A 79-year-old man developed the classical signs of exfoliation syndrome bilaterally ten to 13 years after bilateral intracapsular cataract surgery. There is, therefore, at least one type of exfoliation syndrome where the lens does not produce the exfoliative material.

Ex-Press miniature implant surgery (Ex-Press implantation) is a new alternative anti-glaucoma surgery. It is clinically important to clarify its indications. Ex-Press implantation has been indicated appropriate in primary open-angle glaucoma and should not be used in narrow angle-closure glaucoma and in high risk of failed scarring cases. More evidence-based studies are still needed to convince the pros and cons in other conditions including aphakia or pseudo-phakia glaucoma, secondary open-angle glaucoma and refractory glaucoma.

CASE REPORT: We report two aphakic patients with Brown-McLean syndrome.
DISCUSSION: One patient was affected by Marfan syndrome, after having undergone lens subluxation surgery and aphakia 23 years previously. The other patient was aphakic due to cataract surgery with complications three years before. Our cases demonstrate that this syndrome can show a variety of clinical characteristics, but peripheral corneal edema is always present. A full understanding of the clinical signs of presentation is of great importance in order to detect this syndrome.

This article presents a case of bilateral posterior dislocations of the crystalline lens in a patient with epilepsy who presented with reduced vision and anisocoria 2 weeks after having sustained head injuries during a seizure. The possibility of lens dislocation was raised only at this time, and subsequently confirmed on computed tomography. Such patients may require prompt referral to the ophthalmologist to treat complications such as functional aphakia, uveitis and more seriously pupillary block glaucoma. Endoscopic cyclophotocoagulation utilizes a diode laser to ablate ciliary body epithelium, resulting in decreased intraocular pressure. Preoperative risk factors that may influence the prognosis in penetrating keratoplasty were statistically analyzed on 698 consecutive eye that had been operated on at the University of Tokyo Hospital. In many experimental situations one has to work with suitable optical corrections for ocular elements which have been eliminated (lens in aphakia, for example). Analysis of the optics of the total system (eye plus correction and other optical elements) shows substantial changes in the positions of the cardinal points.
