular torsion surgery among us free-standing children's hospitals.

PURPOSE: There is no robust evidence for the efficacy of intraoperative antibiotics in preventing infections in testicular torsion surgery, the incidence of which is reported to be <0.2 %. In the absence of clear guidelines regarding the appropriate use of antibiotic prophylaxis, the factors influencing these differences in practice patterns remain unclear. Our study aimed to identify both patient and institution-level factors affecting whether a pediatric patient undergoing emergency testicular detorsion (orchiopexy or orchiectomy) will receive antibiotic prophylaxis. MATERIALS AND METHODS: …

Value of prophylactic antibiotics for invasive dental procedures unclear.

DATA SOURCES: The Cochrane Oral Health Group's Trials Register, the Cochrane Central Register of Controlled Trials (CENTRAL), Medline, Embas … s could be seen. No data were found on other outcomes. CONCLUSIONS: There remains no evidence that antibiotic prophylaxis is either effective or ineffective against bacterial endocarditis in people considered at risk who are about to undergo an invasive dental procedure. It is not clear whether the potential harms and costs of penicillin administration outweigh any ben …

Use of anthrax vaccine in response to terrorism: supplemental recommendations of the Advisory Committee on Immunization Practices.

In December 2000, the Advisory Committee on Immunization Practices (ACIP) released its recommendations for using anthrax vaccine in the United States. Because of recent terrorist attacks involving the intentional exposure of U.S. civilians to Bacillus anthracis spores and concerns that the current anthrax vaccine supply is limited, ACIP developed supplemental recommendations on using anthrax vaccine in response to terrorism. These recommendations supplement the previous ACIP statement in three areas: use of anthrax vaccine for pre-exposure vaccination in the U.S. civilian population, the preve

Practical modalities for prevention of fungal infections in cancer patients.

Invasive fungal infections have become a major obstacle to the treatment of patients with malignancies. Candida spp. and Aspergillus spp. now rank among the ten most prominent pathogens in these patients. Currently, there are no adequate means of detecting these infections at an early stage, and optimal hygiene and elimination of well-known sources of infection remain the most important preventive measures. Due to the lack of reliable, randomized studies, the role of antifungal drugs in the prevention of invasive fungal infections is difficult to judge. The clinical impact of the older oral an

RISK FACTORS FOR SURGICAL WOUND INFECTION AFTER ELECTIVE LAPAROSCOPIC CHOLECYSTECTOMY.

AIM: One of the ways to avoid infection after surgical procedures is through antibiotic prophylaxis. This occurs in cholecystectomies with certain risk factors for infection. However, some guidelines suggest the use of antibiotic prophylaxis for all cholecystectomies, although current evidence does not indicate any advantage of this practice in the absence of risk factors. This study aimed to evaluate the incidence of wound infection after elective laparoscopic cholecystectomies and the use of antibiotic prophylaxis in these procedures. METHODS: This is a retrospective study of 439 patients w…

Effectiveness of Quinolone Prophylaxis in Pediatric Acute Leukemia and Hematopoietic Stem Cell Transplantation: A Systematic Review and Meta-analysis.

The effectiveness of quinolone prophylaxis in high-risk hematological pediatric patients is controversial. A systematic review was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, including studies that involved children and young adults undergoing chemotherapy for acute leukemia or hematopoietic stem cell transplantation (HSCT) who received quinolone prophylaxis compared with no prophylaxis. A meta-analysis was performed on bloodstream infections and neutropenic fever. Data regarding the impact of prophylaxis on overall survival, antibiotic

Survey of prophylactic antibiotic use amongst UK cochlear implant surgeons.

Cochlear implant patients are at an increased risk of pneumococcal meningitis. Recent government guidelines require all implant patients to undergo pneumococcal vaccination. The guidelines also suggest antibiotic prophylaxis but no clear guidelines regarding which antibiotic to use or for how long were issued. We asked each implant centre within the UK to describe their antibiotic protocol for cochlear implantation. Our results have showed that 100% of UK implant surgeons use antibiotic prophylaxis. The type of antibiotic and duration vary significantly between centres. Interestingly, however, t…

[The use of the antibiotic cefoxitin (Mefoxin) in gynecological practice].

The aim of this presentation is to study the advantages and disadvantages of perioperative prophylaxis with the antibiotic Mefoxin against the classical postoperative antibiotic prophylaxis with penicillin and gentamicine, as well as to analyse the place of this antibiotic in modern treatment of different forms of pelvic inflammatory disease. The authors conclude that the qualities of Mefoxin (high resistant wide spectrum antibiotic, covering aerobes and anaerobes), make it an ideal antibiotic for perioperative prophylaxis in gynaecologic and oncologic surgery; the clinical effectiveness of Me…

Incidence of infective endocarditis in England, 2000-13: a secular trend, interrupted time-series analysis.

BACKGROUND: Antibiotic prophylaxis given before invasive dental procedures in patients at risk of developing infective endocarditis has historically been the focus of infective endocarditis prevention. Recent changes in antibiotic prophylaxis guidelines in the USA and Europe have substantially reduced the number of patients for whom antibiotic prophylaxis is recommended. In the UK, guidelines from the National Institute for Health and Clinical Excellence (NICE) recommended complete cessation of antibiotic prophylaxis for prevention of infective endocarditis in March, 2008. We aimed to investig…

A review of the factors influencing antimicrobial prescribing.

There are multiple benefits of appropriate antimicrobial prescribing: it has a direct impact on clinical outcomes, avoids adverse effects, is cost effective and, perhaps most importantly, it helps to prevent the emergence of resistance. However, any physician can prescribe antibiotics, which is not the case with other clinically relevant drugs. There is great variability in the prescribing physician's (PP) training, motivation, workload and setting, including accessibility to infectious diseases consultants and/or diagnostic techniques, and therefore there is a high risk of inappropriate presc

Prophylactic antibiotics to reduce morbidity and mortality in neonates with umbilical artery catheters.

BACKGROUND: Umbilical artery catheters are often used in unwell neonates. Infection related to the use of these catheters may cause significant morbidity and mortality. The use of prophylactic antibiotics has been advocated for all newborns with umbilical artery catheters in order to reduce the risk of colonisation and acquired infection. Countering this is the possibility that harm, such as the emergence of antibiotic resistant organisms, may outweigh benefit. OBJECTIVES: The primary objective was to assess whether prophylactic antibiotics reduce mortality and morbidity in neonates with umbi
