[The possibility of the occurrence of hyperventilation apnea and loss of consciousness].

In work influence of various temporary modes of free hyperventilation of the lungs was investigated, usually used by the sportsmen before diving on reduction of the contents of CO2 in exhaling air of healthy men-divers. Attention was paid to the opportunity of occurrence of posthyperventilation apnea and loss of consciousness. It is shown, that washing away CO2 from organism at hyperventilation is limited. Hyperventilation (their depth and frequency was chosen arbitrary), spent during 60, 120, 180, 360 seconds causes in healthy trained men apnea and loss of consciousness.

[Morbidity and mortality in sleep apnea and nocturnal bradyarrhythmia].

The as yet unanswered question, whether patients with sleep apnoea and apnoea-associated bradyarrhythmias have a higher morbidity and mortality rate, was retrospectively investigated in 132 men with sleep apnoea (apnoea index > 10/h). Sleep-associated bradycardic arrhythmias were recorded in 71 (mean age 50.1 years; group 1). For comparison served 61 men with sleep apnoea but no bradyarrhythmias (mean age 51.4 years; group 2), matched for age and weight. There was no significant difference between the two groups as to diagnosis and initial findings on ergometry, lung function tests and blood g

Possible involvement of serotonin in obstructive apnea of the newborn.

Experiments were conducted on newborn rats in order to study the serotonergic modulation of the central respiratory activity during the neonatal period. In vitro experiments performed on isolated brainstem spinal cord preparations demonstrated that serotonin exerts a permanent excitatory modulation on the central respiratory rhythm generator but depresses the inspiratory motor output to the genioglossus muscle. In vivo experiments confirmed that increasing endogenous serotonin levels decreases the inspiratory activity of the genioglossus and elicits drastic obstructive apneas.

[Arrhythmia and syndrome of obstructive sleep apnea in adults].

Many nocturnal cardiac arrhythmias and conduction defects have been reported in the adult sleep apnoea syndrome. The most original is the great variability of the heart rate which is cyclical and related to the apnoeic episodes, and easily differentiated from simple respiratory sinus arrhythmia. It is characterised by an initial bradycardia followed by rebound tachycardia. The bradycardia is vagally dependent (inhibited by atropine) probably secondary to carotid chemoreceptor stimulation by the hypoxaemia. The tachycardia is mainly attributed to the cessation of vagal hypertonicity although ca

Diagnosis and pathophysiology of obstructive sleep apnea in children.

Factors that decrease pharyngeal size or increase pharyngeal compliance increase an individual's risk for OSA. In children, these factors include craniofacial disorders, central nervous system problems, or simply adenotonsillar hypertrophy. Although children with OSA may present with nocturnal symptoms of snoring, respiratory pauses, secondary enuresis, and/or nightmares, their initial complaint may be poor school performance, hyperactivity, or other symptoms of sleep fragmentation. The diagnosis is confirmed by overnight polysomnography in a laboratory with expertise in the care of infants an

The effect of vertical pulsating stimulation on apnea of prematurity.

Placing preterm infants suffering idiopathic apnea of prematurity on the VPS had an effect on the infants' respiratory effort and achieved a reduction in the number of apneic episodes secondary to central and mixed apnea. However, VPS offered no benefits in the reduction of obstructive apnea in this study population. Because central apnea has been reported as the predominant type of apnea and VPS is a nontoxic, noninvasive, and easy-to-implement method of alleviating central and mixed apnea types, it seems prudent to give VPS which has the stimulus characteristics to preterm infants experienci…

Assessment of uvulopalatopharyngoplasty for the treatment of sleep apnea syndrome.

Uvulopalatopharyngoplasty (UPPP) consists in the surgical removal of the uvula, part of the muscular portion of the soft palate and redundant palatal and pillar mucosa, and the tonsils. Since 1981, UPPP has been proposed for the treatment of sleep apnea syndrome. Polysomnographic studies have shown that in about half of the patients submitted to UPPP there is a 50% or greater reduction in apnea index. Attempts to identify presurgically those patients more likely to benefit from UPPP have yielded inconsistent results. Limited retrospective follow-up data suggest that UPPP does not modify the in…

Central sleep apnea and arterial compression of the medulla.

We report a 5-year-old with central sleep apnea associated with compression of the medulla oblongata by abnormal looping of the left vertebral artery. The magnetic resonance imaging findings raise the possibility that compression of the respiratory center by an aberrant vertebral artery might cause central sleep apnea.

Response of obstructive sleep apnea to fluoxetine and protriptyline.

Protripyline is the pharmacologic agent most commonly used to treat obstructive sleep apnea (OSA); however, its anticholinergic side effects make it intolerable to many patients. Because serotonin may be a central respiratory stimulant and because the serotonin-uptake inhibitor, fluoxetine, is usually well tolerated, we wanted to try fluoxetine in the treatment of OSA. Therefore, we compared the effect of fluoxetine to that of protriptyline in 12 patients with OSA. Both drugs significantly decreased the proportion of REM sleep time and decreased the number of apneas or hypopneas in NREM sleep.…

[A case of sleep apnea syndrome with variant angina].

A 56-year-old male was admitted because of respiration arrest during sleep, and precordial crushing sensation which repeatedly occurred early in the morning. He had been hypertensive and aware of daytime sleepiness for ten years. After admission, all night polysomnography was recorded a total of four times. Apnea index was 37.5 times/hour, and central type apnea was predominant. The diagnosis of sleep apnea syndrome was made. In the early morning of the fourteenth day after admission, the patient developed anterior chest pain associated with ST elevation in leads II, III, and aVF of the electr…

Pneumogram recordings in infants resuscitated for apnea of infancy.

Infants who sustained a spell of apnea of infancy during which they were resuscitated have been shown to be at increased risk for sudden infant death syndrome. To determine the value of the pneumogram as a predictor of outcome in this population, the first pneumogram obtained of 51 such infants was analyzed. The infants were grouped on the basis of subsequent outcome during a course of monitoring: group 1--infants who died during a subsequent event; group 2--infants who received resuscitation or vigorous stimulation to terminate a subsequent spell; group 3--infants who survived and did not hav…

Sleep apnea in children without hypertrophy of the tonsils.

Two 12-year-old boys with severe sleep apnea syndrome but normal-sized tonsils were satisfactorily corrected by tonsillectomy and uvulopalatopharyngoplasty. One of the boys had muscular hypotony as contributing cause of the condition. The other had mandibular hypoplasia in combination with a long soft palate. Thorough preoperative anamnesis and examination in a sleep laboratory are necessary to determine which cases will benefit from surgery in spite of normal-sized tonsils and which surgical procedure will be most helpful.

[Role of ORL in the treatment of sleep apnea syndromes].

Over the past few years, the criteria defining the sleep apnea syndrome have been well delineated. This syndrome is a common affliction whose pathophysiology is usually indicative of an obstacle in the upper respiratory tract. The authors describe seven mostly serious cases, with cardiovascular and hemodynamic repercussions, that benefited from ORL surgery (uvulo-palato-pharyngoplasty and/or tonsillotomy). In each case, the consequences of this simple surgery were excellent, resulting in a sharp attenuation of the symptoms or their disappearance. In conclusion, the authors insist on the prepon…

[Apnea test essential in the diagnosis of brain death].

We experienced 11 cases of brain death for the past two years, in six of whom we performed the apnea test to confirm the cessation of the medullary respiratory functions. The cause of brain death was primary intracranial lesions in four, subarachnoidal hemorrhages in three and meningitis in one. Hypoxia of the brain secondary to cardiac arrest resulted brain death in the remaining two cases. Blood gases were analysed before (control) and after pre-oxygenation, after having adjusted PaCO2 around 40 mmHg, and every three minutes after disconnection from the respirator. Blood pressure and other v…

[Apneas and periodic breathing in mature newborn infants].

50 healthy term newborn infants without a history of sudden infant death syndrome among their relationship were investigated between the second and fourth day of life using a modified test of Brady and McCann. In a thermoneutral environment pneumograms (respiration by thoracic impendance, heart rate, tcpO2) were obtained with an FiO2 of 21, 16, and 30% oxygen. In 12 of the 50 sleeping infants (24%) apnea periods or periodic breathing were observed with an FiO2 of 16%. This test will bring up abnormalities in the regulation of breathing. Similarities to the SIDS are discussed.

Acute oxygen in patients with sleep apnea and COPD.

Nocturnal oxygen administered to patients with disordered breathing ameliorates hypoxemia. As a result, an important chemical stimulus to arousal is diminished. This could cause prolongation of disordered breathing events, worsen respiratory acidosis, and induce potentially harmful cardiac arrhythmias. The presence of chronic obstructive pulmonary disease (COPD) could further aggravate the situation since such patients may have depressed hypercarbic responses. To test this hypothesis, 20 obese men with sleep apnea and COPD were studied polysomnographically on two nights receiving air on one or…
